As a patient with significant existing breast tissue and breast ptosis (droop), a breast implant will simply give you a slightly bigger version of what you have. This is because your breast implant is placed underneath the breast tissue you already have. As you can imagine, your natural breasts will be on top of whatever implant is inserted, so the change in size and shape will be very limited. The actual aesthetics of your breasts will only change very slightly if you have just an implant without a lift. If you are wanting to address any aspects relating to shape, symmetry, cleavage, or upper pole, you will not be able to achieve this without a breast lift as your natural breast tissue will always sit on top of your implants. It is likely that any surgeon who has consulted with you is declining to perform a standalone breast augmentation as they do not believe the aesthetic changes are worth the risks involved with invasive surgery.
It's important to obtain an understanding of how breast implant sizing and bra sizing works. There are two issues with asking a question like this. The first issue is that without actually measuring your breasts, seeing the amount of existing tissue you have, and discussing your aesthetic goals, there is no way to determine which implant size is best suited to you, or what kind of size a certain implant type will produce. 315cc on one patient can be very subtle, and on another patient can be very enhanced, it is all dependent on their anatomy. The second issue is that bra sizing is extremely variable. A D cup in one bra brand can be a C or a DD in another brand. As such, we never recommend speaking in terms of cup sizes. A better approach is to bring in inspiration photos to your plastic surgeon. Photos of results you like can help communicate your expectations to your plastic surgeon more clearly. This also provides a more specific view of which aspects you prefer whether it be cleavage, upper-pole fullness etc.
A breast lift is required to address the shape of your breasts and the asymmetry as you have stated that these are two of your concerns. To understand what "full" means to you it's important to communicate your expectations clearly with your plastic surgeon. You should bring in images of results that you like and are aiming for to help your surgeon understand the size that you deem to be "full". Once you have done this, your plastic surgeon will be able to recommend the implant size for you, or may not even suggest an implant at all - depending on your goals.
If you have a desired size then you and your surgeon should choose the implant with a goal to achieve this size. Fat transfer is unpredictable and often requires multiple treatments. Generally, fat transfer in tandem with breast implants is used as a way to create closer cleavage or soften the look of implants to be more "natural". It is not a reliable way to increase size for most patients, unless they have abundant financial and time resources to have multiple procedures. Of course, this is a generalised answer and there would be many questions your plastic surgeon would want to ask you to determine the best plan for you.
