I had surgery with Dr Boahene about a week ago. I got rhinoplasty, Sliding Genioplasty and Buccal fat removal. Rhinoplasty. I wanted a natural symmetrical nose that aligns with my features. My alar is wide and my nose is a little spread out in the middle. During consultation I explained what I wanted and Dr boahene gave me his recommendations. I got my surgery done and I love the result of my nose. (It’s still swollen but the shape of my nose is perfect and it elevated my side profile so much.) Buccal Fat Removal Dr boahene recommended the buccal fat removal during consultation because I want a slimmer face. I love my result so much. The stitches are inside your mouth and they have fully healed. Sliding Genioplasty This surgery is actually what my face need the most. I have a short, recessed chin. Dr boahene recommended it for me and I went ahead with it. My face is aligned and symmetrical and my side profile is perfect. The healing process is a little tough though as the stitches are on my mouth and my jaw is still swollen. Overall I love my results. Dr boahene is amazing and really really nice. I had my surgery scheduled last Friday June 23rd and my cast removed today . I had my consult for African American Rhinoplasty on Mon & I got my surgery date today. I'm so nervous & excited at the same time. I'm fresh out of surgery, but I just wanted to go ahead and start my review, at least. About 7 years ago I had a primary that left me with a very crooked and uneven bridge and tip. My nose tilted dramatically to the left and made my whole face look slightly asymmetrical. I'm probably the only one that noticed the facial asymmetry the but the nose was obviously crooked. The bridge was uneven. The graft were obvious and leaned more toward my right side and didn't spread evenly between my eyes ( hope that makes sense). My radix was also too high for my face. The bridge looked too thin, the tip too wide, especially since I have a thin face. There was an obvious graft in my tip. My bridge was too high and the tip was too low. My profiles looked completely different. My nostrils were highly uneven and one was a notch higher than the other. I also had a bump on the bridge to top it off, lol. That is all I can think of for now. Despite this my overall look was good and most people probably wouldn't notice all these things unless I took the time to point it out. Taking pics was very thought provoking, lol, hmmm, which side photographs better? Which way to tilt my head to avoid my crooked nostrils showing? How to pose so it won't look so big? Just way too much thought went into it. It was time for a change.... Updated on 1 Aug 2014: So in early May I had my consultation with Dr. Boahene. For whatever reason I was having a hard time articulating all the issues I had with my nose in the exam room, but in his office he pointed out all of my nose issues, he was soo on point. It was not hard for me to decide to have him do my revision. He said it could be a closed procedure with ear grafting, because my first procedure was closed I liked the idea of this one being closed as well, to avoid the scars. Updated on 1 Aug 2014: Yesterday, was the big day. Let me just note that everyone at the surgery center is extremely nice, the anesthesiologist was just so sweet. It calmed my nerves about the anesthesia. When the doctor got there we went over what I wanted done, and that's when he told me that he would prefer to use rib and do the procedure open. I was okay with the change in plans because my nose was messed up and I just wanted it to look its best, and I trusted his judgement. So I was fine with the changes. Went into the OR, and was knocked out within 2 minutes. Next thing I remember is being awoken by a nurse and my husband was sitting there in the room. I just wanted to go back to sleep, I was so sleepy, I noticed my rib was sore so I knew that he used rib grafts afterall. My husband immediately told me my tip looks a lot better. I slept most of the way home but when I really looked in the mirror, no doubt the shape of my tip was much better, can't wait to see my bridge because that was very uneven before, but at least I can say my tip already looks a lot better. Updated on 1 Aug 2014: I've been feeling pretty sore, especially my ribs, not the worst pain I've felt but just really sore. My face has swollen up a lot more today, and I've been sleeping a good deal, but overall I feel good. I've been cleaning my nostrils with peroxide and using the bacitracin ointment they gave me for my incisions. Updated on 4 Aug 2014: Apparently, I owe the surgery center $500 more because my surgery went 30 minutes over schedule, ugh Updated on 10 Aug 2014: I know I said I would post pics when the cast came off, but It's just taking some time for me to adjust to my new nose. When you get used to the same face and then after years it all of a sudden changes, for me it's not easy to adjust. I know my nose is swollen, I think it looks fine but I'm just not used to this nose being on my face. I know that once the swelling subsides and everything I'll look great but in the meantime, I've just been a little out of it. As soon as I'm ready, I'll post pics, mind you the doctor did a good job, I just think emotionally it's been hard for me, so far. Updated on 12 Aug 2014: Updated on 12 Aug 2014: Updated on 21 Aug 2014: It's been three weeks since surgery and I'm starting to feel a lot more confident with my new nose. The swelling is going down but slowly, it's not like you wake up and notice a huge change but it's almost like you wake up and notice that you look better today than you did yesterday. My nostrils look more even now, the bridge looks more defined, the tip is definitely still hard and swollen but it is looking better. The swelling in the tip is especially apparent in the 3/4 profile view, it's my weirdest looking angle right now, especially on the left side, but it is getting better. My rib is still swollen and sore at times, but getting better. Overall, I am so pleased with Dr. Boahene's work and highly recommend him. Updated on 29 Aug 2014: So the swelling has gone down some and my nose is looking good, I still have tip swelling but it has definitely improved and my nose has become more symmetrical. My husband and I agree that my nose looks like it should have looked had I gone to a really good surgeon the first time. My bridge looks much more like my original bridge but better. It's like I'm back to my original nose but better.
Long story short I did a my first rhinoplasty surgery with dr s in ny which was horrible my noise was a mess I was left with visible scar upturn noise and so many other issue . In may I did a consultation with dr kofi and I was so impress, with is previous work is knowledge and what I want to accomplish . So far I am healing..not expecting a Kelly Rowland nose; just a softer version of mine..Friday I went to the hospital (White Marsh) the nurses was very kid a and attentive saw Dr Boheine very handsome smiling I was so glad I was very nervous..As this was my second nose job the first one was not what I expected,,Dr Boheines presence reassured me I was in good hands..I am very grateful this is my third day little swelling and bruising..my nose is kinda stuffy..but I feel Good waiting for bandages to come off on Wednesday...I will post pictures soon:) so far so good!! Updated on 16 Jul 2014: I felt this nose was still too wide..a year later I relieved a cortisone shot and my nose swelled up even wider.. Updated on 16 Jul 2014: That first review was not to good OK I am 4 days into recovery..The nurses Dawn and 2nd Dawn was so funny..They kept me laughing so much I guess they knew I was nervous;( Dr Bohene was wonderful I couldn't asked for a better surgeon( I truly believe this man is an ANGEL)..His voice is calm I mean his touch is reassuring ..I told him I was not looking for a kelly rowland nose I just wanted a softer version of my own...I already have big eyes and lips to fill out my nose..I was gonna go with Dr Naderi but his quote was $11.000 plus I heard Dr Bohene specialized in ethnic noses...so here I am feeling good little pain..nose is still stuffy but minimum bruising...I am swelling but should subside...I'm just glad its out the way...so far so good..I will post more pis when the bandages come off:) Updated on 17 Jul 2014: Alright I actually like this nose better... it's more refined.. I do know it will take some time for the swelling to subside...and the natural shape to fall in but so far I am pleased.. i always hated my nose as it grew bigger as i got older. i come from a family of really ugly and wide noses, and i hated it. I had rhinoplasty, and when the splint came off, it was gorgeous, but it keeps changing everyday and it looks like my old nose somedays. However, i did tell my PS i wanted a subtle change,a nd that's exactly what i got, but nosw i feel like i want a drastic change (cosmetic surgery is addictive.lol) i hope the nose keeps shrinking. I had septal cartilage to augment my bridge, and that's all i know. I'm thinking of getting a revision in the future, but i'll watch my nose for another 2 years, and i'll make my final decision. Updated on 15 Apr 2014: so i had steroid shots in february (8 months post op) because i didn't like my tip. my nose was too swollen and much bigger for about 5 days, and i looked horrible. i didn't notice much of a difference after the steroid shot. my tip still looks really wide and i'm considering a revision at or before my 2 year mark, where i will have an open rhinoplasty (my primary rhinoplasty was closed) so my surgeon can see my tip cartilages and do something about them. my nose looks a lot better than my original nose so i'm not too bothered but i want a better nose and i'm willing to take a chance. Updated on 8 Jun 2014: i have decided to crop out my face and post a picture of what my nose looks like today. i am not pleased at all, and i will be having a revision. like i said earlier, it's all my fault, i asked for extremely subtle rhinoplasty because i was scared of being judged by people and i was scared of looking like michael jackson or lil kim. i will also post a picture of what my nose looked like before surgery. my main problem was my wide and fat tip, and the tip looks exactly the same to me. Updated on 8 Jun 2014: my problem with my nose now is the bulbous tip. the pictures do not show how thick or wide my tip is. compared to me features my nose looks big and the tip looks thick too. i will be having a revision rhinoplasty and this time, i will opt for an open rhinoplasty so my doctor can get a good look inside my nose. The healing process after rhinoplasty can be very tasking for both the patient and doctor . Going in to rhinoplasty with a clear understanding that rhinoplasty is a process and not an event that concludes after your surgery will help maneuver through the healing process. The nose responds in different ways after surgery and really needs time to heal. A pollybeak( appearance of a hump ) after surgery may occur as a result of various reasons . Swelling of the thick skin is a common reason for pollybeaks after rhinoplasty. This may resolve with time, taping, massaging or steroid injection. Pollybeaks may also result from a relatively high cartilage that will not resolve with time. This will require shaving down the cartilage. In your specific case , I will recommend that you work with your primary surgeon to come up with a plan to solve the problems you have identified. Give yourself time to heal before any further surgery, if needed. Chasing an evolving process can lead to more problems. Most surgeons who perform rhinoplasties on a regular basis are able to solve these post operative problems.
rhinoplasty for individuals withthick nasal skin is extremely challenging but not impossible. You have to be prepared for a process that only starts with the surgery but requires further care over a year to two years after the surgery. You may need minor revisions to further improve your results . Because thick skin does not contract well to take a new shape after rhinoplasty, your surgeon may spend quite a bit of time selectively thinning the skin as part of the rhinoplasty . The results can be dramatic right after surgery but swelling may blunt the early result as healing progresses . With patience, steroid injection etc the results improve In your specific case there will be the need for tecontouring tour nose to create definition along the bridge, tip and side walls. The nostrils can be reshaped to match the new nasal contours
Nostril reshaping may be performed as an isolated procedure or as part of a complete rhinoplasty. An evaluation of your entire nose will help determine what approach is best for you because changes done to the nostril may affect the appearance of the entire nasal tip. There are several ways to reduce too much nostril show. These include setting the columella back, repositioning the alar forward, reducing the size of the nostrils or changing the rotation of the tip. A combination of the above techniques is often necessary. Nostril asymmetry after rhinoplasty is one of the most common reasons people seek revision surgery. Most people have asymmetric nostrils even before surgery. This asymmetry is usually very minimal and not obvious. Such asymmetry is likely to persist after rhinoplasty an May become bothersome because one analyzed the nose more closely. You should prepare yourself to accept a small degree of asymmetry that is seen only on very close scrutiny. Obvious asymmetry can be corrected with minor corrections .
Your goals for an ethnically congruent and refined nose with natural features are achievable. The techniques selected depends on your skin thickness and other facial features . An inperson examination is helpful but can be preceded by review of your photos and an online consultation . Patients I have seen from London often opt for online consultations followed by an inperson examination and surgery during a single visit .
