Dr. Day is absolutely amazing! Love, love, love him! I would never go anywhere else. He cares about his patients and always wants what is best for them. So glad I found him... he is the BEST. I will follow him anywhere. There is no way I could have made it through my diagnosis without him or his staff. I had the best results from surgery. He is caring, sensitive, and understanding when it comes to women with breast cancer. His #1 priority is his patients and making sure they are happy!

Hi Everyone! Today I got word that PacificSource approved a medically necessary breast reduction, hoping to move from a 32F to a 32 B/C. Using the same surgeon who did my tummy tuck years ago. Hoping to resolve back/rib pain, tingling fingers, achy shoulders and lessen strain on a head/neck injury from 5 years ago. Starting out at about 138 lbs (still losing, hoping to get to 135?) and 5'4". After two kids and combined years of breast feeding, looking forward to a weight off my shoulders. I would love feedback on the best tips to prepare for the recovery afterwards. After the tummy tuck w/muscle repair (which was a TOUGH recovery) I'm familiar with some of the basics, but am curious with how I'll feel after an upper body surgery. Also, since my tummy tuck, I was in an auto accident which left me with a post-concussive syndrome, (mainly difficulty with over-stimulation--too many lights, too much noise, etc.) chronic migraines, and damage to some nerves in my head/face that cause pain. Lots of recovery since it originally happened, but it may pose some unique challenges along the way. Thanks Everyone! I look forward to making this journey with you all! :) Updated on 7 Sep 2020: Hi guys & gals, (Accidentally typed and deleted this once! Please pardon my typos, still rummy from the painkillers.) So, the surgery happened a couple of days ago and it went very well. I got to see them today before my permitted shower and they are SO CUTE! Definitely tiny little B cups, just like I wanted. About 300 grams off each side with a little extra from the left. My Dr. was amazing as always as was his staff. The ambulatory surgical center was very nice and clean and it wasn't hard at all to follow the covid rules. My husband wasn't allowed to go in with me, so we did a quick goodbye then texted during the downtime. I'm glad I scheduled so early, less time to get worked up about it. I woke up from the surgery comfortable and warm all snuggled up in my bed. I spoke with the anesthesiologist about the horrific nausea and vomiting I experienced after my tummy tuck and she decided to only use iv drugs instead of gas (which apparently makes LOTS of people sick afterwards) to give me an extra long wakeup time, and they stuck a patch behind my ear. Night and day difference. It took me 2.5 hours to wake up enough to go home, but was totally worth it not to be sick. Oxycodone makes me vomit so they rx'd hydrocodone which I'm taking every 4-6 hours around the clock. It's not as potent as the other drug, so my pain at best is about a 4, but also part of the balancing act to not be sick. I went in at 6am, started at 7:30, was home by 1pm? The first afternoon I was just wobbly in the legs, foggy in the head and mildly tired. My throat was majorly scratch from the breathing tube and my mouth was really sticky (they said from the nausea patch.) Alli I felt were a few small twinges and nothing widespread. Yesterday, I could tell that the swelling had intensified as even with hydrocodone it felt exactly like a cross between mastitis and needing to pump REALLY bad. Have been using ice packs and the girls are flattened down in a big, wide, ace type wrap. Drain output is still normal. Today, I'm a bit more uncomfortable when it's time for more pain medication and during that time it feels like a wide band across my chest of a burning fresh bruise. I have chronic migraines and occipital neuralgia and this still hasn't reached half of that level of pain. Very manageable. The attached pictures were from the morning of and 48 hours after. I specifically asked for small b cup "sports tits" so I wouldn't have to wear a bra again (after the dr says ok) as it causes my neck injury to flare up. I go back for my first follow up this Tuesday. Take good care, A. :) Updated on 10 Sep 2020: Hi all, Tomorrow will be one week since my reduction, so far so good! They are still adorable and I am still very happy. I had my first post-OP appt this last Tuesday and since my drain output was minimal, they were removed. One side felt a little bitey, the other side I didn't feel. The drains had been hurting so I was glad to see them go. I think all of this healing caught up with me I slept for 12 hours last night and am exhausted still today. I was able to transition from the wide ace wrap to a snug, wireless soft sports bra 24/7 for the next month. I picked one that was really comfortable (from True & Co) but probably not tight enough. The next morning, my breasts were tight and uncomfortable and quite a lot more full than before. The vertical incision on the right side was leaking little droplets of light colored fluid. Switched to a very snug true sports bra and packed on the ice today, and things are moving back in the right direction. It still feels like I'm about 24 hours overdue for pumping milk back from my breastfeeding days. I was given permission to add back in an NSAID which is helpful as usually I take one for an old neck injury. Other things: Hubby helped me with the first two showers, but today I took one on my own! I had also tried to taper down on pain meds a few days ago and that was a foolish, painful endeavor. However, today I tried to reduce down and it went much better. I'm cleared to drive whenever I stop taking them, (and I have a ton of errands) so am working towards that. I took off all this week from work and am planning on starting back on Monday. I had the option to take a second week, but I think I'm just about ready to start using my brain again. I work from home at the moment on a computer so I think I'm up to it. If it were a physical job reaching and using my arms I would definitely need more time to recover. All for now. :) More pictures coming soon.
well 10 days and I will have my fleur de lis and panniculectomy with dr Kevin day in albany or. I'm nervous and excited and worried all at the same time! dr day and his assistant Cathy have been amazing spending so much time explaining and drawing on me to see what will be gone! I have gastric sleeve 4/4/14 and lost 175lbs and now it is time for me to get my ability to run and play tennis and not have my feet go numb!!! Darn the hanging tummy skin!! I've earned this! But it ll be painful to get me to where I can do even more to maintain this loss!!! I ve wanted this so long I'm scared it won't turn out like I hope. I just want to run! Have my feet NOT go numb while on treadmill or in car!!! Pain I can do... But I worry about the incision and infection. Im A splenic meaning I don't have one which makes me a little immune deficient so I ll be on antibiotics. Deep breath!!! I'm coming to the flatter side!! Updated on 21 Aug 2016: well it's two days PO. And I think I'm doing great! Day one I don't remember much except that I woke with no tummy laying on my legs!!! There was pain but honestly not that bad! Went home about 5 pm. Recliner was best idea and worked well. Had some broth. Lovely narcotics and had a pain pump. Will take pics soon. Updated on 21 Aug 2016: well went for my day after check up with surgeon who made a special visit to the office!! I told him the tube didn't look full on the pain pump and that I didn't feel any relief. He was shocked to find it CLAMPED OFF!! So all night I hade no pain med. but honestly wasn't too bad!!! So word to the wise make sure pump is unblocked before you leave the hospital!!!! Dr said all went great. I'm really swollen but he removed 14 lbs of skin!!!! Updated on 23 Aug 2016: okay so my... I got this mentality is out the door! So had to stop the narcotic on night of day three as I was itching and had a headache straight from the devil. I still have the pain pump is so we just went to Tylenol. The incisions don't hurt to bad more than anything it's my back!! Being hunched over and feeling the urge to stretch is hard. Sooo hard. It's funny because before I would've had my back go out because of the skin hanging and pulling now it's holding on but the muscles are so. Sore!!! Going to second follow up today. I kind of feel like a giant water balloon puffy!! But I slept through the night still in recliner. Not sure sleeping all night is a good thing because I'm so stiff in the morning. Walk walk walk! In other news! First BM after four days! My husband clapped for me then got not so happy because he has to flush the toilet... It's too low for me to flush. I was worried about wiping but man is it EASY!! Updated on 23 Aug 2016: well...I'm allergic to derma bond the adhesive they used. So on to steri strips. Rashes everywhere but doing better one the derma bond was removed. Dr day said I'm healing good no sign of infection just irritation from derma bond. Drains still in and pain pump to because it wasn't on the first 24 hours it still has a little left. I'm a little worried about the upper abdomen swelling but dr says it's gonna get smaller. I did this surgery strictly to help me continue to exercise and not have numbness in my feet or hurting back... But part of me wants it to look better too... Nervous about end result. I'm no Barbie and I'm not afraid of working out but I want a head start. Got to take a shower today!!! Ahh even though it was tiring I got all the glue causing rashes off my skin!! Part of my tummy is so tight and other parts are a little looser. Can't wait for these drains to come out! My pain pump will be removed at home by my husband...dr showed him how gulp! Updated on 25 Aug 2016: so I made hubby go to work and babys at daycare so I am home alone!! We got everything within reach and stocked water and TP and Tylenol! I walked around the block today! Pathetic! Hubby will take out the empty pump today as in already getting feeling back. It's kinda like when your foot falls asleep and wakes and burns and tingles and it's annoying and shocks you with electric shock occasionally. Not unbearable. Bowels are going good. Standing a little higher. Last night wee changed pads and bandages and the incisions look way less angry from the allergic reaction to the derma bond. But I have a weird shape on my right lower hip and we are trying to bind it to push the fluid out but wonder if that will need a fix. Updated on 29 Aug 2016: ugh so yesterday got a little fever of 101.7 I'm thinking a cold and not incisions. Took my second shower today awwwww! But dang does that take it out of you! Incisions look good I tjink with steri strips it's hard to tell. Sleeping is the worst trying to get comfy. I see the doctor tomorrow and pray he'll take at least one drain out :( I would say of all the drains are the worst pulling leaking pushng in the wrong way oy!! I feel so bad for my hubby because our toddler is teething and has a low fever also I think he'd rip his hair out if he didn't shave it off!! Getting closer I hope to normal. Updated on 30 Aug 2016: so my left drain is still clear and normal but my right drainage is creamy and gross so headed to doctor to see if this is the reason for fever. We shall see! Updated on 30 Aug 2016: so had my check in with surgeon. He does not like the drainage from the right drain but says the best treatment is to keep it In and let whatever abcess drain effectively. Incisions look good beyond my rash irritation from the original derma bond. He took the stitches outta my belly button and noted that the bleeding means I have good blood supply for healing. He said all looks good if we can get this infection resolved. I have no spleen I should mention I lost it when I was 16 and my immune system is not as strong as it should be. So I originally had a 5 day antibiotic that I took and I think that warded off any infection. So we re doing a stronger one this time! Hope it heals follow up in one week! Updated on 31 Aug 2016: well. I must say I am feeling better!! Heavy antibiotic is working drain fluid is so much better looking still a little milky but huge improvement. Belly button looking good. Right now my biggest concerern is my GINORMOUS MONS PUBIS . Pics to follow I of course know many have said this is normal but holy crap! My doc thinks that's were some of the infection is related to skin reaction from tape for drains . I'm sitting here with ice on the girly part! Hoping some of the pain and swelling will decrease there. Walked three blocks today. And made de arbol salsa by using my grabber to get the ingredients outta the fridge lol!! Updated on 2 Sep 2016: so during bandages we are doing arnica tea swabs and boy does it feel great warm and wonderful!! Drain fluid is clearing nicely and my cc's are dropping and I'm excited for Tuesday to see if these dang drains can go!! Low fever only now 100 and antibiotics are working three walks today. Baby steps ! Updated on 11 Sep 2016: So drain came out on 9/8/16 and now I'm using scar gel and VIT e oil massage two times per day!! So far going good moving around a little better and trying to get more active. Still can't lift baby and Borge baby and I are sick :( ugh Updated on 13 Sep 2016: So I'm finally getting around to posting some standing up clothes on I ll get some of without clothes when I remember. I'm not afraid of working out so that starts next Monday! I'm moving more and sooo swollen at the end of each day! Ice packs help! So saw my PS on Thursday for another incision check. He says looking great I'm doing vitamin e oil massage over all abdomen and scars then twice a day skin medica scar silicone gel. My doctor said I could finally have sex!! Ummm oops. Well we found a way to have me use no muscles and were able to have sex on edge of bed at two weeks. I know they said to wait but hey... My husbands hot lol! So being very careful!! I do have serious itching and a lump above my belly button and a marble on my right side under the skin my doctor says is a muscle incision knot and is having me use powered massager to soften the scar and muscles!! Yay! Updated on 30 Sep 2016: So I'm about 6 weeks ish and got a little red spot on the right lower incision. Had a tiny bit of drainage so I went to the surgeon as I do not have a spleen so my immune system is not great. He said it was just a suture abcess and we are putting antibiotic ointment and a gauze pad as I'm allergic to adhesives. All in all feeling good back to work after a week and a half and tho I'm swollen at the end of the day I'm doing good!! Updated on 1 Oct 2016: Right now my sides are swollen!! But you asked I deliver lol! So fyi when you start to feel better it's easy to over do it!! I think I lifted and moved WAY WAY TOO much and am hurting quite badly tonight :( I am swollen beyond belief and hard :( I'm allergic to the darn bandaid the dr stuck on my little suture abcess for two hours and now have a rash... But still worth it! Icing the belly and my husband gave me the best vitamin e oil massage!! Updated on 1 Oct 2016: Updated on 11 Oct 2016: Well I thought all was going well...and then I went back to the gym...???? so both spots over my hips are irritated. Band aids causes rash and the swelling was intense. All I did was the elliptical for 25 min then arm lifting. Sweeping still in tummy after two days. Dr said I'm over doing it and have to cut back on lifting. Updated on 20 Oct 2016: Ugh so swollen and lumpy! Dr says normal. so I'm 8 weeks definitely doc thinks dog ear revisions on both hips. I'm whole torso covered in rash related to an antibiotic shot I got for an unrelated infection for cold season. I'm immune deficient without a spleen so they treat any infection very seriously in me. Well I was allergic to this injection and now have full torso rash... However he thinks I may also be allergic to the scar gel as I am not getting rash to clear. Seeing dr on Monday. Mons is not getting smaller and dr says my muscle wall was lower and could be pushing it out and may want revision. Not sure what to do yet. Still worth it but frustrated. Fun to shop but irritating to itch!!!!
I had gastric bypass 2 1/2 yrs ago, lost 90+ lbs. and am now ready for the finishing touches to make my journey complete. I had 3 cesareans also, which has contributed to the loose skin & separated muscles. I am having a fleur de lis tummy tuck, with muscle and hernia repair, along with a BL/BA with silicone implants. My wish is to be a large C to small D cup. I had DDD cup breasts for many years before a breast reduction & don't want them that big again! I am 5'2" and about 140 lbs. My surgery is planned May 20, 2016. Updated on 14 May 2016: I had my pre-op appt. on Thursday with Dr. Day and his assistant Cathy. I also met the doc's new Physician's Assistant. We discussed in detail the procedures and risks involved. The only thing I have to decide upon by surgery day is size of implant. He will bring two sizes for me to see on that day, but ultimately it is going to be 325 to 400. Initially, I was leaving it up to him, but I asked about how many CC's he would use & he got out his catalog and told me with my measurements, he recommended between 2 different ones. One would make me larger than the other, more of a full D, the other large C, small D. Oh boy... why is it so hard to decide?? I don't want to be really big & noticeable in clothing, but I want to be big enough that I am happy again. Any suggestions out there??? I know it is hard without pictures... Updated on 2 Jun 2016: I wanted to give an update on my surgery, which was May 20th. I had a rough start with nausea when I woke up and the feeling that I couldn't catch my breath. That was awful! I was in recovery for about 3 hours and by then, felt a little better. My hubby drove me home and kept me drugged up every 4 hours. The pain meds made me nauseous & I had to take meds for that, which knocked me out. I ended up throwing up Sunday night, which wasn't fun, but that was the worst of it. I went to the doc on Tuesday and everything was healing well and he took out the pain pump. Wow, I really knew how much it helped when i didn't have it any longer!! If you have the opportunity, get it for your surgeries! It was very worth the cost! I had quite a bit of pain for a while longer, then by the 1 1/2 week point, it seemed to dissipate and I was over the hump. I can see why so many over do at this point because you feel so much better after feeling so bad. But I have continued resting each day and just walking around the house. Even right after getting home, I walked immediately every time I had to go to the bathroom or take meds. It is so important! I saw the doc today and got rid of one drain! Hurrah!! One left to go since it will do the job of 2 in the next few days, hopefully weaning away from it by next Tuesday. Dr. Day took off the tape over my incisions and said they looked great. I heal very well. I am starting silicone treatments on the scars now, which are already flat. I had to wear a strap to lower my right breast, but it already worked over the last week so it went bye-bye too. My breasts look natural and as I read somewhere on this site, they look just like mine only bigger. I got 350cc in one and 375cc in the other, to make them even, under the muscle. I feel twinges of shooting pain and achy at times, but much better. The first week, if you didn't move, you didn't feel much pain, but if you moved then...whew!! Pain!!! Anyway, I am happy with the results so far and thankful to be at this point of the healing. Thank you to so many who share their stories. I appreciate all the info I have gotten from this website!! Updated on 2 Jun 2016: I ended up not needing the breast lift, just the implants. I was so thrilled!
First of all, I would completely agree based on your photographs that you should not need a mastopexy (breast lift) and augmentation alone should be fine for you. When it comes to implant sizing, however, it is hard to determine if a 365 cc implant would be right for you or not from photos alone. Implant selection is based on measurements of your breasts and chest wall as well as on your specific goals regarding cup size and projection. Those are questions that your surgeon can answer. Based on your photos, though, a 365 cc implant doesn't look like it would be out of the question as an appropriate sized implant for your frame.
Every surgeon will recommend postoperative breast supportive garments according to the surgery they performed and the expected recovery. I typically will use a supportive wrap in the first week that can be adjusted based on postoperative swelling and then transition to a sports bra at one to two weeks postoperatively. There is a great local store in my town that will obtain precise breast measurements for cup size and order the most appropriately sized bra based on that. If you have a similar store where you are, I'd recommend that once your final results are achieved. Your surgeon likely has a very specific reason for requesting a smaller cup size. You should not feel at all uncomfortable asking him his reasoning for the smaller size. I'm sure he'd be happy to answer it for you.
On occasion, the absorbable sutures that are used to close your incisions may work their way out of the skin before they are fully dissolved. This will expose the superficial layers of the breast skin and tissues underneath to bacteria and possible infection. The bad news with this is that it will take longer to heal and will typically result in a wider and less aesthetically pleasing scar. In a worst case scenario, the infection can move deeper and infect the implant necessitating a return to the operating room, though this is pretty rare. The good news in your case, based on your pictures, is that it appears to be healing well without evidence for surrounding redness or cellulitis (skin structure infection) or diffuse redness or swelling to indicate implant infection. It sounds as if it is being treated appropriately. This has happened on occasion in my patients and when it does, I offer free scar revision under local anesthetic if the scars remain unsightly. I will usually wait 6 months to 1 year from the end of wound healing to revise the scars. You may wish to discuss this possibility with your surgeon if you're displeased with the appearance of the scar once it has healed. Best of luck!
Judging from the pictures, it would appear that you have some hypertrophic scarring around the right areola. At 11 months, typically any absorbable suture material should be fully dissolved so I doubt it would be a reaction to suture material this far out. It does look as if you have more volume and fullness superiorly on the left breast than the right. Some more information would be helpful in answering your question. An additional photo with you lifting your breasts so the breast creases (where the breast meets the chest wall) can be seen could show if there is asymmetry in breast fold position. Additionally, I'm assuming that you had a periareolar mastopexy (breast lift) with your augmentation? Do you know if your surgeon put in a permanent suture to support the areolar skin and if so what type was used? If they did not, you may have areolar spreading as the skin and scars have healed. Also, what is the size of your implants and the position relative to the muscle (above or below). It looks like they are above the muscle but it is hard to tell definitively from your photos.
Breast implant associated anaplastic large cell lymphoma (ALCL) remains a very rare entity and of the known cases, infection is not a known cause. As other surgeons have noted, low grade infections or inflammation around an implant can lead to a capsule contracture (tightening of the scar tissue capsule around the implant) but not ALCL. ALCL is associated with textured surface implants and typically presents as a late (several years postoperative) seroma or fluid collection around the implant which would appear as unilateral swelling and fluid around the implant. ALCL remains very rare, so don't let it cause you a large degree of anxiety. If you note the above symptoms, be sure to get evaluated by a board certified plastic surgeon.
