Excellent question. As you have astutely observed, the "ptosis" only occurs when you are talking or smiling - meaning when you activate your facial muscles. At rest, you do not notice a problem. Fillers are great to correct volume deficiencies, but neurotoxins such as Botox or Dysport are the way to go for dynamic issues like the one you point out. I would recommend a tiny dose of Botox (2-4 units max) to help. There is some evidence to suggest that doing this will lengthen the chin a bit - but with a small dose of Botox, the weakening effect is less, and will likely give you the result you're looking for. The good news is, if you don't like it, it'll wear off in a few months.

However, if you have decided that you want to change things, one additional option that could be considered is submental liposuction. I accomplish this with three tiny incisions - one under the chin, and one behind the jaw under the ears on each side. This can help tighten up that jawline to add more definition.
