You should really discuss all your options with someone who is comfortable with all aspects of bulgy eyes and eyelids (Oculoplastic and orbital surgeon). It is always difficult to base a judgement on single photos but you have upper eyelid ptosis which is causing your eyebrows to elevate and give a hollow appearance. In addition you have relatively prominent eyes. The concern is that if you address bulging eyes it will make the appearance of hollowness above your eyelids worse. There are ways to show you these options in a consult

To answer your question would require more information.  Orbital surgery can damage or disrupt the nerves or muscles that open the eyelids or move the eye. Time is usually a good start but depending on the circumstances this can be improved. It is best to get a consult with an orbital surgeon (in person or video)

These types of scars are especially difficult because the biology of bone removal increases the scarring. I will not get into the reasoning and issues that led to the problem but at this point decreasing the biologic scar response is critical and creating a slide plane between the skin and bone. This may require several surgical and non surgical interventions to gradually improve. Using biologic agents to reduce scarring and expand the tissue planes is the first step. I have used these successfully for 15 years and there is hope :)

You have mentioned each of the important variables that must be considered on BOTH orbits and surrounding tissue to create more symmetry.  Your question requires a thoughtful consultation and review of your scans but the idea would be to create a customized solution to give symmetry. IT is critical to consider the eye height, eye prominence, contour and position of eyelids (upper and lower), periorbital fat and brow and cheek position .  This is a lot to consider but also means there are many options.   You will need to consult with an oculoplastic surgeon that has done hundreds of these cases like ours.

There are several options for improving prominent eyes. Filler or volume can be placed around the eyes and to the midface etc or one can move the eyes back.  Moving the eyes back can coke with risks but our group has done this thousands of times.

There are many causes of bulging eyes but when they occur without thyroid disease it is often a cosmetic problem but can also cause dryness.  There are several options form moving the eyes back to camouflaging the prominence.  We specialize in moving the eyes back and reducing the bulging but this is very specialized and you should seek an opinion from an orbital and oculoplastic surgeon who has done hundreds of these cases.  Downside can be double vision but in the right hands it is very low. 


Fat decompression may have fewer risks but in our hands the rate of complication is very low with all forms since we have done thousands of these procedures.  However, decompression is just one method and it is best to have a complete evaluation and a customized approach. Many times non surgical methods are also available.  We discuss all options on consultation either skype or in person before recommending anything.

Some people have very prominent eyes. Often this is caused by a medical condition called graves’ disease or thyroid eye disease.  But often it is due to trauma or just being born that way.  This can be quite concerning for many people in that it makes them appear intense or not normal.  Many patients ask if there is anything that can be done and in the past the answer was not too much at least safely

Now that is untrue. We specialize in moving the eyes back or improving this bulging appearance for patients. This is specialized surgery that very few doctors would offer but our group has done thousands of these surgeries and our patients are very happy. In fact most find it life changing as you can find out from our patients Patients, come to us looking to improve their appearance if their eyes are bulging.  Not everyone has bulging eyes so it is important to customize the process.  If someone has bulging eyes then we can consider several options including a surgery to reduce the bulging. To reduce eye bulging we will remove fat or shave a little bone to reduce the prominence.  This surgery is done under general anesthesia and takes about an hour and patients recover in about two weeks with bruising and swelling

this can often be treated in a variety of surgical or even nonsurgical ways. We do This type of reconstruction commonly and the biggest issue is trying to do to much eyelid or soft tissue surgery to hide bony problems. This can result in complications and problems. It is hard to see from your picture but I think you could get a nice realistic improvement. It will be important to see an oxuloplastic surgeon familiar with facial and orbital trauma

Dr. Douglas, the best - highly recommend for orbital/oculoplastic surgery

As a physician myself, I highly recommend Dr. Raymond Douglas to anyone who is considering orbital surgery/occuloplastic surgery. I had orbital decompression and my results are amazing. I did my research and found who I consider to be the best. When you are considering having a procedure where the stakes are this high, it’s important to do your homework. You do not want your appearance altered by someone who does the procedure you are seeking a few times yearly. You want someone that does it so often that the younger generation of physicians seek out his expertise. You want someone with so much experience that they can handle the most difficult case, even if your case is straight forward. You want someone where you can rest assured that you’ll never need a revision. Make sure your physician is board certified by the appropriate boards and make sure his/her training was done at a reputable University Program. Dr. Raymond Douglas is the best of the best and I am eternally grateful for his expertise and his kindness to me. Look no further, he is not only skilled, but also kind, compassionate, caring, accommodating, and just a warm, lovely person in general. His office staff, especially Diana, were wonderful as well and easily accessible to me throughout the entire process. Overall, an amazing experience. I would recommend him without reservation.

(Orbital Decompression) Best Decision Ever! Dr. Douglas Positively Improved my Appearance and Mental Health!

Dr. Douglas is a god sent angel. After orbital decompression surgery with him, I started to get so many compliments on my eyes, nothing else about me but my eyes! It positively changed my appearance as well as my perspective on life. I owe him a good review here! I was born with very protruding eyes and shallow orbits and got bullied for them since I was child. I never heard of orbital decompression surgery until one day I was on Youtube and decided to give it a try. I consulted 2 doctors after scouring the internet for best doctors and decided to go with Dr. Douglas for many many reasons. I live in a different country so we did a remote video consult and decided to push my eyes back for 5mm to 6mm and we had a few photo mockup back and forth and Dr. Douglas was patient and kind at all times. and then I flew to LA to do the surgery and I was nervous before the surgery. I'm sure anyone would be nervous when they go behind your eyes to operate and your eyes are the most precious thing you don't want to injure or lose. But rest assured, you'll be in best hands with Dr. Douglas. He was so kind, caring and attentive to details every step of the way. The first 2 days of recovery can be tough, but since then I have been all good, never had any vision problems or discomfort. This has been the best decision I've made for myself. I stopped putting on heavy eye makeups to hide the protrusion of my eyes. Most I do now is just putting on false eyelashes and I always get compliments from people I just met and others who knew I got the surgery also told me it was a good decision and that my appearance drastically improved even though it was only a few millimeter changes. As for the price, Dr. Douglas might be more expensive than other doctors but he's well worth it. I've never met any other plastic surgeon this kind, caring and amazing. During my recovery I lived in an Airbnb and the host happened to know of a friend who's in need of a surgery like this and been doing research and she told me what I paid is reasonable given it's Beverly Hills and Dr. Douglas is the best. Bulky Ear Cartilage Grafts Gone and No More Eyelid Webbing

Below are my prior surgeries and what initially started my long journey. The reason I had surgery with Dr. Guy Massry and Dr. Raymond Douglas was to correct round eyes with outer webbing and I really hated the bulky ear cartilage in my lower lids. I've very pleased with the results. I do have some asymmetry and slight retraction in my left eye but have no regrets about this surgery. It was totally worth it. Hopefully it will last for years. December 2004 Went to a Plastic Surgeon Surgery Upper and Lower Blephoplasty with the incision on the outside of my lower eyelids. (Ask me why 95 percent of the time you should not get cut under the eye on the outside) February 2005 Went to an Ocular Facial Plastic Surgeon Surgery Canthopexy on both eyes (Ask me why 95 percent of the time you should not get a canthopexy to treat lower eyelid retractions) Reason: Eyelid retraction January 2010 Went to an Ocular Facial Plastic Surgeon Surgery Thread lift to hold cheeks in place, ear cartilage grafts in the lower eyelids, and canthoplasty. (Ask me why a Thread lift is not a good idea and why to never get ear cartilage as an under the eye graft) Reason: Eyelid retraction November 2017 Went an Ocular Facial Plastic Surgeon - same Doctor as the last Surgery Lateral canthal lengthening on right eye with 1 tarsal bite from the periosteum Reason: Right outer corner eye bump and corner slightly pulling apart with webbing. December 4, 2020 I had surgery with both doctors below. Dr. Guy Massry & Dr. Raymond Douglas - LA Dr Douglas performed orbital decompression. By pushing the eyelids back it lengthen the canthal tendon and provided more skin under the eyes, which I needed. Dr. Massry then removed my spacer grafts, performed a mid face lift, lengthen canthal tendons and corrected outer corners of my eyes.

Dr. Douglas made me look like myself again

I could not recommend Dr. Douglas more highly. I was diagnosed with Graves Disease and Thyroid Eye Disease almost 2 years ago. I was lucky enough to participate in the Teprotumumab clinical trial for patients with TED that Dr. Douglas was involved with. His knowledge and experience with TED is impressive. I had seen a couple of doctors prior to seeing him and neither of them possessed the knowledge and confidence for the treatment of TED that that he did. I actually felt hopeless after leaving their offices but after I left Dr. Douglas’ office I knew I was going to get through this with positive results. The trial drug helped me so much but I did require minor surgery after the trial was finished. Dr. Douglas made me look like myself again. His work far exceeded my expectations and I couldn’t be happier. I finally feel like myself again. I also need to mention his bedside manner. Dr. Douglas understands what his patients are going through and the hopelessness and isolation that is associated with TED. That meant as much to me as his knowledge as I was going through one of the worst times of my life and no one seemed to fully acknowledge or understand my struggle. Thank you Dr. Douglas. Thank you for caring and for all the wonderful work you do for people who are in need of your expertise.

My experience with Dr. Raymond Douglas was amazing. I went for a consultation with Dr. Douglas for a surgery I have been researching (eye decompression/eyelid retraction for possibly having TED or Graves) and have felt hopeless, embarrassed and have lost all self-esteem since having these eye issues. I can barely make eye contact with people anymore as I notice them noticing. I have lost all confidence and feel extremely insecure. Thankfully the RN, office operations people and Dr. Douglas, all put me at such ease. I couldn't have asked for more. I Had a Very Bad Experience

Because of the incorrect information provided by Raymond Douglas before the surgery, I made the wrong decision. He did mention the risk but also provide wrong information about the result. After the surgery, he still didn't acknowledge some information about what he said was wrong , and during post-operative consultations, his explanations varied. Don't naively believe that a doctor with mostly positive reviews is necessarily a good person, because he probably has other patients who might have had negative experiences but are hesitant to leave bad reviews due to potential legal risks (something happened which makes me very careful to use words, but I am not going to keep quiet). Objectively, he may have certain skills and credentials(Maybe? I don't know), but his attitude towards patients is not right, which may not result in good outcomes for you. If you decide to see him, make sure to scrutinize his statements carefully and verify them from multiple sources. This is my best attempt to give you objective advice based on my personal experience. And I am telling you having this doctor perform my surgery is the decision I regret the most in my life.
