I would start with glycolic acid and hydroquinone, but some procedures could also be needed.   Chemical peels and possibly fractional ablative laser may be required as well.   See your board-certified dermatologist for appropriate in person guidance.

Coolsculpting selectively targets the fat that is "pinched up" into the applicator.   Organs cannot be "pinched up" into the applicator, so they are unaffected.   The temperature/ time required for coolsculpting success selectively targets fat and is not cold or long enough to destroy the veins.   

Some patients who are very consistent in their Botox injections will not require as much Botox over time due to the muscle atrophy.   Other patients may experience dose-creep over time due to autoantibodies.  In the second scenario, they require higher and higher doses to get the same effect and these same patients may require more frequent injections as well.  Everyone is different.  You can ask your doctor to try to lower your doses and see if you get the same result-- you can always put more in if needed!
