After having my nose smashed and having had 3 previous surgeries, I was referred to Dr. Constantian because he performs surgeries without cutting and pealing back the nose. He was able to fix the appearance of my nose and my breathing to my satisfaction. I would recommend him to anyone. He is very compassionate, understanding and is a great surgeon. His staff is also wonderful!
Dr. Constantian is the real thing. He has years (and years) of experience. Many of the other Drs I met with (I did my research!), thought of him as the best...the original. He is professional; immediately upon meeting him, he may seem aloof, I believe that's because he's approaching you professionally - his job is to make you look like your true self, not to make you feel warm and fuzzy inside. I have a lot of respect for him and highly recommend him. *For background: I had a revision rhinoplasty after a botched revision previously. I looked damaged...awful, it's hard to even relive and think about. I thought it was hopeless but now feel like myself and can move on! Something I couldn't dream of doing 2 years ago.
Dr. Constantian has done an awesome job on my sagging cheeks using Radiesse over the past 4 years. He also fixed my husbands eyelids and the results were spectacular. I wanted buccal fat pad removal and Dr. C did not recommend it suggesting I wouldn't like the results. I went ahead and had it done with a different surgeon along with some filler and found out the hard way that Dr. C was correct, I now wish I had left them in. The cheek filler by the other surgeon wasn't as well done as Dr. C. I'm delighted with his work every time!
I recently had a revision rhinoplasty with Dr. Constantian. I had previously had a surgery with an ENT at Mass Eye and Ear for a broken nose. The surgery left my nose unrecognizable, removing a dorsal hump, drooping the tip of the nose, leaving a nose that made my face look unrecognizable to me and my family. Dr. C understood the pain I was going through, the importance of identity, and was able to help me through the most difficult period of my life. After multiple consultations Dr. C performed a closed revision rhinoplasty using ear cartilage to restore my natural dorsal hump on the bridge of my nose, create a saddle/tip break, and restore the look and function of my nose beyond what I had expected possible. Other doctors I consulted with informed me that it was not possible to undo the changes that the first surgery had caused, including a large protrusion in my left nostril and a valve that did not function properly. Not only was the profile view, corrected to my natural appearance, the oblique angles and face on view came out spectacular. The skin, rather than being shiny and stretched as it was after the first surgery, looks entirely natural and I was able to return to work after only eight days of recovery. The first surgery at MEEI was performed in the open style and caused significant trauma to the tissue and skin. Having experienced both closed and open I can tell you I would take the closed approach over the open without hesitation. I would also urge anyone who is considering revision or a primary rhinoplasty to consult with Dr. C, as he takes the time to explain the entire process. The staff are all sincerely caring and take care of their patients. I have talked to others who have had horrible experiences with unscrupulous surgeons who are now horrified to have their noses fixed and do not trust surgeons and I was in this category myself. Dr. C literally change my life and saved me from the trauma of the first surgery and gave me back my life. The result is better than I could have ever expected and I cannot thank Dr. C enough for the happiness he has given me and my family by restoring my natural appearance.
I have been a patient of Dr. Constantian for over 20 years, and am so fortunate to have found him. He takes a conservative approach to achieving the most natural results for the patient. What some people may not understand is that results are never going to be perfect, and they have an unrealistic idea that you can completely alter a nose to create something chosen from a magazine. You have to be realistic and realize that your nose has limitations to what can be done to enhance its appearance. Dr. Constantian fully disclosed this to me beforehand and started off with a moderate approach on my first rhinoplasty. He also let me know that if I felt like there was another change to be made after the surgery he would be more than happy to get the best result for me. It is easy to make a small adjustment ( for a much smaller fee) after if needed, but if you are too aggressive there is no going back, REMEMBER THIS! This was my situation. After my first surgery I was very happy. It is important to realize the nose will still be settling and healing over many months so be patient! After a year I returned for a follow up and we decided the result was much better than before but there could be room for more improvement. And he was right, I had another procedure and it resulted in the most subtle natural improvement to fit my face. Dr. Constantian is genuinely concerned with the satisfaction of the patient. I have since gone back for breast augmentation which I am so happy with! I look back on pictures from before rhinoplasty and feel so much better about myself inside and out. If you are considering rhinoplasty, and are searching for the best doctor, I highly recommend Dr. Constantian. He was referred to me by a plastic surgeon 10 years ago that said for my difficult case, Dr. Constantian would be the best plastic surgeon in the U.S.. This was after I visited three plastic surgeons that would not take my case. I was born without nasal cartilage, and through the years, I had six surgeries to correct the problem. Each time, the plastic or bone slipped and my nose was wide and crooked. During my first visit, Dr. Constantian explained that he would use my rib bone, as he needed more to work with than my ear cartilage would provide. It would be an improvement, but I would most likely need a second surgery to increase the tip and narrow my nostrils. He said there was a possibility that the rib could curl or slip after a while. That was ten years ago and over time, my nose again looked crooked with a bump. I returned to Dr. Constantian to ask what he could do to fix my nose. He explained that he would decide at the time of surgery if he needed more rib or could use ear cartilage. I totally trusted his skill and judgement. Dr. Constantian reshaped the original rib bone and inserted cartilage from my ear. Now my nose is straight with narrow nostrils, a nice tip, and looks symmetrical, and I can breath well through my nose. It now looks normal! I am thrilled to recommend Dr. Constantian and his staff. They treated me with professionalism and respect; like a friend. I also appreciate that he does his operations in a hospital. Dr. Mark Constantian gave me back my life.. he's brilliant-let me tell you why..nowadays I'm location does my research but Zacs several years ago I had a standard rhinoplasty hoping the initial surgery with a different doctor would. Give me a small nose.. what I didn't realize at the time was there are many factors one must take into account for example the type of skin you have your internal structure etc... The initial surgery went fine ir was NOT a botched job by no means but it changed my face so inside part of me died.. after doing a ton of research I realize I could not ever have a small no because my skin type and facial structure was not made for it so my journey to get my face BACK or as close as I could get to looking like I did before any surgery. Many doctors turned me away "leave well enough alone" they were afraid that they would not be able to make me happy. I suffer from a condition called body dysmorphic disorder I know this because I am not only a therapist who can diagnose this disorder but I am also a patient received CBT and learned so much from my condition. Dr Constantian did not turn me away because although he knew that I was looking to achieve my old luck he also explained very carefully but was realistic and unrealistic in my achieving the results that I was looking for. Dr. Constantian is an extremely skilled surgeon who stands out as one of the most honest and transparent doctors in his field. He was actually referred to me by another esteemed local plastic surgeon who was nervous to touch my nose. I suffered a lot of damage to my cartilage and skin from a past open rhinoplasty - collapse in one of my nostrils and a depressed tip. Dr. C was able to greatly improve my breathing and aesthetics after just 1 surgery. He's truthful and upfront about what to expect, there's no fluff or false promises. He took a careful approach to my case and the results speak for themselves. Besides his talent and the obvious passion he has for his work, he and his staff have always been very respectful and kind to me - I come out of every appointment feeling like I have a whole other family who's looking out for me. If you're considering rhinoplasty surgery, you absolutely need to choose the closed approach - it yields more natural results, is less invasive and the recovery is so much easier. My experience with Dr Constantian could not be have been better. He performed closed rhinoplasty on me (one of the few surgeons that know how to do this right) and the results are spectacular. In my humble opinion, and based on my results, he is a genius. My case was difficult as he had to not only correct a breathing problem but also correct some side effects from a previous surgery that was not done correctly. I was not happy with the way I looked but I also had trouble breathing. I visited several surgeons, but I knew Dr. Constantian was the right choice right at the very first visit. I could tell that he takes great pride in his work; I could see it in the passion with which he described the procedure, the openness in telling it to me like it is, and the great deal of care he put in examining me. The day before the surgery, he worked late at night going over what he was going to do and before the surgery he again described some new ideas he had to correct some of the very difficult problems. His plan did not leave any stone un-turned. The surgery went extremely well, I almost had no bruising. The first two days were the most difficult as far as pain, but a week after the surgery I had the nose I wanted, I could breathe with no problems and nobody could see any signs of trauma on my face. I would recommend Dr Constantian in a heartbeat. He is polite, results oriented, he masters a technique that minimizes the post-op trauma, his staff is absolutely wonderful and highly skilled and he gives you enough of his time to make sure he knows what you need and want. I couldn’t have asked for a better experience.
I was tired of looking tired! Went to see DR. Constantian and the minute I met him I knew I had the right doctor. He explained everything, put me at ease and told me everything would be fine. . .and it was! The surgery well extremely well, no swelling, no bruising. Very pleased with the results. Never needed pain pills either. I recommend you follow doctor's orders for the best results, I did.
It's impossible to tell anything from one photograph, though your nose seems narrow, your tip is pinched, your nostrils are too arched, and your airway must be compromised. The solution is not to choose the approach, but rather what is done. I only do closed rhinoplasty for a reason: the skin determines half of the shape of the nose and the open approach makes it impossible to assess the skin and to perform certain maneuvers. Some surgeons and patients have the idea that they can "see better" through the open approach – – and they can see the skeleton much better, but they cannot see the skin at all. That is why so many secondary patients have already had open rhinoplasties. If it were that perfect in operation, there would be no need for revision. You need support, shape, and proportion done with the greatest possible precision by someone experienced. So find a surgeon you trust, but I would encourage you to have a closed rhinoplasty, which is not only more limited, but much gentler on the circulation.
I do not see your columella as wide from the inferior view, which is good because that is a largely uncorrectable complication of open rhinoplasty that cannot be repaired. However, it is low as your nostril contours are irregular. These, and the scars, are common problems after open rhinoplasty, which is why I never perform it. Without seeing the surface, the surgeon has no validity to fine-tune the nasal shape and must rely on sutures and struts, any of which create new problems.You are not imagining what you see. Find a surgeon who is competent in revision rhinoplasty and whose photographs in correcting problems like yours show results that you like.
Rib is the most difficult building material to work with for the nose, but the fortunate thing is that as we age, the rib becomes more calcified. That is actually very good because it makes the rib much more stable. At your age, your rib should be excellent. I discussed this in my textbook in detail and lecture about it all the time.Even young, white, elastic rib can be used easily, but the best way is to make a laminate out of it to minimize the warping. That is not your situation.The only reconstruction that will give you a lifetime solution is your own rib. I would find a surgeon who is confident in this operation and did not use anything but your own tissues. You need an operation that will work perfectly after so many disappointments.
The internal valves (the sidewall in the middle third) depend on the height and integrity of the bridge. When the bridge is reduced even 2 mm, the internal valves become unstable and can collapse and decrease the airway by 50%. I know this by my published 600 patient rhinomanometric study over 16 years.The external valves (the sidewall in the lower third) depend on the position and strength of the tip cartilages. If the lateral crura (the tip cartilages above your nostrils) are reduced, or if they are anatomically malpositioned by the way your nose naturally is, the airway can again go down by another 50%. Thus a rhinoplasty can theoretically reduce your airway by 75%.You clearly have external valvular collapse, but it is likely that your internal valves are collapsed also. This is readily correctable as long as the surgeon is experienced in this kind of work. It can be done without changing your profile or nasal shape at all, unless you wish other changes there as well. The proven techniques are spreader grafts or dorsal grafts for the internal valves and grafts into the alar walls to strengthen the nostrils. Although other methods are advocated, none have outcome evidence in the literature.Your doctors are right, but there is certainly a cure. Good luck to you
The important thing to remember is that the inverted V is not a cosmetic problem: it is a manifestation of collapse of the internal valves, which almost always happens when the height of the bridge is reduced. That's why I always place spreader grafts when I take down the dorsum.Certainly septoplasties and spreader grafts can be done safely at the same time. The surgeon can also use ear or rib cartilage if you have no septum left. In addition, if your bridge is over-resected, a large dorsal graft will do exactly the same thing as spreader grafts and fix the inverted V. I have measured 600 patients with airway obstructions and published the findings, and both spreader grafts and dorsal grafts at least double airflow when done correctly. You should be able to have an excellent result with an experienced surgeon.
