    I rarely recommend extracting teeth prior to orthodontic treatment but in this case the crowding was so severe that we extracted four second molars, not bicuspids. Extracting second molars allows wisdom teeth to erupt in the second molars' position so there is no collapse of the patient's face, no collapse of lip support and no damage to the patient's airway or TMJ.
