Facial aging often follows a pattern that involves both descent of soft tissue as well as loss of volume. While it is not common to undergo a facelift in your late 30s or early 40s, it is not unheard of. Ultimately the key to any successful surgery is making sure that the procedure offered will improve what bothers you. From these photos I notice that you have excess skin of your upper and lower eyelids as well as moderate bags under your lower eyelids. You also have a moderate nasolabial fold and loss of cheek volume. The distance between your lower eyelid and cheek also appears to be slightly long which is a sign of facial descent.

In my opinion you would be a great candidate for both and upper and lower eyelid blepharoplasty. If you were my patient I would give you the option of fat transfer to naturally enhance areas of your face that appear slightly sunken versus a facelift. I do not believe age alone is a reason to not offer these procedures.

While I cannot tell exactly what is exposed from these photographs, cartilage exposure is not a "normal" outcome following rhinoplasty. If this is causing discomfort and you do not like the way it appears then it is reasonable to consider a revision surgery to remove the exposed cartilage and restructure the nasal tip.

A: Hi and thank you for providing photographs with your question.

From your pictures it appears your nasal bones at the top of your nose were left too wide after your first surgery. This is why the top of your nose appears wider than the middle portion. This can be corrected with a revision rhinoplasty procedure and if done well will improve your nasal shape and appearance.

A: Hi and thank you for providing photographs with your question.

Swelling after a rhinoplasty takes approximately 1 year to fully resolve. However at 5 to 6 months post op most of your swelling is likely gone. The irregularities that you see will likely not change over the next 6 months to 1 year.

It is important to realize that it is not a good idea to re-operate for at least 1 year since your previous surgery. Do not rush into a revision procedure. If a surgeon recommends intervening sooner than 1 year I would be cautious of that plan.

Overall you have a pretty nose. I agree that your nasal bones are wide and will better fit the mid portion of your nose after being narrowed. I also notice that your tip is "bulbous" or rounded which can also be improved with a rhinoplasty procedure. Creating a balance between the upper third (nasal bones), middle third, and lower third (nasal tip) is key to any aesthetic nasal surgery.
