Dr. Donath was recommended by a friend and within minutes of meeting him I knew I’d found “my doctor”. He is very professional, but also funny and relaxed. From the start he set very realistic expectations and had even my mother at ease about both the procedure and the results. From start to finish my experience was excellent. His staff is lovely and both his office and the medical facility where he performed my surgery was clean, comfortable and well staffed with amazing people. I highly recommend Dr. Donath! One of the best things about my experience is that he really had my best interest at heart. He was honest in saying “I’m not the guy who’s going to give you enormous breasts that make you look ridiculous.” If he’d let me chose my implant size, I probably would have gone at least a full cup larger. I kind of fought him on it, even waking up from my surgery I looked down and exclaimed “where are they?!” But in just a few short months I’d fall in LOVE with the size and they really are the most appropriate choice for my body.

Dr. Gary Donath has the best bed-side manner on the Central Coast; he is easy to talk to and always willing to explain the confusing. I am a 32 year old mom of two. I always wanted breast augmentation but waited to get consultations until I was finished breastfeeding and having children. I had two consultations and my husband and I both knew Dr. D was the one for us. Every question and concern I had was addressed and I felt very well taken care of. I was set to put my deposit down when we found out that Sientra was went off the market. I was devastated. It was hard to try to decide whether to go with a different brand or wait to see if Sientra would be released again. I was so set on Sientra that I knew in the long term if I didn't wait just a little longer for Sientra I would be bummed in the long run. But 3 months feels like so long to wait when you've finally worked up the courage to commit to surgery! My wait was well worth it and my Sientra implants are perfect. I tried to have children in my twenties and I last two pregnancies. I figure I've lived long enough now that I know I'm not having children so I'm focusing on my future without that possibility. I have always (since childhood) carried a load of fat in my mid-section even when I was running and going to aerobics as a lifestyle. Even as a child my mean Big Brother called me Jabba the Hutt and told me there were ditches because I laid on the ground (and many other mean things consistently for many years until my self-esteem was beaten to a pulp) so I am sure there's some mental scars there. Anyway, I eat very good. I can eat too much sometimes but no binging style eating and no fast food, candy, cookies, donuts...to explain how disciplined I am. My fat rolls are the type of fat that pushes up my boobs when I sit down. I always wear frumpy clothes to hide the rolls. I won't even tuck my shirt in even if it's encouraged for a company photo. I work in a professional office setting and I feel that my insecurities with a lot of professional outfits that keeps me from wearing them would be hurting my career growth. That probably sounds silly but I just look at my competition and it is a characteristic that sets us apart. When I starved myself to my lowest weight 10 years ago, my face arms and legs were skeletal and my hip bones stuck out and my ribs showed yet the shape of those fat rolls always remained just in a smaller version. That was incredibly frustrating. I could not get rid of them even if I worked out 2 to 4 hours a day. Sorry, I have a life and I'm not going to do that. So after many years of dreaming, I finally realized I don't have to be a celebrity to give myself this tummy tuck gift and my husband is very supportive. People are living longer these days we might as well make ourselves during the roller coaster ride. Yesterday at 1 Day post op I had some rough spots in my fresh recovery and I even threw up a little bit so that was a bummer. Here I am today, two days post-op and happy and spunky in a way; today I washed my hair in the kitchen sink and took the dogs for a very slow walk for about a mile. I have had enough surgeries (including back surgery and a breast reduction) to know that this kind of recovery isn't possible without a skilled surgeon. Yesterday I went in for my first day-after post-op appointment and the Doc took off my garment so I got to look at my pretty, smaller tummy for the first time. I like my whole self a little more for the first time possibly ever...fresh wounds, bandages and all. My shape is smooth...no more "lady lumps" as my husband affectionately called them. I took before pictures the day before the deed and when I get a chance to get after pics I will post some to share.
I have always had small breasts, but after breastfeeding two children for over a year each, my breasts were non existent. I never liked the way that clothes fit on my chest and felt that my body was out of proportion. After implants I am so happy with the results and getting dressed is so much fun!
On my 59th birthday I was diagnosed with stage 3+ invasive lobular breast cancer which was followed by a bilateral mastectomy and reconstruction. I had Seline implants and was having problems with ripples and dicided to see dr Garry and Made my decision to change to gel implants the best decition I ever made.. I highly recommend them. One of my eyebrows was higher than the other. Dr. Donath injected botox above my right eyebrow as well as along my lower forehead. It corrected the raised brow and kept my forehead from wrinkling too with excellent results. He did a super job! His staff has always been caring and courteous. The wait times have always been to a minimum to see the doctor. Very pleasant experience.
Tuberous breast anomaly is not very uncommon and board-certified plastic surgeons are trained to understand the nuances of the aesthetic challenge and the approaches for correction. Not all plastic surgeons are comfortable treating these patients as the anatomic features are often challenging and variable. There are, differing features that participate together, and should be addressed during your consultation. These include: breast ptosis (droopiness), large areolar size, herniation of gland into the nipple/areolar complex, breast asymmetry, constriction at the bases, infra-mammary fold asymmetries, poor volume, and skin laxity. No two patients are identical. You should ask your surgeon how many of these patients he/she has treated and specifically, which steps are needed for optimal correction. From you photos it is clear that you will benefit from a mastopexy (lift) and an implant. We often place the implant in a sub-glandular position for a true tuberous deformity to avoid a "double bubble appearance". It appears that your right breast displays more of a constricted base that the left side. Good luck and be a good listener during your visit !
"Bottoming Out" is a common problem in the breast augmentation arena and is related to several factors that are anatomic and physical. The data is clear and mirrors my personal experience; the larger the implant, the greater the risk. It is, however, not that simple. Submuscular placement is an advantage over sub-glandular position, but even when an implant is placed in a sub-pectoral pocket, there is no muscle coverage infero-laterally. Bottoming out can still occur! When implants are too large for a given patient, this accelerates the problem and less of the implant is covered by muscle, and therefore, there is less support. Lower pole support is the pivotal issue here. This is variable from patient to patient and is something that should be discussed during a consultation. Patient behavior and activity is a major contributor after surgery. Patients who are avid runners or ride horses tend to have more repeatd vertical displacement of the implant over time. I typically recommend consideration toward a textured implant in these patients as they tend to move less and do not stretch out the pocket. Bottoming out is rarely an issue with pure breast lift surgery, but is a common problem with breast reduction, particularly with inferior pedicle designs. This should be discussed with your surgeon! Remember, that surgeons can only control anatomy in the OR and not down the road in the future. You should never assume that this is your last breast lift procedure, or that you may not require revision surgery in the future. Good Luck!
800 cc volume is very large, regardless of your size/shape/anatomy. In 20 years, I have never seen a patient with that large a volume, including a breast reconstruction following a mastectomy. I can assure you that you will not have good muscular coverage due to the volume of the implant; the pectoralis major muscle will not cover very much of your implant. Poor muscular coverage and significantly large implant volumes have associated increases in risk for patients, including lower pole deformities, pseudoptosis, thinning of the soft tissue coverage, wound dehiscence and poor wound healing. I would strongly recommend that you speak with your plastic surgeon very carefully about this plan and the associated risks. You might also want to obtain a second opinion before you embark on such a journey. Good luck!
Round implants are only round in one plane. They are not spherical and the moderate profile designs have less projection than high profile counterparts. Thus, for any given volume, the moderate profile implants have less of a "round appearance". Placing any implant beneath the pectoralis major muscle (subpectoral) provides a more natural appearance and smoother transition as one moves from the infra-clavicular area toward the nipple. Therefore, implants placed in the subpectoral position appear less "round". The pocket dimensions also influence aesthetic appearance and the pocket must accommodate the implant so that it behaves naturally after swelling resolves. The capsule that ultimately forms (scar) will line the pocket and, to a great degree, determine shape and feel. Obviously, capsular contracture, which is a complication of implant surgery, tends to make implants appear more spherical. Most of my patients are interested in a natural appearance and I therefore use moderate profile Sientra implants. A patient who desires a "round appearance" should consider a higher profile design. My colleague's comments are correct, in that it may take up to 3-6 months for one to appreciate "final shape". I disagree, however, with the other comment that Mentor and Natrelle implants "...give a softer and more natural look...and Sientra implants tend to stay higher and fuller and give a more rounded look". Textured implants may tend to stay higher, particularly if they are positioned high during surgery. This is true for all 3 of the manufacturers.Sientra implants are in my opinion, the most cohesive implants, show no visible or palpable rippling and provide a natural feel, both due to the gel's cohesive properties as well as the gel-shell relationship. Good luck with your decisions and review all of your options and expectations carefully with your surgeon!
Hi - The photos certainly show breast ptosis. If your nipple is lower than the fold under each breast, augmentation helps a bit, but a small lift may be necessary. There is nothing wrong with augmentation with Sientra gel implants first and determining whether a lift is going to be necessary after a relatively short period of time.
