CLAIM YOUR PROFILEInteresting question, Ascend. You really dont have enough skin to go through a traditional tummy tuck but you do have the mucle separation associated with pregnancy. You will still have the bulge if you don't repair the diastasis recti(DR) and only fix your shelf. One nice option is to do a "modified mini" tummy tuck. This is where the skin is elevated from groin (where the usual incision is) up to right under the rib cage. The lower incision can usually be quite a bit shorter since we arent removing much skin at all. The skin is only elevated centrally as opposed to more to the sides in a traditional tummy tuck. When we reach the belly button, it is separated from the abdominal wall but still attached to the skin of the abdomen. The muscle/DR can then be closed under the skin and then the skin allowed to drape back down. The stalk of the belly button would be re-secured to the abdominal wall, pretty much where it was before. The shelf can be smoothed out during the procedure. This a nice option for very lean people who dont have much extra skin but do have a big abdominal bulge!

By this time( one year out), your rhinoplasty result is pretty close to being completely healed. You should expect your final result to look very similar to what you have posted. If you feel like your tip is up and overly projected( my personal aesthetic agrees with you), you will need another procedure to adjust this. In my hands, I would not be able to do this comfortably under local anesthesia. Best of luck

Hi Silvia, tjhanks for your question. In general it is a good idea to wait at least a year to make a decision to have a revision. Before that time, there is swelling that will cloud the "real picture" of what is happening. I do see what you mean about the tip but this could be the swelling, scar tissue that is developing, and/or thick nasal skin. Interventions include watching and waiting, steroid injection to the scar tissue, or revision surgery. Stay in close contact with your surgeon at the year mark to see what the next steps should be.

Hi Inventive, it is important that you bring up any concerns prior to surgery. Depending on what you discussed with your surgeon previously, adding a dorsal/bridge adjustment may signficantly alter his/her surgical plan or not much at all. For instance, if this was meant to be a purely functional rhinoplasty, they may have to perform surgery in different parts of the nose than originally planned.
The goal is to make this rhinoplasty your last rhinoplasty! If you want to have additional adjustments to your nose, make sure your surgeon knows this before having this procedure to avoid possibly having another surgery in the future unnecessarily

Hi Ms. Mermaid, thanks for the interesting question. I see the tip deviation that you are talking about. Generally, surgical rhinoplasty will be the best and most reliable option for correcting the appearance of the nose. It also does not have to be a tough procedure. An experienced surgeon will be able to make the experience very tolerable.
However, for non-surgical options, depending on the actual defect you have (which is hard to assess with just one photo) it may be possible to fill that defect with some sort of filler, thereby pushing that side of the nasal edge forward and driving the tip back toward your left. You should see an experienced plastic surgeon with injection experience to make sure this procedure is done reliably and safely. The central face has many blood vessels that can be blocked with filler if done improperly.

Hi Tasteful, thanks for the question. I think it brings up an important point about post-operative care after rhinoplasty. We rhinoplasty surgeons use casts to help protect our work especially after bone has been cut, shifted, and moved. Your tissues have "memory" and will want to spring back to their original state. After rhinoplasty, this could mean that bone will shift back and take away some of the nice result that was achieved. For this reason, after removing the cast the first week, I will actually have patients try to wear it as much as possible for about one month after surgery to help set the bone. The reshaped nasal bone is like a broken arm bone: it requires a few weeks of stability to make sure it stays in place. Of course, discuss your plans and questions with your surgeon but in my practice, this has significantly helped reduce the nasal bone wanting to shift to its original position.

Hello, thank you for your question. Everyone has different tastes in what they want and it is really important to convey that to your surgeon. Rhinoplasty is a complex art because changes that we make to the front or sides inevitably affect other parts of the nose's appearance.
I agree with you! I do like your dorsum on profile. I do think it is possible to maintain the essense of your dorsal profile from the side while changing the front. From a "classic aesthetics" point of view, yes, your dorsum is wide on front view and 3/4ths view. I believe that the width of the dorsum needs to be narrowed and then the boney base brought in to close the roof of the dorsum. This should be able to be done without dropping the dorsum significantly. Any changes you desire for tip refinement can also be done conservatively according to your wishes.
As to whether you need an ethnic rhinoplasty specialist, they tend to focus on races and noses that lack bony bridges such as some people of Asian or African descent. These patients actually require augmentation of the dorsum with rib or a prosthetic, something you aren't looking for.
Just find someone that understands what you are looking for, has experience with rhinoplasty, and has the same goals as you for the final outcome!

The nasolabial fold is a natural crease in the face. As we get older, our cheeks lose some volume and begin to droop. But the cheek droop stops at the fold and will tend to "pour" over the fold since that is where the cheek has to stop! If someone added volume to the cheek in the wrong scenario, that can actually enhance the fold, NOT what we want.
If you can see the bluish tint and you feel that the folds have gotten worse, your injector can use a special enzyme that breaks up the filler, called hyaluronidase. This should get rid of the filler but may take a few treatments. You can assess your options from there. I would recommend a board certified plastic surgeon or facial plastic surgeon. They can give you a full spectrum of treatment options depending on your goals, your tolerance (or not) of surgery, downtime that works for your life, etc.
With regard to Botox for the corners of your mouth, it can be injected into the muscle that "downturns" the corner of the mouth. This is a slightly more advanced injection technique, and I would recommend an expert injector (plastic surgeon, facial plastic surgeon, dermatologist, etc). The same injector could also potentially use filler in this area. Actually a combination of both Botox and Filler, when done correctly, can give you an even better result and longer lasting result than either alone.

Good evening rustimoosechic, I find that by about 8 days, the effects of Botox are quite apparent but there are reasons why you might not have your full or optimal results yet. You may have stronger pull from the forehead muscles that requires higher dosing. It may just be slower to take effect. I like to bring patients back in two weeks because by that time, the drug should have taken effect and touch-ups can be made. I would schedule an appointment at two weeks and your treating doctor can reassess you and adjust things as necessary.
Alternatively, if the forehead wrinkle is very deep and always there even without movement of the forehead, Botox may not be able completely remove the line. Other options, such as filler or resurfacing maybe help in this case. Best of luck and let us know if you have other questions!

I had a terrible reaction to a non-invasive procedure that left my body with enlarged fat deposits throughout my abdomen and flanks that required liposuction. I visited several doctors for my procedures as it was not traditional liposuction and required someone who could work with such complicated and stubborn fat as a result of my original procedure. Out of all my consults, Dr. Lahar gave me a full 1 hour to not only go over the issue but to understand my concerns, explain what can and cannot be done and to discuss my optimal results. He is very honest, knowledgeable and kind. With the type of condition I had, there was no guarantee that all the fat could be removed smoothly and that my body would return to its original form however Dr. Lahar was able to do just that. Not only did he remove stubborn fat deposits, he thought about how it would affect my entire shape and took that into consideration during the procedure. Most doctors I consulted with said the procedure would take 1.5 hours but Dr. Lahar operated on me for 3.5 hours to ensure a safe and optimal surgery.
