Hering's effect after unilateral ptosis surgery - if I correct my good eye will the originally operated on eye droop again?

Asked By:AnonymousANSWERS (1)
Hi greetings from the UK. Yes you can develop contralateral consecutive ptosis in your previously operated eye but typically it's likely to be minimal. You can ask your surgeon to try to simulate it by performing a phenylephrine test. Best Wishes David

If you had incisional double eyelid surgery and the crease is too high, can it be released non-incisionally? (Photo)

Asked By:AnonymousANSWERS (1)
Hi greetings from the UK. No a non incisional suture technique won't help you. Your situation is very difficult to treat predictably. What you're talking about is trying to create a new surgical crease lower than your current higher crease that you don't like, ablating the previous crease, herniating what available orbital fat you've got left to re inflate the superior sulcus hollowing and then hoping that your previously created surgical crease that you don't like doesn't reform. It's technically challenging, moderate in risk and unpredictable I'm afraid and most sensible oculoplastic surgeons would advise to leave them alone. Best Wishes David

Can an eyelid lift be done safely if the patient is on one aspirin per day?

Asked By:AnonymousANSWERS (1)
Hi. Greetings from the UK. Just to say that yes we do ask our patients to cease aspirin prior to oculoplastic surgery if possible especially for our higher risk operations when excessive bleeding would be dangerous e.g. skin graft surgery. However for our patients in whom they cannot safe stop their aspirin safely e.g. coronary artery stent patients requiring long term aspirin, we would still offer them the option of surgery whilst still on the aspirin 75mg once a day but just warn them that there would be a slightly greater risk of haemorrhage. Good luck!

Should I get a blephroplasty or endoscopic brow lift? (photos)

Asked By:AnonymousANSWERS (1)
Hi greetings from Birmingham! It depends very much on what you're trying to achieve. If you're happy with the position of your brows but just want increased upper lid show then an upper lid blepharoplasty operation is a pretty reliable way of increasing upper lid show. Ideally I would recommend that you seen an oculoplastic surgeon who will examine your eyelids properly and find out why your eyelids still look hooded. It may be due to the position of your orbital septum insertion, orbital fat and your skin crease being low and if this is the case then an anchor blepharoplasty to change these factors would likely work well for you. Indeed this is more likely to be the case if you find that upper eyelid tape works for you. Sometimes my patients attend my clinic bringing eyelid tape with them. If there is a particular look that they like with the tape in, then I try to recreate that surgically. Of course hooding may be due to brow ptosis causing upper eyelid skin hooding and if this is the case and you're unhappy with your eyebrow position then sure, redo brow lifting may be a preferred option instead. Good luck and best wishes David

What is the best Eyelid Surgery procedure for mild case of Ptosis? (photo)

Asked By:AnonymousANSWERS (1)
Hi. Greetings from Birmingham. Yes your oculoplastic surgeon is right. In patients with minimal amounts of ptosis the margins for success are much narrower. i.e. for the patient to be happy they've got to be pretty perfect. Any minimal overcorrection or under correction and they're more likely to be disappointed. No I wouldn't agree that the risk of disappointment outweighs the chances of success and a happy patient though but the risk to benefit profile is more adverse though for reasons described above. Treatment options include doing nothing (safest option), lid elevating drops (difficult to get in the UK as not licensed currently) and surgery. There are multiple ptosis operations with pros and cons for each approach. No- simple blepharoplasty surgery wouldn't work for you. Ensure that the surgeon you see does a lot of ptosis surgery as your primary operation is usually your best chance for long term success and also ensure that he/she does a phenylephrine test too in your surgery planning. Good luck. David

Am I a good candidate for a unilateral upper blepharoplasty? (photos)

Asked By:AnonymousANSWERS (1)
Hi. Greetings from the UK! Yes you would benefit from having a unilateral oriental blepharoplasty. They’re more simple than bilateral as from a surgeons perspective all we need to do is clone the other side for you. You need to ensure that you’re seeing someone who does a lot of them as the Asian eyelid is very different to a Occidental eyelid and is it very specialised. Your best chance for long term success is the primary operation done properly and well so make sure you research your surgeons and review their previous example cases. You may have a ptosis too but one can’t tell that from those photos. If you do then that can be repaired simultaneously. Good luck! David

Where does the anchoring sutures go in Incisional double eyelid surgery?

Asked By:AnonymousANSWERS (1)
Hi Greetings from the UK. It can be either. From a surgeon’s perspective into the tarsus takes less skill and you can do that via a DST type approach to give a static skin crease. Into the aponeurosis takes more skill, needs a full width incision usually but would give you a more natural dynamic skin crease

Double eyelid surgery to correct asymmetrical eyelids: surgery on one eye? Do I have ptosis? Incisional technique? (Photos)

Asked By:AnonymousANSWERS (1)

Hi greetings from the UK! Yes you do have a mild degree of left upper eyelid ptosis with multiple poorly formed skin creases. If you are happy with the appearance of your right eye then yes you can have surgery on the left upper eyelid only to try and match the right side. Ideally you should see an oculoplastic surgeon with some experience in Asian blepharoplasty but most most oculoplastic surgeons should be able to help you. It's easier if you do go for unilateral left upper eyelid surgery only as then all they need to do is essentially clone your dominant crease on the right side and tighten your levator aponeurosis only. Yes most oculoplastic surgeons (like myself) would do this all incisional from the front, whereas some would do this via a posterior approach and a DST type technique at the same time.

Best Wishes

David

What is wrong with my eyes/eye area? Please excuse eyebrows, all the salons are closed! (Photo)

Asked By:AnonymousANSWERS (1)

Hi greetings from the UK! I've got to agree with Dr Steinsapir too. I don't think your eyes look abnormal at all. Yes you do have a minimal degree of asymmetry with a tiny degree of ptosis affecting YOUR left upper eyelid  but that's pretty normal to have a degree of asymmetry.

Put it this way, if you'd attended my clinic I would have tried to reassure you that everything looked pretty normal, that they look great and individual to you. I'd strongly try dissuade you from having anything done at all.

Having said that though, Dr Taban's work on almond shape eye surgery/ lower lid recession and lateral canthoplasty does look beautiful too.

All the best. By the way don't feel too bad about the natural eyebrow look. It's become very fashionable suddenly and we're seeing it everywhere for some reason...

Does anyone have patients who travel to see them or travel themselves to do surgeries?

Asked By:AnonymousANSWERS (1)

Hi. greetings from the UK! First thing to say is that ptosis surgery is a 'bread and butter' procedure for oculoplastic surgeons. It's what we're very much trained to do and so most oculoplastic surgeons of any experience will be able to help you with this condition. Having said that as most ptosis surgery is typically done as a day case procedure under local anaesthetic then it's not uncommon for patients to travel to undergo treatment for it. Indeed we get patients flying in  for their consultation and their surgery from mainland Europe so ptosis is  very much amenable for treatment as a 'health tourist too'. Best Wishes and good luck

David

Amazing Transformative Surgery

I recently had an upper and lower Blepharoplasty procedure performed by Mr David Cheung, Consultant Ophthalmic and Oculoplastic Surgeon, and I couldn’t be more delighted with the results. Three months post-operation there is absolutely no visible scarring or evidence of the procedure itself – the outcome has been truly transformative and has greatly enhanced my quality of life. From start to finish, David was caring, professional, and reassuring, ensuring I felt confident and well-informed at every stage. He is supported by a hand-picked team who made the entire process seamless and trouble-free. The skill, attention to detail, and genuine patient care I received have been outstanding, and I cannot recommend David highly enough to anyone considering this procedure.

The procedure was quick and painless - couldn't be happier

Highly recommend Dr Cheung. I had lower eyelid surgery and couldn’t be happier with the results. The consultation was thorough and relaxed with a check of the health of my eyes to ensure I was suitable for the procedure. The procedure itself was quick and painless. After the surgery, Dr Cheung was on hand to answer any questions I had and I also had the opportunity to contact an out of hours nurse. The whole experience was professional and unpressurised, a stark contrast to another consultant I had seen who took a picture of my face and proceeded to pull my appearance to pieces and then try and sell me face cream afterwards. Alongside this, Dr Cheung’s fees were considerably cheaper than London based consultants.

Best Thing I've Ever Done - bilateral lower eyelid blepharoplasty

I will never ever be able to thank David enough for what he has done for me. Four weeks post operation bilateral lower eyelid blepharoplasty, and it has exceeded my expectations and given me my sparkle back. Friends and family say that it has taken years off me and I am no longer upset when I see myself in the mirror. I would also like to thank the anaesthetist, nurses and staff at the Westbourne Centre for taking such great care of me on the day. It truly was a pleasant experience and not what I had imagined in my own mind. I healed extremely quickly, had no pain and bruising was virtually undetectable within a fortnight. From the initial consultation with David, he made me feel at ease. He took the time to explain everything thoroughly so that I understood exactly what was going to happen on the day of surgery and post surgery. I was a very anxious patient and had plenty of questions for David which he always answered promptly by email. If anyone is looking to get this done, I would say go to David and just get on and do it. I wish I'd had it done years ago.

My only wish is that I’d done it sooner!

I have long known that the one elective surgery I would undertake was blepharoplasty, due to drooping lids at a young age. I chose to have a consultation with David after undertaking research.  There was no question of me going abroad to have any procedure done.  It may be cheaper,  but the potential for something to go wrong, the language barrier and aftercare needs meant staying in the UK was paramount. I went to David with thoughts of upper and lower blepharoplasty (dark circles under eyes).  David was very friendly,  easy to talk to, explaining about every element of the procedures, taking measurements,  doing drawings, really listening to me and understanding my reasons for wanting the op.  Crucially he explained why I was not a suitable candidate for lowers and why alternatives (e.g. fillers) would also not work for me. Being very risk averse I went away from the first consultation armed with lots of information supplied by David and decided not to go ahead.  After 18 further months of deliberation and research I decided to go ahead and booked a further consultation. Shortly afterwards I undertook the procedure at the Westbourne Centre in Birmingham.   From start to finish of the whole process I cannot fault all of the staff I encountered.  All so very polite, friendly and caring, putting me at ease. Throughout the procedure David was his usual calm, reassuring,  friendly and humorous self.   I had approximately 13mm taken from each upper.   I have a medium tolerance to pain and can honestly say the most painful part was administering local anaesthetic to the inner corners of the area nearest my eyebrows (and even that wasn’t so bad).  During the operation I experienced no pain,  just a sensation that something was happening around the area.  I concentrated on staying calm and listening to David's eclectic operating music list and chatting to him and the theatre staff. Post surgery I was very well briefed on what to expect and instructions to follow (which I did to the letter).  The rest of the day after surgery was uncomfortable but after that,  no problems.  It actually looked worse than it felt. I am now 5 months post op and the results of the work are nothing short of amazing.   I no longer look tired and angry,  I wear only the lightest of eye makeup to achieve the look that I couldn’t achieve before and it has bolstered my self esteem no end.  Thank you so much David, I would (and have) recommended you to several people for a job well done.  My only wish is that I’d done it sooner!

He clearly has an understanding of the "natural asian look"

The consultation was extremely detailed, and lasted over an hour with him meticulously measuring and checking the structure of my eyelids. This was was followed by him explaining the anatomy of the eyelids and explaining the risks of surgery carefully. As a specialist in Ophthalmology, he was clearly an expert. He has a very relaxed & non-judgemental demeanour. Following the consultation, he was very happy & quick to communicate via email and answer all my questions to make sure that I was 100% sure about everything. He was very efficient and was able to offer me a surgical date as early as 3 weeks after the initial consultation. The surgery was with local anaesthetic, and very quick (maybe 30-40 minutes) and casual, it almost felt like getting your hair cut. And the nurses were very friendly and helpful. The major downtime after the surgery was just a few days. Although swelling persisted for three months, the results after look good and natural. He clearly has an understanding of the "natural asian look", unlike many others who seemed to want to try to westernize the eyes. I highly recommend Dr Cheung for any cosmetic eye surgery.

Thanks So Much! This experience has done wonders for my confidence

Mr Cheung was lovely! I had a upper and lower lid blepharoplasty under twilight sedation at the Edgbaston hospital 6 months ago. I felt that I was in safe hands after my friend's mom needed cancer surgery to her eyelid at Russells Hall Hospital and I saw her result. I had a remote Zoom consultation with David last month. I so wanted to show him what the final result was but this pandemic put a stop to that. I so want to show my pictures and although this experience has done wonders for my confidence, I'm still not brave enough! Anyway thanks so much David!

Specialist surgeon with a very friendly personality

Right from the initial consultation, David put my mind at ease, explaining all of my options clearly, along with pro's and con's for each. There was no pressure at any point to opt for any of the procedures, and all consultations were very relaxed. We had a few consultations leading up the surgery (Frontalis Suspension), each one giving me more confidence that I'd chosen the right Surgeon. The day of the surgery was again, very relaxed, which eased the slight nerves. David and his team were great throughout, providing commentary throughout so I knew exactly what was going on at all times. The surgery itself was slightly uncomfortable, but the only pain was from the initial anesthetic injections. Post surgery, I was given all of the medical ointments that I'd need, along with some advice for the following few days. The package also included a number of follow-up aftercare appointments, which again, David was great with, explaining each time the healing process, and what I could expect between the current and the next appointment. Overall I'm 100% happy that I made the right choice in choosing David, and wouldn't hesitate to recommend him to anyone needing surgery, or just a friendly consultation to explore their options.

A brilliant experience —thank you Mr. Cheung.

I cannot rate Mr Cheung highly enough. I’ve just looked at my before and after pictures and I am so, so pleased! I knew I was in great hands from my first consultation - David was knowledgable, kind and honest when explaining my options. He is fantastic. Thank you, David.

I am delighted with the results!

From the outset I felt totally at ease and confident with the advice and care I was given. Dr Cheung was friendly yet professional and responded to my emails promptly. The operation and recovery went smoothly and I am delighted with the results 3 months post op.
