Q: I’m not sure what size implant I should get and if I should get Saline or Silicone implants - advice?

A: The positives for saline are lower-cost. The negatives are increased firmness, more prominent rippling, and most significant is their tendency to completely deflate after rupture, demanding additional surgery.

The positives for silicone are their more natural softness and appearance, leakage does not result in a deflation therefore it does not demand secondary surgery. The negative is the additional cost of the silicone implant.

In my practice. I use approximately 98% silicone gel prostheses

A: This is usually technique related. Deep fascia suturing is the most important stitch placed. However some patients can form weak scar tissue that may contribute to this suboptimal result.

Q: In order for you to take off fat how much dose a persons weight have to be

A: My patients are told to be at their normal/stable body weight. A measure of height to weight ratio is also considered. A BMI (body mass index) over 30 should be scrutinized closely. Limits for fat volume removed are present. If your requests cannot be met then you do not want to pay for the surgery but  feel unhappy with your outcome.

Q: My left implant heads towards my armpit when I lay on my back and it feels like it's floating when I bend over or use arm.

A: There are many causes of pocket asymmetry. Your case needs to be evaluated thoroughly with an examination. Revisions are very common for a multitude of reasons but it appears that your breasts were asymmetric. Capsule repair with suturing and or synthetic mesh is an option. Another possibility would be to create a new pocket over your muscle. Your case is unfortunate but not uncommon. 

Regarding preoperative information, revisions are common. It should be explained prior to surgery that they may be necessary and how they will be financed. You have purchased a service. Every potential complication cannot be explained in minute detail but general issues should be covered. In my practice we discuss general outcomes and results. If I can fix a complication I do so within 1 year of surgery. If it was a technical error on my part then I cover the expense. Most reputable surgeons would rather you be satisfied than unhappy.
