I appreciate your thoughts on being proactive and forgoing too much time on fillers. That being said, I do think they have their place along with fat transfer to restore volume and maintain even facelift results. That aside, I believe the deep plane face and neck lift is the most natural and durable option that addresses the true reasons for facial aging by releasing the tissues and not applying any tension on closure. A midline neck lift would also be an excellent option for you, which involves a small incision under the chin and sometimes small incisions behind the ear to primarily address the neck and also the jawline. If the Xomed microdebrider was used for the turbinate reduction, then the majority of what it accomplishes is removing erectile soft tissue within the turbinate that prevents them from swelling and becoming as large as before. There is a very thin bone within the turbinate as well, which can be also be minimized with this device in the process but that is not the purpose. This bone is typically addressed by out-fracturing the bone gently with another instrument to open up the airway after Xomed is used. I hope this helps answer the question for you.

Great question and every surgeon has somewhat varying opinions on exactly when to stop steroid nasal sprays prior to surgery. Typically, I recommend stopping these 1 month prior to surgery to avoid nasal dryness and bleeding during and after surgery. Flonase can be taken daily for years without any systemic side effects so continuous use is reasonable for treatment of allergies otherwise.

Great question and the truth is there is often a marked difference between 6 months and 12 months post surgery particularly on frontal view with narrowing. Taking standardized photos is the best way to fully appreciate these changes and can typically be done during each visit with your surgeon. The nostrils can definitely be made MORE symmetric, but perfect symmetry is something nobody has in reality. By straightening the nose and the septum inside, these procedures alone will greatly benefit nostril symmetry on frontal view. Other maneuvers are also able to be performed to help with this as well and can still give you a very natural appearing result.

First of all, you have great facial structure overall. When I look at the features you mention, it is my opinion that chin augmentation would offer a very nice balance to your profile and also give the perception of less nasal projection. As far as the tip of the nose having asymmetry, it’s difficult for me to assess that based on these photos. Buccal fat can be a great defining procedure for the face, but if done needs to be conservative especially for a young patient like you so as not to take away facial volume that will serve you well down the road. I think the best option would be to start with a couple rounds of steroid injections to see if that offers improvement before undergoing a revision for that issue alone. The surgeon who performed the facelift should be able to perform those or another specialist. Ultimately, elevating skin and thinning that area surgically would also be an option and would be something I perform in office. Hope this helps answer your question.

Looking at your pictures, it's a subtle bump more notable from left than right and slightly visible on frontal view. Trauma can definitely cause this even if the bones aren't broken through callous formation at site of injury. A closed rhinoplasty with incisions inside the nose and minimal downtime could be performed. A small amount of rasping to the bone and possible osteotomies to narrow the bones a little would be best surgical plan. It is certainly able to be addressed, but ultimately up to you if a surgical intervention is worth it. I hope this helps.

This is a great question and there's a lot of different answers for small changes to the nose. I don't know your history, but any type of fillers in the nose can (not always) cause changes over time secondary to swelling, migration, or just resorption. If you've never had fillers, then day-to-day changes can result from salt intake, allergies, inflammation, or really anything that causes swelling to the soft tissues and skin in the area. Long-term changes to the nose occur secondary to an increase or decrease in sebaceous glands as well as gravity, which draws the nasal tip down over time and can accentuate a subtle dorsal hump to a more noticeable one in some cases. A rhinoplasty (nose job) can help make some of the changes more predictable, but many will just occur over time unless the underlying causes are treated. I hope this helps in answering your question.
