Dr. Joshua D. Rosenberg is a double board-certified plastic surgeon in New York City, with over 11 years of experience. I can’t speak highly enough of Dr. Rosenberg as a facial plastic surgeon both cosmetically and functionally. I had a septorhinoplasty and chin augmentation done with him in January 2020. My functional goals for surgery were to breathe better and have a more open nasal passage so that other nasal treatments (sinus rinse, nasal sprays, etc) would be more effective for my seasonal allergies. My cosmetic goals were to still look like myself but straighter and more symmetric, especially in my profile since I had a bump in the bridge of my nose and my tip drooped downward. I didn’t initially want a chin augmentation, but Dr. Rosenberg recommended it to balance out my nasal proportions and I am so glad I trusted him. From a cosmetic perspective, he achieved a really natural and subtle change in my nose from the front, while getting a significant change in my profile and straightening the bridge of my nose. The chin augmentation was the perfect size and it balances out my nose and gives me a stronger jaw line. The people who know me well say I look even more beautiful, but still so much like myself. The people who don’t know me well assume that I look a little different because I had to wear contacts after surgery instead of my glasses. To me, that is the highest compliment for a plastic surgeon, that they are able to enhance someone’s natural beauty, rather than make them look like a different person. From a functional perspective, I can already tell that breathing is much easier and that my allergies are more manageable, with less sinus pressure and discomfort. Additionally, I am a singer and Dr. Rosenberg was great about ensuring my voice would not be negatively impacted by surgery. Overall I am so happy with my results and can’t thank Dr. Rosenberg enough! Updated on 28 Apr 2020: I am posting before and after photos here that were not included in my original review. The post-op photos were taken at 3 months after surgery. The before photos were my worst angles in pictures, which now I don't have to worry about. Again, I'm very happy with the results and feel like I still look like myself.
I have been a patient of Dr. Rosenberg's for years. He is a true artist with Botox and fillers. Even though I no longer live in New York, I make sure there is time in travel schedule to see him every few months when I am in the NYC. Trust me, he is worth the effort.
It is 2 months since I had nose surgery with Dr. Rosenberg. The swelling has begun to subside and I am delighted with the results. I always had a big hump I wanted to get rid off. Now I have a nice, straight nose from the side. I can't say enough about the whole experience from my initial visit until now.
I was seriously nervous about getting Botox for the first time. But a deep line on my forehead has really been bothering me. I decided to give Botox one try and two weeks later I am so happy I did. Dr Rosenberg did an even better job than I could have hoped for. It didn't hurt & I look 10 years younger!
Short answer, Yes. Your goals are reasonable and achievable. I would consider a preservation approach to your nasal bridge. Go for it, you will look great!
Your family is right, you do have a very feminine appearance. “Need” is challenging word here. It really depends on your goals. If having the most feminine appearance possible is that goal, then some aspects of feminization surgery may help you achieve it. Brow/orbit reduction would likely be the most powerful procedure for you. The prominence of the brow and orbital rims is a strong subconscious indicator of masculine vs feminine for most people. Reducing this and creating a smooth transition from the brow to the nose and midface will definitely help you look more feminine. Depending on the height of your hairline the incision for this surgery can be hidden in the hair, making this essentially “scarless.” If you are decidedly surgery adverse, consider Botox to your masseter muscles. It will be a nice touch for you. Finally, jaw and chin reduction could also be nice for you but I would consider them a second priority to your brow.
Your surgeon will likely check if there is enough cartilage left in your septum. The classic septoplasty typically removes most of the septal cartilage that could be used as grafts during rhinoplasty. But, this is not always the case. If sufficient septal cartilage remains, my assumption is this would be your surgeon’s first choice. If there is not enough septal cartilage, ear or rib cartilage is typically the second choice. Ear cartilage has limited utility and rib cartilage is really the best secondary option. Finally, some surgeons use cadaveric rib cartilage. The plus side is that there is no rib donor site (at least for you). The downside is that it has some characteristics that make slightly inferior to your own rib cartilage. It’s also not always available. Good luck. Do keep in mind that the rib donor site is fairly minor. If you go forward with surgery, I’ll be you will love your new nose way more than you care about the donor site.
There is no data proven, scientifically correct answer to this question, but I strongly prefer liposuction Liposuction can be done in a precise manner to address the excess fat in your chin/neck/jawline with near immediate results. It is designed as a one time treatment. The recovery is straightforward and fairly quick. Kybella typically causes significant swelling and soft tissue “induration” (firmness and inflammation at the treatment area) than can last for weeks +. Needing multiple treatments is not uncommon. The results develop over time.
From your history and photographs any treatment plan needs to be focused on actual skin tightening, not fat removal It is important to understand that although there are a number of reasonable non surgical options, non provide the dramatic results of surgery. Your expectations have to be line with this reality I would consider either ultrasound based skin tightening such as Ulthera or radio-frequency based skin tightening, perhaps coupled with non ablative laser treatments such as IPL.
