A rhinoplasty and blepharoplasty can be performed at the same time. It is usually preferable to perform a lip lift secondarily and not at the same time as the rhinoplasty since the lip lift incision is made along the base of the nose. We most commonly will perform a lip lift under local anesthesia, so there isn't an anesthesiologist cost associated with that procedure. See a board certified plastic surgeon who is experienced in these procedures in your area for a consultation and to discuss further.
Your concern about the tummy tuck scar is a common one. Based on your pictures, your loose skin shown will only be improved by a tummy tuck. This requires an incision which we place low down beneath revealing clothing and try to keep it as short as we realistically can. After your loose muscles are tightened during the procedure and loose skin removed, we carefully close the incision with multiple layers of absorbable sutures to allow the incision to heal so it is as unnoticeable as possible. We have the patient start using scar cream or tape beginning 3 weeks postop to help the scar heal quickly and well. All of these things we do will not eliminate a scar, but by keeping the scar as low as possible, as short as possible and closed with multiple layers of stitches to keep the scar from spreading, a tummy tuck scar in our hands is usually a positive trade off for the markedly achieved contour it affords.
Best advice is to return back to your surgeon for a re evaluation. I will commonly thin out the subcutaneous tissue in the abdomen and mons during a tummy tuck procedure. In fact, we call the procedure a lipoabdominoplasty for this reason. As the swelling subsides, the improved contour in these areas will become apparent. Not all surgeons will perform liposuction at the same time as a tummy tuck, however. It is worthwhile to have this discussion with your surgeon to see if liposuction was performed at the time of the tummy tuck as well as if they think anything else is needed.
Based on your history and pictures, you should benefit from a tummy tuck. Given that you have never been pregnant, you may be able to avoid rectus abdominus muscle tightening. That would indeed shorten and make your recovery easier. See a board certified plastic surgeon near you for a consultation to discuss further. They can examine you and clarify the need to rectus abdominus muscle repair. Best to review their before and after pictures on their website to try to evaluate their results and give you a comfort level before proceeding to surgery.
Based on your history and photographs, a tummy tuck should improve your appearance and likely your back pain by tightening your loose rectus abdominus muscles, also known as diastasis recti.
