I can’t say enough great things about my experience with Dr Riolo and her staff. They are considerate, kind and wonderful. They explain everything to you before doing any treatment or procedure. I love just being in the office it is a wonderful experience.

Q: Avoiding Vertical Breast Lift Scar?

A: For some women there is a new option for a breast lift WITHOUT a vertical scar and WITH the upper fullness usually requiring an implant!

Bellesoma Breast Lift is for women who are satisfied with their overall volume, (or perhaps feel they are somewhat too big) but are sagging too low.  You know who you are - you are always saying "I like my breasts if only they were up where they used to be"!!!

Bellesoma takes the breast tissue from the nipple down to the fold under the breast, and shapes it into a cone.  Straps of extra skin from the area are turned into "shoelaces" that are laced in and out of the pec muscle to hold the breast "where it used to be".  The skin from the collarbone to the top of the nipple is used to cover the cone and the nipple is brought out through a round opening.  That creates only the scar under the breast, and the scar encircling the nipple.  No vertical scar.

More importantly the breast is full at the top as if it were in a push up bra.  While it will not work for everyone, it is a wonderful option for those with sufficient volume and sufficient excess length on the breast. Invented by Dr. Horndesky in Houston, it is now being taught to other plastic surgeons across the country.  You will be hearing about it more often in the future as it produces a remarkable result.

Q: Donor Area - Will it grow back as thick?

A: Dear "C", Congratulations on your decision to proceed with NeoGraft FUE hair transplant.  From our experience we believe you will be very pleased with your results.

The procedure really involves a redistribution of the hair population on your head, a "Rob Peter to pay Paul, so to speak."  So, no, the hair in the donor area will not be as thick as it is now.  As surgeons, we evaluate the density of the donor area to be sure it is thick enough that the hair density decrease will not be noticeable.  But a good rule of thumb is that  you can lose 60% of the density without it showing. The hair on the back of the head tends to lay like shingles on a roof, decreasing the visibility of the change. 

In exchange for that, the hair in the grafted area will increase in density.  In our practice we find a very very high rate of satisfaction with that trade off.

A: Dear Popcorn
I know how frustrating it is to watch something on your face heal slowly.  On the inside the tissues are still swollen and repair is ongoing.  It will take 6 to 12 months to see the final result.  I have seen many situations like this work out just fine in the end.  But if you are still concerned about the appearance at 6 months or greater, see a Board Certified Plastic Surgeon who can discuss scar revision with you.

A: Dear Real Self Reader,

Congratulations on choosing to do something nice for yourself.  It is easy for us to get so caught up in caring for the rest of the family that we neglect ourselves.  And kudos to your husband for his support!

Of course, recovery times will vary among individuals, but here is what I tell my patients who have chosen to undergo the procedures you have picked.  It is very much like a C section; the tummy rules the recovery; and the first three days are miserable!

Let's break it down.  Just like your C sections, you have something nice to show for the discomfort you are going through.  In fact, this time, it won't wake you up every three hours all night! 

You don't think of how much you use your core until it hurts.  So getting up and down from a chair, or turning over in bed are a big deal at first.  While your pecs will be sore from your breast surgery, it is much easier to avoid those upper body activities that cause discomfort. 

Most importantly, just like with child birth, its better when you know what to expect.  So I don't want to scare you about the first few days, but rather reassure you it is normal.  You just can't get comfortable.  You don't feel like eating much, and you have gas that builds up.  You will probably be in a girdle or garment of some type that is uncomfortable.  You walk hunched over so now your back hurts!  Its not any one thing, but rather the accumulation of a lot of annoyances that cause the misery.  The pain medicine and muscle relaxants will get you through.  The key is to go with the flow on those meds, and not try to be too tough at first.  After a few days you will be able to taper off those drugs in the daytime, saving them for when you are trying to fall asleep.  It is realistic to plan that your husband or some other wonderful individual will manage the house and the kids, while you take care of yourself.

A: Dear Real Self Reader,

A tummy tuck is a great way to deal with the changes you see in your belly, including the vertical scar that causes the indentation in the center.  Most of the time, all the skin from the belly button down to your C section scar can be removed.  It is possible to combine the tummy tuck with a hysterectomy.  The chance of complications such as a wound infection rise a little bit, but that risk is worth it to some women to combine two recovery periods into one.  Your plastic surgeon and your gynecologist just have to be willing to schedule the surgery together, which is not quite as efficient as when they each work alone.  I have coordinated with the gynecologists in my area in all sorts of ways, including with both laparoscopic and open hysterectomies.  The best approach is to have a consultation with a member of each specialty until you find doctors who are comfortable working with one another.
