No she is not fully skeletally grown. She will be at least 18 before the implant is placed. You will want a zirconia custom abutment with an emax crown when the time comes

Crowns and veneers are the same price in my office, if I don't do a wax up. If its 6 units or more then I do a wax up which increases the cost. Both will last much longer and look much better than bonding. You still have a lot o years left so I would do it.

It sounds like you have decay under the crown that has made its way into the nerve. Its called irreversible pulpitis. The options would be an extraction or a root canal through the crown.

What dental surgery are you trying to qualify for? If its implants and you have Medicaid/Medicare in NC then the answer is no. They basically cover nothing in dentistry in NC.
