Thank you for your question. It appears from your photos that you have mild brow asymmetry and deeper set eyes. In that setting, sometimes the lower positioned brow can start to hood over the eyelid area, especially in the morning or when we are fatigued. It does not appear that you need upper eyelid surgery or a brow lift operation at this time. You may be a good candidate for what is called a "chemical brow lift" usually accomplished with Botox, Dysport, or other Neuromodulator. The nice thing about that approach is that small asymmetries can often be corrected with the amount of toxin that is injected into the muscle groups to give you a more even brow appearance, and lifted! Keep in mind that effect only lasts about 3 months, and then the injection pattern would have to be repeated to renew/maintain the effect. Your next steps should be to seek consultation with a board certified doctor who is a facial specialist that is experienced in this procedure. Best wishes!

Thank you for your question. It appears that you have mild asymmetry in the upper eyelid fold, with slightly more redundant skin on the right than the left. The presence of an injury to an eyelid or eyebrow area can cause tethering or differences in fullness and fluid (lymphatic) flow around an eye, making that side appear more full or heavy. In this setting, it is worth seeking in person consultation with a board certified surgeon that specializes in eyelid surgery and can identify your nuanced condition. Since your brows appear to be in symmetric position and your upper eyelids seem to sit at the same height or level on the eye, you may be a good candidate for a conservative right upper eyelid blepharoplasty (limited skin removal only, not remove fat) to equalize the amount of "eyelid show" closer to the lash line (lowest part of upper lid). Why is one eye drooping?

Thank you for your question. It appears you may have mild brow asymmetry with the right brow sitting lower than the left. This could be due to different reasons, but sleep position (sleeping mostly on one side) can affect our facial soft tissues leading to some drop or drooping. Mostly we see this in later decades, but depends on the person. Also, even though one side may appear lower, it could be the opposite side (upper eyelid) that is higher that is causing the asymmetry. Sounds like you feel the left eye/eyelid/brow are in normal position for your face, but do keep that in mind. Sometimes a more prominent eye on one side (different reasons for that happening) can push the upper eyelid up more and the lower eyelid down more, giving a larger eye appearance. One cause of this could be a thyroid imbalance. There are a variety of different reasons that may cause eyelid and brow asymmetry, so best to get this checked out to know more. For your lower eyelid appearance, your photos suggest mild hollows or volume loss in what we call the tear trough area. They may not be actual eyebags (meaning extra tissue), but rather a hollow or groove giving that appearance. This can often safely and very effectively be corrected with hyaluronic acid (HA) dermal filler, such as Restylane-L. Injected in limited amounts, HA can provide a very natural and rested appearance and be long lasting. May be worth investigating for you. I recommend you seek consultation with a board certified surgeon who specializes in the eyes and surrounding structures (eyelids, brows, etc.). Best wishes to you!

Thank you for your question. It appears you have mild Ptosis (pronounced like Tosis) of the left upper eyelid, where the lid sits just a little lower on the eye surface than the opposite side. This is very common, especially when there is a small difference of about 1-2mm, like your eyelids. This can be corrected by a conservative upper eyelid muscle tightening, and there are generally two different ways of doing this - front of the eyelid or back of the eyelid approach. That will usually correct the issue long term. Please note that a skin removal only operation will not accomplish your goal. Another option would be injections of Botox or Dysport, which can also be used to make subtle changes to eyelid height, but have to be injected in very specific locations to achieve a symmetric result, and only last about 3 months in most people (would then have to be repeated to renew or maintain effect). Lastly there is a relatively new eyedrop medication on the market called Upneeq that can be used to very temporarily lift your eyelid height a small amount, but would have to be used daily to maintain the effect. Most people use it more for temporary effect before social or work commitments, important video calls, etc., not as a long term solution. I have included a link below for more information on that medication. You would need a prescription. Thank you for your question. I appreciate your bravery in coming forward with this question and seeking answers, and I am sorry to hear about what you experienced in the past with comments about your natural expressions when you smiled and laughed. Unfortunately people of all ages can be unkind about others' appearance even when the person they have targeted appears completely normal. And "normal" can mean many things, as everyone's body is different, and these differences should be valued. We do sometimes see in our younger patients differences in eyelid shape and function based on genetic or developmental (medical) issues, but I do not see this to be the case for your eyelids. I would not recommend Botox or eyelid surgery to alter your lids at this point in time. One can always consider this as a later option. If you want to know more, I would recommend you seek consultation in person with a board certified surgeon who has experience with eyelid or facial surgery in younger patients to get further guidance for your concerns. Thank you for your question. Neuromodulator injections near the eyes, particularly around the brows before upper eyelid surgery, and in the lower eyelid wrinkles before lower eyelid surgery, can affect how the surgeon visualizes your skin redundancy. This may affect measurements or nuanced expectations of how your tissue will respond based on how smooth or wrinkled it appears at surgery time. If it is still a full 3 months before your planned lower eyelid surgery and if BOTOX has usually worn off for you around 3 months, then you should be ok to proceed with your injections, but if it is less time than that, it may be too close to surgery time. Most importantly, you should consult with your surgeon on any changes to your medications and/or procedures you are having done near your eyes, as it may affect her/his plan to optimize your outcome. Thank you for your question. We don't have a photo available to go by, but based on your description, it may have something to do with the shape of your upper orbital rim (brow bone) combined with how your brow (hairs) have been sculpted over the past 4 years (by plucking). Your sister's brows may be shaped differently based on hereditary factors or the position of her brow hairs (she may be plucking differently). Also keep in mind that brow skin and eyelid skin are different thickness, so you may be seeing a light demarcation line between the two areas of natural skin difference. Any surgery or alteration of the facial bones (such as brow bone surgery) is a fairly major operation. Thank you for your question. Upper eyelid appearance, skin fold, and shape are very much driven by hereditary factors. This appears to be true for your eyelids as well, since you have had extra folding of upper eyelid skin for most of your life, as you described. Since your brows appear to be in normal position, as evidenced by smooth skin at the outer corners of your eyes (no outer hooding) and smooth skin over your upper nose (no skin bunching), I recommend upper eyelid surgery as your best option for reducing the skin excess that you see - specifically a conservative upper eyelid blepharoplasty. The skin incision should be made in the crease line of your upper eyelids to best hide the incision, and I would not recommend any fat removal from your lids (not needed) so as to preserve your youthful appearance. I recommend you seek consultation with a board-certified surgeon who specializes in eyelid and facial surgery. Best wishes!

What is the downtime for Opus Plasma?

Thank you for your question. The downtime after Opus Plasma fractionated resurfacing depends a lot upon your skin care regimen prior to treatment. If you use a hydrating and restorative regimen that reduces risk of hyperpigmentation, and you stop your retinols (my recommendation, some may have you continue through) in advance, then recovery will be faster. A number of MD office-grade skin care lines have excellent post-procedure recovery regimens. Ensure that is offered to you or included in your package to take the guess work out of what you should be using afterwards. A thorough provider will generally have done that research for you, though bear in mind there are a number of skin regimens that could work great, not just one. Assuming you have pre- and post-treatment skin care in place, your social downtime after each Opus treatment (we recommend a package of 2-3 treatments for optimal results) will be about 5 days. We tell everyone less than one week, but it does depend upon how intense a treatment you have done. Within reason, the more energy that is (carefully) applied, the better the final outcome, but the longer the downtime. Usually you can start using a mineral makeup that is less irritating within 3 days, and at least be productive with video calls and work within 1-2 days. This timeline is quite a bit shorter than even fractionated laser resurfacing, so yes, Opus has a shorter downtime per procedure, and then you can repeat about once a month until you have completed your cycle. Seek consultation with a board-certified specialist as you go forward.

Thank you for your question. Yes, Opus Plasma could be a good option for you based on what you described as your primary facial skin concern. Opus fractionated plasma resurfacing is not a laser, as you probably know by know. It is a relatively new and innovative technology that uses electrical energy in a very focused way to create a spark that ignites the oxygen and nitrogen molecules at your skin surface (using a specialized roller). The resulting energy layer that forms at your skin surface is plasma, and when applied in a fractionated form (*new technology), can produce improvement in tone, texture, tightness of skin, improvement of scars, and even improvement in superficial reds and rosacea in some patients. The procedure lightens pigmentation from sun damage, as well. Think of this as a lower downtime procedure than laser resurfacing, but requires more treatments to achieve your goals. We have Opus at our practice, and have generally recommended this as a package of 2-3 treatments to achieve optimal outcomes, even though results and improvement can be seen with even 1-2 treatments. Seek a consultation with a board-certified specialist as you go forward.

Upper Bleph - Dr. Gandhi Is Sheer Awesomeness!

CONSULTATION & PRE-OP: For fear of NOT looking like myself, I’ve always been hesitant to have anything done to my face; however, my eyelids got so droopy that if felt like there was a constant shadow over the top of my eyes and I had a “resting frown.” After a lot of research, I am so glad that I found Dr. Gandhi! He has a great bedside manner. In addition to taking preliminary measurements and providing me with plenty of details about the procedure, he listened with such patience and was very thorough in answering all the questions I had. A big concern for me was that I wanted only removable sutures. Right off the bat, Dr. Gandhi listened to me and agreed to do only removable sutures; additionally, the consultation did NOT feel rushed at all, and it gave me such a high level of confidence that Dr. Gandhi was the right doctor to give me the desired results for my upper bleph. He provided Arnica beforehand, with instructions to start taking pre-op. SURGERY: Surgery date (April 2025) went off without a hitch. I was relaxed and comfortable. Altogether end-to-end was about 3 hrs, but that includes setup, prep (numbing, measurements, etc.), and then surgery itself (local anesthesia). My eyesight was blurry for the first 3 days due to the antibiotic ointment, which I expected. 2 MONTHS POST-OP: I also had dryness, which gradually tapered off in the 1st 2 weeks, to none at 2 months post-op. I woke up quite swollen the next day (I expected that), this gradually subsided by the 2-week mark, and was hardly noticeable 1 month post-op (I was able to attend Mother’s Day with family and got comments on how good I looked, but no one isolated my eyes in particular). My eyelids would still look slightly puffy in the mornings right when I wake up; however, I doubt anyone else noticed – also, this subsided early mornings, after I’d been up for about 2 hrs. I also noticed sensitivity to bright screens (phone, computer), but that gradually improved, I just couldn’t have the screen at its max brightest. There has been NO detriment to my eyesight – I don’t wear glasses/contacts and I could still see are normally as I did pre-op. I still had a slight tightness around the outer corners of my eyes, if I open my eyes wide in a somewhat exaggerated way; however, I am ok with this, since this is wide-eyed expression is NOT standard daily-use – I don’t feel this tightness and my eyes feel “normal” when just going through daily use. 3-MONTHS POST-OP (NOW): My eyelids no longer swell in the mornings, eyesight is still good and no issues with dryness. I love, LOVE my results!! POST-OP AFTER-CARE: Dr. Gandhi’s after-care has been excellent! The post-op instructions were clear, and I had various appointments – starting with immediately after, 2 days post-op, 1 week post-op, 2 weeks post-op, 1 month post-op, 2 months post. Also, when I texted questions along the way, Dr. Gandhi or his staff responded quickly. REALISTIC EXPECTATIONS: Depending on how your skin historically heals, your experience will be unique to your body. I think it’s important to have realistic expectations. Since excess skin is being excised, there is going to be a suture line, which depending on how you heal, you’ll need to tend to. I see people complaining about incision lines extending a bit farther out than the end of their eyelid (a few mm). From what I now see, this is necessary; otherwise, you would have that extra skin bunching at the outside corner of the eye. Also, scars have faded away with the silicone scar sheets I was using. SCARS: My scars tend to be naturally raised, so I know to be on top of it with silicone sheets as soon as the doctor okays it. After suture removal, I applied silicone scar sheets (basic CVS brand) for 1-3 days in a row, then remove to clean and oil the area (vit E oil), after which I’d reapply and repeat. I cut thin slices out of them, just wide enough to fit over the suture line (I did NOT use any scar gel). Closer to the insides of my eyes took a bit longer to flatten. MY THOUGHTS ON RESULTS: This is the best thing I have done for my face!! I absolutely LOVE the end-result! My face looks more youthful. I am SOOOO glad I chose Dr. Gandhi!! He did a fantastic job because: (1) my eyes are evenly matched – same shape/size; (2) my eyes close fully (even immediately post-op); (3) the incision line is matched on both sides and is right inside the crease of my eyelids; (4) he recommended NO fat removal, which I now realize was a great suggestion because my eyes look “normal,” just more open and refreshed; (5) he has integrity and did NOT pile on unnecessary add-ons – he told me I did NOT need an eyebrow lift; (6) his bedside manner is top-notch and leaves nothing to be desired – he is very responsive, kind and friendly, he listens patiently and actually truly hears your concerns – and better still, acts on them; (7) his staff are very friendly and highly responsive (8) his after-care is stellar! (9) No surgery is worth NOT looking like myself, so, most important of all for me is that I still look very much like myself! Dr. Gandhi is sheer awesomeness, and I absolutely highly recommend him!!

Don't Postpone Joy

I wanted an upper bleph for several years now, but as a 36 year old I was cautious that I may be jumping the gun a bit. After doing some research (I 100% wanted an expert in eye plastic surgery - an oculoplastic surgeon - rather than someone who was only doing eye procedures once a week and breast aug every other day), I decided to just go for it and not postpone joy, since I'm sure I would end up doing it in my 40s or 50s. I found Dr. Gandhi here on RealSelf and he has fantastic reviews. I was impressed at my first consultation, rather then just tell me 'Yes, there's enough skin to take off, let's book you now for the procedure' instead he said he'd like to see me in another month after my Botox had completely worn off so that he could get a better baseline. To me, that signaled he was most concerned with making an accurate assessment of my needs rather than rushing to get me on the books - green flag! So I went back for another consultation, he was very thorough measuring and doing a full eye exam (my vision is not obscured but it's good to check overall eye health as well). All interactions leading up to my surgery made me feel comfortable and supported. The procedure itself wasn't exactly pleasant (unsurprisingly!) but Dr. Gandhi and his assistant checked on me constantly so I felt well taken care of. Now for the most important info: I LOVE my results! It's so much easier to put eyeliner on and I no longer have the asymmetry that has bothered me for years. As an aside, during my healing process I had a routine skin check at my Johns Hopkins dermatologist and the derm asked me where I had my eyes done (still had visible stitches). When I told her it was Dr. Gandhi, she said "Oooooh he is very well regarded!" and said my eyes looked great. One more thing, Dr. Gandhi and his assistant never pressured me to buy their skincare products or to get other injectables they offer ('tox, filler, etc.) which is really refreshing considering most medspas absolutely make you feel pressured.

After suffering with unknown eye issues for months I finally found relief with Dr. Gandhi. I had been passed around to numerous Ophthalmologists, Allergists and Oculoplastic Surgeons. But Dr. Gandhi took as much time as I needed to come up with the proper treatment plan. He was the first provider to let me speak and explain my history with an open mind. Healing nicely due to his craftsmanship of putting me back together

My dentist is in the same building at Dr. Gandhi and I passed his door many times. I had had a lipoma removed from my head back in 2020. The biopsy came back Negative for any cancers and I am cancer prone. Just runs in the family. Right above where I had the lipoma removed I had gotten what I though was a Zit. It wouldn't heal and stuff would come out of it once in a while. After two years of fooling with it I decided to pay Dr. Gandhi a visit. He did a biopsy on the area right away. It took about a week for the results to come back and it wasn't good, it was Squamous Cell Carcinoma. He got on my case right away contacting a Mohs Surgeon to take the cancer out. I went to have it removed and the next day met Dr. Gandhi and the Timonium Surgery Center so he could close the wound. Dr. Parag Gandhi of Maryland Eye and Face is the Awesome! He saved my forehead and me from other terrible complications from the Cancer. He is so Caring and he listens to your concerns and answers all your questions. I'm only 3 weeks out healing and am healing nicely due to his craftsmanship of putting me back together again. He is my go to Doctor for anything that has to do with my face. Whether it be more cancer problems in the future or cosmetic procedures such as fillers and any other procedures I may need down the road he's my guy!

The short version of my review: Set up a consultation with Maryland Eye and Face, you won't regret it. The long version: For background, I am a 31 year old, white female that has never had any type of work done (including injectables). I contacted the office for a possible lower eyelid lift/blepharoplasty. I have a very prominent, herniated fat pad under my left eye that has been there since I was a child and I have always been incredibly insecure. Its gotten worse the past few years and I finally decided to get something done about it. The assistant Trish is amazing. She is so kind and efficient at her job. She made me feel welcome and relaxed, and went over a very detailed health history with me. Dr. Gandhi was incredible. He and Trish spent over 2 hours with my at my consult. I never felt rushed or that I was in any way a bother with all of my questions. Dr. Gandhi took the time to explain all my options, and in the end I decided on under eye and cheek fillers. My results are incredible. I would say I look like I'm 20 again, but I actually look better than I ever have. No one is telling me how tired or sick I look all the time. I barely spend time with concealer anymore (so much time/money spent trying to correct my under eye bags...) All in all, I would highly recommend Maryland Eye and Face. I will be going back in the future.
