Q: Local Anesthesia Toxicity?

A: In the interest of patient safety AND comfort, my preference is to have an anesthesia provider present. Usually this is general anesthesia, but it could also be monitored intravenous sedation. In both instances local anesthesia is also used. Personally I would not want to be wide awake during the hours it takes to do a proper facelift, and would want my surgeon to focus on the surgery and someone else to focus their attention to keeping me safe and comfortable.

Q: Wanting to have mommy makeover done in stages for best results - which procedure should I start with?

A: Unless there is a specific reason to stage the breast surgery and tummy tuck, they are almost always done together. Factors that could change that are if you are having circumferential lipo with the tummy tuck, which takes extra time, or if the plan exceeds 6 hours. If you do stage them, do the tummy tuck first because there could be some traction that might affect the level of the inframammary fold.

A: Although I can't see the upper abdominal skin in the photo, it does show that your belly button is rather high. For that reason, a mini tummy tuck with umbi float is probably your best option. In your case, the belly button would be re-attached a bit lower, which would allow for more skin removal in the lower abdomen than could be done with a mini only. A full tummy tuck is too risky.

A: Since you have lost upper pole fullness, your breasts need more support than the skin is providing. Consider an internal bra with Galaflex. Removing more skin without adding support is less predictable.

A: Since the breast shape is pretty good when the muscle is relaxed but forms a groove when you flex your pec, you have animation deformity. This is due to the detached portion of the pectoral muscle adhering to the capsule, right where you see the groove. IMO this is not correctable with fat transfers. It requires re-attachment of the muscle behind the implant, which can be done by converting from dual plane to split muscle. Many examples on my website.
