Both a loose screw and a loose implant have similar clinical characteristics .... the crown restoration moves.  To determine which is which, the best thing to do is see your implant dentist.  The dentist will take an x-ray to look at the bone health around the implant.  The dentist may also need to disassemble the crown to figure it out.  

To correct the tooth midline to face midline problem, braces is the best and potentially only solution.  This is because the tooth root as it is position now is directly at the midline.  There is no veneer that can correct this  

The fee range for a lower all on four bridge is between $10,000 to $15,000 depending upon the number of implants.  

Bonding alone will make your teeth too wide for how tall they are.  It would look aweful.   It is possible that you may have too much gum tissue covering your teeth.  In that case a gum lift followed by bonding would be ok.  You need to diagnose whether the gum line is in the correct position or whether you have “altered passive eruption”.  Also, consider no prep porcelain veneers over bonding.  

Closing this space with bonding or veneers will make the teeth look too wide.  Use braces to pull the teeth together
