Due to Ductal Carcinoma inSitu, I opted for a double mastectomy with DIEP flap reconstruction. Dr Pisano and team performed the 10 hour surgery. Removed all breast tissue, sparing the skin and nipples. They removed tissue from my abdomen, giving me a ‘tummy tuck’ in the process. Every step was explained to me prior to surgery. I stayed in the hospital 2 days, walking the morning after surgery. They have a FANTASTIC recovery protocol which controlled my pain so well I never took a narcotic! The folks at PRMA in San Antonio TX are absolutely THE BEST!!! ( one of my greatest fears was waking up with nothing on my chest and feeling deformed). Thanks to PRMA I had the reconstruction at the same time as the mastectomy so this fear was completely eliminated! I am now cancer free, have a ‘new’ bustline as well as a flatter tummy. As I am expecting my very first grandson, this all means more to me than words can express!!! Dr. Pisano is very knowledgeable and an artist in his field. He thoroughly explained all the options available and answered questions. I felt very confident about my surgery despite my very complicated case. The hospital staff that Dr. Pisano works with at Methodist Hospital are very good. They work as a team to give you seamless care. I followed all the instructions before and after surgery. PRMA has the flap procedures down to a science. I was healing well then back up and going before I knew it. The ability to recover without narcotics and anesthesia hangover is well planned out. Dr Pisano is up for any breast reconstruction challenge. I was a huge challenge! His final aesthetics are tops in his field. I am blessed to have him as my doctor. You will not be disappointed!

I'm a breast cancer survivor and had the DIEP Flap and have zero regrets. By the grace of God I found Dr Pisano and his team and appreciate everything they did to help me through my journey. My breast look better than before and I don't have ugly scars. Yes I have a tummy scar but that is not a reminder that I had breast cancer. Thank you Dr Pisano for taking the time to explain all my options and your genuine care for my well being. I share my story so that other women know the importance of finding a great doctor and to find out all their options before they have a mastectomy.
My GAP flap involved: prolonged surgery (7+ hours), prolonged sedation(7+hours), damage to nerves, chronic leg pain, persistent seroma, ambulatory impairment, disfigurement, and muscle removal. For Expander to Implant attempt I had to return to surgery within 12 hours with internal bleeding, was prescribed cyclobenzaprine, instead of antibiotics, which interactes with another medication I was on, eventual expander removal due to uncontrolled infection. Cannot recommend at the present time.
Great question! At PRMA Plastic Surgery, we routinely perform DIEP flap breast reconstruction for out of state (and even out of country) patients. We typically have the patients stay in town for about two weeks after the surgery. This way we can monitor the healing process and remove most of the drains.
