It depends on the size and the color of the tattoo. if it is tiny a simple excision in a person that heals well is fine. However larger tattoos are best removed by laser. Remember when getting a tattoo that the darker colors such as black and green are easily removed by laser and the lighter colors are much more difficult .

Preferably it would be best to wait for an elective procedure such as ear piercing while on accutane. healing can be impaired with the potential for a worse scar or even a keloid. I recommend to wait a few months after accutane as this is not a life or death procedure.
