Dr. Kotlus is a superlative and outstanding Oculoplastic Surgeon. In the Spring of 2023, I got lower eyelid fat pad belpharoplasty and fat repositioning by Dr. Kotlus. Truly satisfied with my results, I decided last Spring of 2024 to go back to Dr. Kotlus to have him perform Asian double-eyelid surgery, upper eyelid blepharoplasty, and blepharotosis repair for my droopy eyelids. I am completely and thoroughly satisfied with my results. I initially came across Dr. Kotlus's website and reviewed several dozens of reviews by his patients on Google and Real Self and was very impressed by his specialized credentials and overwhelming patient satisfaction. After a very detailed and informative consultation with Dr. Kotlus, I decided to trust him with my ocular area appearance, because of his highly specialized credentials and surgical expertise in oculoplastic surgery. I believe he is also one of the few Oculoplastic surgeons in Manhattan who is certified by the American Society of Ophthalmic Plastic and Reconstructive Surgery. He also takes a conservative approach in performing procedures which eliminated any unwanted surprises in my results. He has amazing surgical hands and I was completely confident and assured while he performed these procedures of his proficiency. Finally, his follow-up consultations for a couple of months after the procedure were very important and most helpful. He doesn't just take your money and "abandon" you after so to speak. It's been 1 1/2 years since my blepharoplasty (eye bags, not eyelid surgery. Realself does not give you the right options). It still looks amazing. The man is an artist. He gave me back decades. I looked better at 54 than I did at 36. Highly recommend Dr. Kotlus. There is no pressure. His consultations are comfortable and reassuring. He makes you look natural. The entire experience was kind of magical. I will return to him when it's time again. I researched many doctors for months. He's the best.
Dr. Kotlus is outstanding. If you are considering eye surgery with him, don’t hesitate! At the initial consultation, he was kind, answered all of my questions and was thoughtful in his recommendations and advice. The surgical suite is off site from his office, but is state of the art and has very competent, professional staff. The anesthesiologist he works with is also outstanding. The procedure went very well and I had no pain after the surgery. Dr. Kotlus was available by email and phone for anything I needed after the surgery. Always available to answer questions or address concerns. I love my results! Procedure: upper and lower Bleph, fat transfer, CO2 laser to under eyes area.
48 y/o female that had lower blepharoplasty with fat transfer and laser.... plus a minor canthopexy to address minor drooping of lower outer eyelid. I cannot say enough great things about Dr. Kotlus. First and foremost he is board certified, safe, sets realistic expectations, and has follow up appointments to ensure things are healing according to plan. His staff members are kind, warm, organized and pleasant. Absolutely wonderful staff. The lower bleph was the most straight forward and lowest risk part of the treatment plan. I also respond very well to laser so I was confident that would turn out well. However, I did a ton of research and thinking about the fat transfer bec/ (1) the reabsorption rate varies widely per patient and (2) if you overfill you are screwed. NOTE: I had tear trough filler twice with another doctor that eventually looked great, however, (1) I swelled for a month both times and (2) it isn't permanent. I had another consult with another top surgeon who prefers filler versus fat transfer because it's more controllable (less absorption variation per patient). After a lot of thinking I decided on Dr. Kotlus with the fat transfer because (1) we agreed less is more... better to underfill and look like it wasn't done then over fill and have caterpillar face, (2) I swell with filler, and (3) the fat transfer is more permanent. I had the stitches removed after 5 days. If I didn't have the canthopexy I would have looked totally normal within 7-8 days. I had very minor swelling, very minor bruising. It was the canthopexy healing and scars that took a bit longer to heal/fade. I was worried the canthopexy might make my eyes smaller and rounded... they look freaking awesome. Back to my almond shaped eyes. I am off the charts thrilled with my results. Everything went as expected. I can't say enough great things about Dr. Kotlus... and I will be seeing him in 3-5 years for an upper bleph.
Wonderful, precise, kind surgeon! Dr. Kotlus did a fantastic job with upper and lower blepharoplasty on my 53 year old eyelids. The results are youthful but natural. I had absolutely no pain post surgery and the recovery was fairly fast.
Very happy with the results of my upper and lower bleph with fat transfer. I barely bruised, which was I guess rare but awesome surprise! Only a couple of weeks after surgery friends and family said they couldn’t tell I had anything done - but said I looked great (and skinny!). When I showed them before and after they were shocked at the results! I would recommend Dr. Kotlus to anyone looking into a bleph as he also specializes in this area - and one of the main reasons I chose him. I didn’t trust a general plastic with something as important as my eyes!

In this situation a lateral canthoplasty may be a reasonable option. You should have an in-person consultation with an oculoplastic surgeon for the best advice.
The answer depends on your situation but most surgeons will advise you don't do these procedures at the same time. Primarily because each can cause dryness of the eye surface. With lasik, some of the corneal nerves can be temporarily injured, reducing your motivation to blink and distribute tears over the surface of the eye. It's not worth the risk of causing a problem that could potentially be avoided by doing these at separate times, preferably at least 6 months apart.
Hi,If you treat the glabella (the 11 lines) only, the forehead should not worsen, in fact it often gets better because the eyebrows lift slightly, eliminating the drive to raise the brows by tightening the forehead (frontalis muscle). Some people exhibit something called muscle recruitment when the eyelid closing muscles try to take over for the glabella muscles that are not working as well. But this doesnt impact the forehead.
It's difficult to tell from your photo but the most common cause of eye position asymmetry is a difference in the position of the orbit which is the bone around the eye. If there is a skeletal asymmetry (very common) the eye height will appear different as will the hollowness around the eyes. There are some treatments to disguise the asymmetry (e.g. fillers) but the treatment for skeletal asymmetry may not be appropriate for everyone.
The best remedy for your eye bags depends on the underlying cause. It could be allergy, prominent fat pads, or other issues that are best determined when you have an in-person consultation with a cosmetic eyelid specialist physician. Based on your photo it is difficult to determine the cause.
