Dr. Gregory A. Buford, MD, FACS is a board-certified plastic surgeon with two decades of experience, based in Denver, Colorado. He is an exceptionally skilled Voluma, Juvéderm, Botox, and Kybella Master Injector who trains other injectors across the nation. Additionally, he is renowned for his expertise in breast enhancement surgery, having performed more than 6,000 surgeries on clients from around the world. Q: Am I a candidate for a lollipop/short-scar breast reduction? (Photos)

A: First, be wary of a Plastic Surgeon who says that they "never" do a specific technique and always do it one way.  Everyone is different and so different approaches and techniques may be necessary for different people.  

I personally use the anchor technique quite often because it reduces the distance from the nipple to the fold and creates (in my opinion) a much more stable breast lift than with a lollipop incision alone.  That being said, I used to do the lollipop method almost exclusively but have changed my preference over the years.  

In your case, I would definitely do the anchor method and would also think about using mesh as an internal bra to help better support the lower pole of your breasts.

Q: Will Botox injections before a temporal brow lift affect my results in a negative way?

A: As the previous physicians mentioned, this is actually a good thing and may potentially even improve results of a brow lift (especially with injections in the forehead itself).  I would, however, consult with your physician that is actually doing the surgery and work with him/her to get the most optimal results.

Q: Is it possible to get bigger inner calves with Botox to treat false curvature?

A: The simple answer here is...no.  BOTOX (and the other neuromodulators) work by effectively blocking the effect of neurotransmitters on the recipient muscle and so they will actually help to reduce muscle activity and potentially muscle mass.

Q: Tear trough or mid cheek filler? (Photos)

A: Thanks for a great question!  

I will politely disagree with the previous PS and suggest using an off-the-shelf filler to address your concerns.  While fat is a very nice filler, no type of grafting procedure can assure 100% take and so this can be a little unpredictable.  Also, in the tear trough area you have to be very careful with lumpiness.

I would personally begin by injecting your cheeks.  Your mid face is flattened and would benefit from volume in this area instead of the tear troughs.  I would use 1 syringe of Voluma XC/cheek to assure a more optimal result.  And while fillers like Voluma are not permanent, they do last up to 1.5 years and provide a very natural looking result.

Q: I’m a circus flyer (aerial silks) and I’m looking to get breast implants, would you suggest under or over the muscle?

A: As the previous Plastic Surgeons so eloquently described, there are advantages and disadvantages to placement of breast implants beneath or over the muscle.  

I personally work with a large number of very fit women with very little body fat and usually a minimal amount of breast tissue.  As such, I generally prefer the sub muscular approach. This approach tends to produce a much more natural result because of the overlying soft tissue as compared to placing implants over the muscle.  The disadvantage is that when you are activating the muscle, the implants tend to "dance" or animate which some patients do not prefer.  However, in the long run my personal preference is strongly towards the sub pectoral approach and I feel that this would be a good recommendation for you.

On that note, I actually operated on a patient in a similar profession this past year and placed the implants under the muscle.  She is extremely happy and has returned to full function with a very nice result!
