Four years after my breast lift with implants in 2018, both breasts had become encapsulated causing severe disfiguration. Having surgery to correct the problem wasn’t even an option for me, so I began searching for a surgeon. I knew I was going to be a very challenging patient, so finding a surgeon who I believed was the most qualified was crucial. After ‘interviewing’ surgeons in Mobile, Fairhope, Pensacola and Gulf Breeze, I miraculously ‘happened’ upon Dr. Koehler. His warm bedside manner immediately set me at ease and the more we talked, the more confidence he instilled in me. After hearing his surgical plan which involved redoing my lift and creating an internal bra using mesh, I felt absolute assurance in his ability. The actual day of my surgery, when I was able to see the outcome, I was stunned - my breasts, although quite bruised as you’d expect, were exactly what I’d hoped for all along but never got. At the age of 71, I finally had perky, shapely breasts much like I had in my 20’s before 6 years of breastfeeding took its toll. My first 3 surgeries were at the hands of big city, very reputable plastic surgeons that I feared had more sophisticated training and expertise than any small town, Alabama surgeon could have… was I ever wrong! There just aren’t words to adequately express how happy I am with my new breasts and how incredibly thankful I am to have found Dr. Koehler. Not only is he a truly gifted surgeon, but he’s also an incredible human… unlike so many surgeons that I’ve met over the years, as a patient and as someone who worked in the medical field for many years, Dr. Koehler truly cares about his patients, and it doesn’t take any time at all to realize that about him. And his staff… all of them... are wonderful, caring, and thoughtful women who have clearly been selectively chosen to serve and represent Eastern Shore Cosmetic Surgery. My only regret is that I wasn’t living in Fairhope in 2018 when I had my first lift… I have no doubt that Dr. Koehler would have been able to give me a wonderful outcome with superior results that lasted.
While researching online I noticed very few surgeons in the area who posted quality results on African Americans. This alone made me consider the Miami surgery trend. After consulting with Dr. Kohler and reviewing his digital portfolio, I was sold. He answered all of my questions and never suggested additional cosmetic procedures. My bilateral mastopexy without implants was completed February 2023. There were no drains, minimal pain, the inscions were precise and pencil thin.
Doc did a facelift and neck lift. I could not be happier. Work is precise and excellent. Recovery was easy. Staff on top of after care. Care in surgery was phenomenal. Scars are not seen and work looks natural.

After having my third child, I finally decided to get rid of the pooch. After doing tons of research, I decided to use Dr. Koehler. I am 2 weeks out and so far happy with my results. Dr. Koehler and his team were amazing! I am still very sore from the liposuction of the flanks, but each day it gets better. Updated on 17 Jan 2019: 2 inches gone and about 40% swollen (mostly swollen in back/flanks). Still bruised on my back but it improves each day. Updated on 18 Jun 2019: 6 months post MTT; I am excited with my outcome. Dr. Koehler did an excellent job and I highly recommend him for this procedure. The swelling is completely gone and I finally feel confident in a two piece bathing suit.
Dr. Koehler is brilliant. He didn't do what I wanted him to do by giving me the lift and implants together. He assured me that left first with implants 3-8 months later would be ideal for me. He was right. I am so thrilled with my results both after the lift where I ended up with baby boobies (compared to before), and after augmentation where I have big boobies. He does fantastic work, and couldn't be happier with my choice in surgeon. Updated on 24 Aug 2018: I had my one week post op check with my doctor yesterday. Everything is going well. I was finally able to come off the pain medication on day 6, but still took the muscle relaxer once before bed and once in the middle of the night when I woke up in pain. I'm having a good bit of pain in the left one this morning after the doctor did the massage yesterday. It was rough getting through my husband doing the massage this morning, too. Otherwise, they are great. I love my results. Updated on 31 Aug 2018: Yesterday made two weeks since my augmentation surgery (lift was in April). All is going well. I'm FINALLY starting to hurt much less. Boy, I was not prepared for how much it was going to hurt considering my lift was a breezy recovery. I took nothing for pain last night, but still took the muscle relaxer before bed as a precaution. I'm able to sleep on my sides now. Hooray! I'm 41 and 5'4" 117lbs (119 since surgery). By 13 I was a 34 DDD, and never had cute, perky boobs. 4 years ago I was an H cup. At the time of my lift I was a DD, but mostly saggy slop. I'm not even sure what I was after my lift or now with the implants. I went with 405 cc Natrelle inspira cohesive gummies under the muscle. Husband wanted 450, and doc said he would have recommended 600, but there was no pressure from either. I'm thrilled with my choice. The only time I've felt down or sad was when the pain was getting to me. So thankful that has eased up, and allowing me to enjoy my new boobs. Updated on 6 Sep 2018: Everything is going great. They are further apart than I'd hoped for, but I was anticipating this prior to surgery. I'm SO wide. Pain is no longer an issue with the rare exception of shooting pains. I still cannot feel my nipples. Will that come back? I'm not sure I care. As you can see by the evolution of my boobies picture, these babies are a million times better than what I had. I'm so completely in love after having hated my breasts my whole life. Best thing I've ever done for myself. Updated on 13 Sep 2018: Everything is going really well healing wise. I still have no feeling in my nipples though. If it is going to come back, when does that usually happen? I do still have random burning pain on the left side near the incision, but I can't look at it and see why. Everything looks fine and well healed. They've softened up a lot, but they just feel like bags. I can feel every part of the bags when I do the massages, and I don't much care for that. They feel like a science experiment instead of my boobies. I have no idea if this will change. I also am not impressed with them in clothes; they seem small. Naked is a different story; they look big and fabulous to me. I do love them, especially when I think back to my old bags of sag. I don't see my doctor again for another few weeks. I think at 7 weeks is when I'm scheduled. Updated on 17 Sep 2018: After one month, they have dropped well. They are a bit wider than I would prefer, but overall much better than what I had prior to BL/BA. I'm happy enough with them as long as I don't nitpick over the small things. They are so heavy, and that annoys me because it doesn't feel like the natural heaviness I had with my own pre surgery breasts. They very much feel fake. I can feel the bags. I hope this changes and they become more natural feeling. If not, I'm sure I'll get used to it eventually. It would also be nice if the numbness and tingling goes away soon. They are so sensitive to touch. Updated on 11 Oct 2018: I had my 7 week post op last week. Doctor is thrilled with my results. I'm a tad less enthusiastic. Don't get me wrong, they are a million times better than any stage of boobs I had prior except maybe just after my lift. I loved my post lift baby boobies before implants. The implants still feel foreign to me, but it's still early yet. My only real grump is that they are just so wide. I'm already a wide person, and these things are even wider. So much side boob! Im also not a fan of how they look in clothes. Im right back to where I was when I had huge natural breasts in terms of I feel fat. Also, I am so sick of wearing bras 24/7. He wants me in an underwire at all times. I find myself waking up in the middle of the night sometimes and stripping out of it. Speaking of bras, I am about the same size I was prior to my lift as all of my old bras still fit. They just look a lot better now. I did buy two 34DD Victoria's Secret bras, but they are a tad too small, nipples on full display in them. Going to have to go get sized for the actual proper fit. The feeling in my breasts is still weird, too. It's like a tingly, numb but not numb feeling. I don't know how to explain it really. Still no feeling in nipples. This update isn't oozing with positivity, but overall I don't feel too negative about them. I think it will probably just take some more time to adjust. I'm sure it's more that I am not a fan of my body in general vs. anything being wrong with my implants Oh, I also got some free botox for buying the implants I chose. I love the botox results! My wallet is in trouble now. Updated on 16 Dec 2018: It's been 4 months. I mostly am pleased with them, however, you can see and feel the bags on the sides. I do not like that at all. I also still have no nipple sensation. This surprises me as I had no issues getting sensation back after the lift where he cut and moved the entire nipples, but no sensation now after having the augmentation. It's weird. The breasts in general have an odd sensation. It is not pleasant to have them touched. Updated on 9 Apr 2019: It's been 1 year since I got my lift. I'm still thrilled with my lifted boobies. Less thrilled with the implants. The wideness of them irritates me, and there's not all that upper pole fullness I was seeking. Also my nipples are so high!
I recently had my one year tummy tuck surgiversary. I am so happy with the results. Dr. Koehler is very professional and an excellent doctor. His office staff was great and the facility was impeccable. I did not experience any problems or complications during my post-op recovery phase. My tummy is flat and the belly button is perfect. I feel that my results (scar, overall shape & contour) just keep getting better. I would definitely recommend him to my friends and family.
Great question. In some cases, BOTOX® can have a preventative effect, so it would make sense that you’re seeing fewer wrinkles pop up with long-term use. If you’re seeing results that last more than 3 to 4 months, there shouldn’t be a need to return for a touch-up appointment any sooner. If you’re not seeing lines on your face, you shouldn’t need more BOTOX. You might consider asking your provider why he or she recommends that you come in more frequently and whether the pricing might change one way or the other.

Caffeine consumption can cause dehydration, which can make the skin appear less voluminous and even a little “sunken in,” especially in areas like under your eyes, where the skin is thinner.
