I recently had the privilege of being under the care of Dr. Pafford, and I can confidently say that she is a remarkable plastic surgeon who deserves extreme recognition. I contacted Quintessa to get a consultation for neck lipo and Buccal fat removal and apon seeing my facial injury from a dog bite Dr pafford was happy to evaluate it on the spot for me. I was told by the hospital plastic surgeons at the time of the injury it could not be fixed, and I called multiple independent plastic surgery practices over the years who all told me they do not do any trauma reconstruction. She gave me options and I gave her the trust to fix it. I’m getting married next year and thought I would need the photographer to edit it out of every photo, but now I’m confident I will feel beautiful in my wedding photos, without any need for photo editing to my previous injury. To describe Dr. Pafford, I’d say she has more of a type C personality with her work In the way that her extensive knowledge in the field is evident, she has great attention to detail and gives an absolutely beautiful end result, but what really sets her apart is she also has exceptional bedside manner. She has a nurturing quality that is rare to find sometimes in this field. As we spoke i became more curious about her and she was happy to tell me stories about her life. I found out during her service in the Air Force, she served as a reconstructive surgeon, specializing in assisting soldiers whose lives had been permanently altered as a result of their dedication to our country. This background shows in the care and respect she gives her patients. Having had a previous experience with a plastic surgeon for a breast augmentation, who treated me more like a project than a person, I can't emphasize enough how refreshing it was to be treated by Dr. Pafford. Her and her team went above and beyond to ensure that my needs are met promptly and efficiently. Whether it's answering questions, addressing concerns, or providing support, they were always there, ready to assist. They treated me like family. During my time with Dr. Pafford I felt like I was her only client. It's clear that she approaches every patient with the same level of dedication and care, making each one feel special and valued. Dr Pafford performed two procedures for me. One was a chin lift and those results are amazing. Everything was done for my comfort both physically and mentally. Dr Pafford was there to explain everything as we went. The second procedure was Ellecorr. I was skeptical that the results would be as advertised but I couldn’t be happier with my jaw line and neck tightness. Great care was taken to make any pain to a minimum. Wonderful staff!
The wonderful Dr. Butz removed my implants for me in March 2022. Wonderful experience with local anesthesia only. Never got them redone, just removed and I am very happy. In February 2024, Dr. Pafford performed an upper blepharoplasty on me, and I’m very happy with the results! I’m still healing but I’m looking exactly the way I wanted to look! She is intelligent and has a wonderful way of making you comfortable. My peripheral vision also dramatically improved. This place is absolutely wonderful and the people are kind and knowledgeable.

I recently had a consultation with Dr. Pafford in February and couldn't speak more highly of her. She was patient, genuinely kind, answered all of my questions, really took her time and I never felt rushed or pressured to make a decision on the spot. Looking at your last picture I would recommend lower lid resurfacing with erbium laser over a lower lid skin pinch. It tightens skin, builds collagen, and would help with some of the deeper lines heading towards the cheek. There is a little more down time with the erbium laser lower lid resurfacing compared to a lower lid skin pinch, but I think you would get amazing results base off the posted picture.
These procedures are often done all together so it may balance your look all at the same time. If you haven't found a surgeon you like that does both then you could keep looking or if you are determined to stick with these two physicians you could split them up. The area around the eye starts aging first so I would usually recommend starting there. It's also one of the best surgeries to make you look less tired and just overall more refreshed.
I usually recommend my patients dissolve the filler under their eyes and in their cheeks if they are having a lower lid blepharoplasty (that's more than a skin pinch) and or midface lift. The first reason is I usually do periorbital fat grafting with my lower blepharoplasty. The second is because whenever you are lifting tissue, like you would in a midface lift, you are moving tissue back to a normal position but also tailoring height and final placement to the current anatomy of your face. Fillers aren't stable or permanent so I wouldn't want them distorting my surgical judgement and potentially affecting your final outcome.
It's hard to see your under eye bags in this photo. You could consider reposting with additional pictures from the sides and also from the front while looking upwards with your eyes only (not your head) to get a better evaluation. If you have had filler near the eyes before, it can sometimes be filler that is breaking down. When this happens it attracts water and can give the appearance of bags under the eyes. Otherwise the bags are usually caused by prolapsed fat pads around the eyes. In younger patients this tends to be genetic. The type of surgery to address both your upper lids and your lower lids is called a blepharoplasty. I'd recommend finding a local Facial Plastic Surgeon or Oculoplastic Surgeon to discuss your surgical options.
What I see in your pictures is residual prolapsed fat in the lateral fat pad. This fat pad is the most common area to see incomplete removal after surgery. I recommend you discuss your concerns with your surgeon if you haven't already spoken to them. They can do an in person exam and discuss your options if you aren't entirely happy with the result.
