I thought I'd share my before/after results from my surgeries with Dr. Steinbacher. I'm now about 9 months post-op from my revision DJS and browlift, and since my DJS revision last September, I also underwent a genioplasty and septorhinoplasty revision with Dr. Steinbacher in February of this year, to correct polybeak deformity, as well as straighten my septum, and address the contraction of my alar base on one side from previous revisions. So there is still some residual swelling under my chin and around my nose in these current before/after pictures, however, even at this stage of healing I'm super happy with my results and the healing process, especially my jaw and nose revision, and can't wait to see how everything looks as it continues to heal and at a year post-op. I have a cleft lip and palate, and have had a multitude of previous surgeries and even revisions before going to Dr. Steinbacher, so I was hesitant to undergo such a intense revision surgery, particularly as revisions in general are more complex, and my history of cleft lip and palate and prior surgical reconstructions only added to that complexity. However, in comparing where I was pre-surgery with Dr. Steinbacher, to my results now, I'm so happy I did decide to go through with everything, and that he was able to successfully address my aesthetic concerns in addition to my functional needs. Additionally, he had to remove much excess fat grafting during my jaw surgery (which you can see really clearly in the 3/4 view photo how puffy my cheeks and especially the area around my lips was pre-surgery), and I'm so ecstatic to not feel like I have as much of a heavy upper shelf lip as before, even though I know it'll never be perfect because of my cleft. All in all, I finally feel so close to the end of my surgical journey and I couldn't be more grateful. My experience with Dr. Steinbacher and his team at West River Surgery has been nothing but positive, from planning my surgery to post-op care. I really can't recommend Dr. Steinbacher enough, both for complex revision cases, as well as reaching aesthetic goals. Over the course of multiple surgeries, Dr. Steinbacher performed revision double jaw surgery / orthognathic surgery, revision rhinoplasty, revision septoplasty, browlift, addressed aesthetics, facial bone jaw implants, genioplasty, fat grafting, laser, prp on me. I'm grateful to Dr. Steinbacher (and his team, who are also amazing!) for his care and extensive skill in delivering both functional and aesthetic results that I'm incredibly happy with and which have improved my quality of life and confidence tremendously.

I went to Dr. Derek Steinbacher in Guilford, CR (Not far from New York and Boston) for facelift, necklift, eyelids, brows, facial rejuvenation and facial aging and couldn’t be happier. Treatment also included laser, PRP, and stem cells and fat grafting. He is an artist as a plastic surgeon with impeccable credentials. He has patients traveling from all over the country and world in his waiting room. His office is so relaxing and serene, and his team and staff are so nice. The recovery was not bad at all, and he personally saw me during the early postoperative period and was always available for any questions.
Probably the most qualified in the entire country. THE BEST Here's my background: Had some of the worst sleep apnea imaginable, at least from the standpoint of how it affected me. Every single day of my life I was exhausted to the point where I'd constantly doze off for a split second driving to and from work. Keep in mind my commute is only about 30 min... I'd wake up feeling dizzy, my brain in a fog, very similar to a hangover. My life was miserable but I was petrified to have jaw surgery. After years of procrastinating I randomly stumbled upon one of Steinbachers YouTube videos and it intrigued me enough to do some more research into his background He's Ivy League trained, Harvard and UPenn, and does not only maxillofacial surgery but also craniofacial and plastic surgery. Of course the results are what matter most and his before and Afters were amazing Ended up making an appointment after he left Yale to start his own practice, and 10 minutes into our appointment I knew he was the right one. Not only did he have all the accolades you could ever imagine, he was a truly nice and soft spoken guy who listened. He never once tried to sell me on anything I didn't need, he carefully thought about every single question or suggestion I had. I was even more encouraged by the fact that he has a 3D CT Scan machine in his office and he planned my entire surgery mathematically, to the exact millimeter. I came to him with a very short, compact face, none of my top teeth would show when I smiled, and I needed large movements to solve my issues and open up my airway. He brought me down about 10mm and probably close to an inch forward. Had my surgery last summer, I'm about 7 or 8 months post op at this point and my life is changed, I can breathe at night for the first time in a decade. Years ago I had been injured by a different surgeon, and since Dr. Steinbacher also does craniofacial work I'll be going back to him in a few months to correct those issues as well. It is so tough to find a good doctor, especially someone who's a genuinely good person who wants to help and isn't greedy. Dr. Steinbacher came highly recommended to me by my dentist for surgical implants that were needed before a dental prosthetic could be created. Due to a congenital defect, I was considered to be an extremely complex case. Without the well placed implants, my prosthodontist would never have been able to create such a perfect prosthetic device. The result of their combined efforts was incredible and far exceeded my expectations given the intricacies of my mouth. I highly recommend Dr. Steinbacher without reservation!
Earlier this year, my teenage daughter had a cheerleading accident and she had broken her nose. We saw Dr. Steinbacher and not only to ensure us her nose be fixed, but also assured us she would breathe better. Dr. Steinbacher was very thorough in the entire process. He found she had a deviated septum as well. He was very kind, compassionate and explained the procedure in detail. He made my daughter and this nervous mom very comfortable. Finding Dr.Steinbacher was definitely the luckiest thing in my life(best academic background and double board-certified). I have always been self-conscious about my weak chin since I was a child. About 15 years ago, I did chin implant which is totally a disaster. After few months research, I quickly decided to choose Dr. Steinbacher to help me remove my implant and do sliding genioplasty. It has been 7 months since the surgery. I am very very very pleased with my results. My new chin not only changed my facial profile and but also balanced of my face. I can't put into words to describe how happy I am! Overall, Dr.Steinbacher is a GENIUS! He is 100% the best doctor in the world! He will be my doctor all my life!
Narrowing your ala would not necessarily lessen your nasolabial folds. Removing skin of the nostrils where they meet cheek helps the flare and width of the nose. The nasolabial folds would be well treated with filler or fat. You may also want to consider an upper lip lift -- depending on how much tooth show with smiling.
Genioplasty would help give your chin more projection, and vertical height. It may help lip incompetence too. However, you should check your occlusion, and if you have a malocclusion (class II, and or open bite) full orthognathic surgery may be the better choice.
