It depends on the size and the color of the tattoo. if it is tiny a simple excision in a person that heals well is fine. However larger tattoos are best removed by laser. Preferably it would be best to wait for an elective procedure such as ear piercing while on accutane. healing can be impaired with the potential for a worse scar or even a keloid. I recommend to wait a few months after accutane as this is not a life or death procedure.
