Based on your photograph it looks at though you have only recently had the Augmentation.

The first few weeks post operative your breast might feel tight and uncomfortable , it can take up to 6 weeks for the feeling to settle and the breast to "soften".

Just like your breasts undergo fluctuations during the course of the month being more sensitive during certain times, than others it is quite normal to experience exaggerated breast symptoms in the immediate post operative phase, this will settle down in time, if not consult your Surgeon.

Breast droop is called ptosis , ptosis can range from severe to moderate. You seem to have a moderate level of ptosis. Position of your nipple is good. If you would prefer your breasts to sit higher on your chest , you may consider Polyurethane coated breast implants (PBI) placed in the sub glandular position. Polyurethane breast implants biointergrate with your bodies own tissues effectively acting like Velcro preventing breast ptosis in the long term.

No lift will be needed if you use PBI correctly, the only scar will be in the inframammary crease.

Your surgeon is perhaps correct in saying a "standard" breast augmentation would not provide a suitable solution to your tuberous breast condition. 

Tuberous breast have a very narrow breast base and your right breast seems to have a higher inframammary crease. It would seem that you would require a prosthesis that will broaden your breast base and the right breast inframammary crease would need to be lowered.  Most surgeons have been advised to avoid adjusting the inframammary crease as lowering the crease can often result in the prosthesis dropping below the breast mound. 

In my practice I exclusively use Polyurethane coated breast implants that are proven to provide stability and durability when readjusting the level of the inframammary crease.
