Where as I’m reluctant to post a before photo of myself I feel it’s necessary to do so just to highlight to everyone what an amazing surgeon Dr Lev is as my before, after photos will testify .I live in the UK but having had a facelift with Dr lev ten years ago I was willing to travel the long distance again as I was so pleased with the previous facelift I had . I had lost a lot of weight in September 2 and a half stone in order to get healthier and fit into the wonderful clothes I own but could no longer fit into . Loosing that amount of weight three stone in total since may made my face sag and I was very unhappy with my under eye wrinkles . Not crows feet but terrible crepiness below my eyes. Dr Levs coordinator arranges everything from forwarding your before photos without makeup front and side to Dr Lev” s recommendations there is a video consultation available for a small fee . You then let Didi know if you want to proceed with the surgery and find available dates . The only thing you have to do is arrange the flights, pay the Deposit and choose which of the recommended places you wish to stay at . I chose Verdeza I can’t speak highly enough about them the staff are genuinely absolutely gorgeous so friendly and helpful the meals are delicious which I ate at the restaurant before the surgery but from then on I chose room service and they were all brought to my room an hour before which a lovely staff member shows you the menu for you to choose. The meals are all freshly prepared and delicious and healthy. When you arrive in San Jose a taxi is waiting for you that took me to Verdeza it was late at night but on opening my room a beautiful meal and dessert, juice , coffee was awaiting. You spend a day there we walked the two minutes to the designer village beautiful Starbucks where we took advantage of the beautiful weather. The following day a taxi is at the foyer waiting to take you to your final consultation with dr lev . Some people just want to look refreshed and don’t want people on returning home to think they have had anything done just look amazing but can’t put their fingers on it . I’m from the camp of make me look as young as is humanly possible. Obviously when you get back home people are going to realise you look 20 years younger that’s not a problem for me . The following day you are picked up for your operation and return back at your place of stay the following day . Verdeza staff looked after me extremely well bringing my medication checking on me and bringing my meals on the third day they help you to shower washing and conditioning your hair for you . All appointments to dr lev are arranged and your taken from the foyer to see dr lev and returned . After a few days my daughter and myself resumed our short walk to Starbucks with the help of sunglasses. When I felt well enough the staff in reception ordered me a taxi and we went to the massive designer village five minutes drive away which then became a daily habit . Taking it easy but the clothes in Zara in San Jose are amazing I could hardly close my two suitcases . I would stress that if you stay at Verdeza it is never cold I packed two wool dresses, two wool jumpers and a dressing gown all were not necessary and I’m mrs cold . The weather is so nice you only need t shirts and leggings I did take one zip up light fleece on operation day as you do get cold after an anaesthetic. If you stay at the other recommended places it might get colder in the night but in Verdeza I had the overhead fan on and the cool setting on the air conditioning . I also met some amazing residents at Verdeza who had the most incredible life stories and what they have achieved and like I said the staff were gorgeous. Well needless to say I am ecstatic with my results. The initial photos I sent back home to friends thought that I was having a laugh and had applied a face app to make myself appear decades younger . I assured them they were taken with my phone with no alterations. I had a deep plane facelift this time which Dr lev introduced three years ago before that I had a smas facelift ten years ago with him which is also long lasting but not as long lasting as the deep plane it is known as the “ one and done” facelift. The reason for having this facelift was because of the dramatic weight loss which made my face sag and the fact my other facelift was ten years ago . I was delighted with the previous one that’s why I decided to return all that way for another.

Hello there. October 25th, 2023- I have just decided on a doctor and I wanted to start posting things for my review. This is the beginning of my journey..I have chosen Dr Alejandro Lev MD from Costa Rica. So far all of his reviews had been amazing. His work looks seamless. This is a scary process and the knowledge I have gathered from resources tell me I'm in good hands with Doctor Lev. So far I have reached out to his assistant Didi and she is amazing. That's all I know so far but will keep posting as the weeks go on.
Nov1 -I have been selected to be patient of Dr Lev for a facelift. Didi give me a few options for dates and I have chosen Nov 29th.
Jan18th- 7 weeks post op.. I want to share my story with all of you trying to make a decision about who to go to. The answer: Dr Alejandro Lev in Costa Rica.Price was under 9k and soooo worth it.
I found my doctor here on the real self… I did so much research prior because going out of the country seemed a bit scary at first thought. I’m so thankful that I chose Dr. Alejandro Lev and his after care of "Mercedes home." This surgeon treat you like you’re his only patient… So loving and kind. Dr. Alejandro Lev takes extra time for everything and anything you may need. He is absolutely amazing and truly cares about his patience. He greets you with a hug and is so sincere when he listens to what you re wrong to get out of this surgery.. what exactly do you want. Everyone is different and he wants to get it "just right" for each and everyone of his patience. He is amazing! H is "perfectionist" with hi surgeries. He has Ben doing plastic surgery for over 30 year and le me tell you.. he knows what he is doing. He is the ONLY choice. For real. From the moment that i landed in Costa Rica I felt right at home. The aftercare facility that I chose was called Mercedes Cassa… She has been taking care of Dr. Levs patience for over 20 some years. Her place is just a few block to Dr Levs office. So convenient.
When I arrived at the costa rica airport, there was someone waiting for me with a sign with my name on it. I was never scared or worried.I landed on a Monday… Went and checked into the place that I had chosen ahead of time… Mercedes Cassa...sh sent the driver for me to be picked up. ( options are in his website "eternally vain")Tuesday I had my consultation with Dr. Lev and Wednesday morning I had my surgery.Eliecer my driver brought me in and got me checked in.I chose the cheaper option and used the local clinic versus a nearby hospital option and that saved me about $2000 by using his local clinic instead of the hospital in a nearby town. I felt completely safe and clean. There were two nurses that stayed with me in my room all night long for post operation care for the fort 24 hours.Thursday morning I woke up Dr. Lev spoke with me ...checked everything out and make sure that I was good and then my driver, Eliécer, picked me up and brought me back to Mercedes Casa the after care facility.Mercedes (Mercy was what we call her) was extremely knowledgeable and knew exactly what I would need from moment to moment and day to day.. it was amazing after care with her.I rested all day Thursday and then on Friday they took off the hard helmet and put on a softer wrap. I rested again on Friday and Saturday, and by Sunday I was feeling good enough for a quick trip to the market… just to get out of the house for a bit.I had several appointments with Dr Lev to remove stitches and just wellness check on me. This doctor I so amazing he gave me his personal phone number and has called me several times after I got back to the states to just check on me. This man is wonderful and very sincere.
There is an amazing driver, who picked me up and took me everywhere I wanted to go during the entire two weeks… All of my appointments… Trips to the market… Even a touring trip to the original Starbucks coffee farm that’s only a quick 30 minutes from Mercedes casa. His name is Eleazar and he’s absolutely delightful . Super sweet.. fluent in English and has lived in this town his whole life.. you are in very very safe hands the entire time you are there. I am telling you this because I just went threw this and I realize how scary it is to think of leaving the country. Well this Doctor … the way he has everything you could ever need already prepared and set up for you....there is absolutely nothing scary about Cost Rica. This surgeon has been doing plastic surgery for over 30 years and he’s extremely good at it. I would say that Dr. Lev is an artist for sure. Just check out all of his before and after a on his website "Eternally Vain" I would trust his hands on my mother on my sister on my daughter. That’s how good this man is.I have to Xpress just how amazing Mercedes and Ingrid were. These are the caretakers at Mercedes home. This place is so clean… Ingrid is constantly wiping and mopping and sweeping and keeping things sanitary and sparkling at all times. This home is huge and Mercedes is an amazing home care and recovery individual .Mercedes has been caring for Dr. Levs patients for almost 30 years. She knows what sort of vitamins your body needs during recovery and has 15 ways to prepare them for you. From breakfast with a fresh beet and carrot juice delight… to complete protein low fat dinners… she is a former chef with the knowledge and experience under her belt that you will never find a better aftercare facility than hers. She knew I would need smoothies.. fresh fruit from the market .. every morning for the first 2 days. I always felt so taken care of and loved threw the entire process. She knew what and when I needed something way before I even had a clue. She is so kind and knowledgeable…absolutely amazing experience. I stayed with her for 14 days total and it was such a BLESSING! I loved this place and her wonderful supernatural care for me that I CRIED when I left that place. I will tell you she has even texted me and checked on me since I have been home… now that is a truly LOVELY woman.Everything went exactly the way it was supposed to… Praise God… I would tell everyone that I know to trust their lives and face with this doctor and his aftercare team. The best of the best. You will want for nothing.… It is an all inclusive aftercare with Mercedes and her helper Ingrid. She will have you back on your feet in no time I know that my beautiful healing progress was directly related to the wonderful aftercare that I experienced.. the peace of mind …the proper food… the clean environment… And the organization that goes on between Mercedes and the doctor is quite something.You can request to stay with her threw their website on “Eternally Vain” and when you set things up threw DiDi.. DiDi is another super wonderful part of what Dr Lev has put in place for your visit to Costa Rica.Look up his website… Eternally vain… you will see that every question you have will be answered by his lovely and extremely amazing liaison Didi. DiDi is your number one go to girl… From first contact until you land in Costa Rica… She is Johnny on the spot with your all your answers and reminders to get things done and in place prior to your adventure across the world to Costa Rica. DiDi is delightful and never let me down… She is your number one cheerleader that has dedicated herself to getting you prepared and to the surgery date with ease and peace. She is amazing… D.d. does most of Dr. Levs website and truly truly knows what she’s talking about… PLEASE TRUST HER!
I will say this.. bring cash for tips that don't have any TEARS on the bill.. ( it is a thing over there). And make sure you take care of the ones that took care of you. They would never ask.. but boy do they deserve it.

I wanted to say I also got a chemical peel around my mouth while I was under ($500.00) it took the wrinkles out above my lip. This was the first time I ever got a chem peel and It is pretty strong. The skin healed in about 10-14 days but stayed a SHADE of red until last week.. (5weeks) you can easily put make up over it.. I just don't wet make up so I didn't. But it now looks like the rest of my face skin color.

I will post a few pictures in here... these were while I was in the aftercare facility except the one in blue shirt.. that one is the airport picture and is actually on day 10 post op... on my way back home. So that's how healed the Chem peel was when I left for home. None of these pictures I post will have any make up on me except for ONE of them in the original review has eyeshadow.. all the rest I am not wearing a drop of makeup.

The hairline incisions was m option because of my large forehead I couldn't do the cranial cut. Dr Lev did a fantastic job and the hair is all ready regrowing and covering it at 3 months.

Everything is healed. I feel my neck muscles are loose again.. I have full range of motion. My hair is growing back great. No scars on my ears. Dr Lev is amazing doctor. A very close friend and I spent time together in October of last year. I seen her this week.. we spent everyday together for 2 weeks and she still has no idea what is different about me. I look perfectly natural and normal. She knows I've lost sone weight but m procedure has not even crossed her mind. I feel fantastic and healthy.
Thank you Dr. Lev for the tiny stitches and care you put into making me the most beautiful me I've been in many years.

I gladly add my voice to praise for Dr. Lev. He is an artist and magician, and possesses a marvelous sense of humor that puts you immediately at ease. My before and after photos confirm his skill as a leading Costa Rican plastic surgeon. I am lucky and thankful to have found him.

Also, I must mention my wonderful stay at Casa Mercedes. Mercedes takes care of her residents during their recoveries in so many ways. She sees to their health and comfort in a totally supportive fashion. I could not have asked for a kinder and more competent caregiver. Casa Mercedes is a lovely house with individual bedrooms and bathrooms. The garden and patio are pleasant and are a super place to hang out. In addition, Casa Mercedes is conveniently located 1-1/2 blocks from Dr. Lev’s office and a large supermarket.

To sum up, I had a very positive experience in Costa Rica, and do not hesitate to recommend Dr. Lev or Mercedes.

I obsessed over Dr. Lev’s work for years, and when I sold my house and had the cash, I decided it was my turn! He is such a wonderful surgeon and human being. I experienced no pain (and not a ton of discomfort), and the results were just what I wanted!
My experiences with US plastic surgeons hasn’t been great. I’ve mostly found them to be condescending, arrogant, incredibly expensive, and generally of the opinion that they deserve more space on the planet than everyone else. Dr. Lev is night and day from that. He’s kind, responsive, eager to do great work, and a lot of fun! He answered my emails promptly, always ending with “abrazos” (hugs).
But even if he were an evil gnarly troll, in my opinion, his work is just better than anyone else’s I’ve seen. So happy I got to be his patient! Love his staff too!
(I’d also highly recommend staying at Chetica for your recovery center. Private cabins, amazing food, constant, loving pampering, and 80 acres of rain forest! The host, Ruben Dario is a hoot, and the whole staff is wonderful.)

I thought a few more pics from the first two weeks might be helpful.

It has been six weeks since I had a full facelift with Dr. Lev. I don’t know what to say except that he is a true artist and gentleman. Dr. Lev turned the clock back 15 years on my face, but the result looks so natural. My friends and family are amazed by the nearly imperceivable scarring and swelling after such a short period of time. He is also just a truly kind and caring individual — a real pleasure to work with. I highly recommend Dr. Lev and his full surgical team, including his anesthesiologist, Dr. Greenwood.
If you are traveling internationally, Dr. Lev requires a two week post-surgical stay in Costa Rica. Of the three aftercare facilities he recommends, I chose Casa Mercedes.
Casa Mercedes is conveniently located just a few blocks from Dr. Lev’s office. The care I received from Mercedes Castro and her staff was excellent. For one low price, she provides a private room and bath, three healthy meals, transportation to and from the airport and medical appointments, assistance with medications, and so much more. Her experience with the recovery process just cannot be duplicated at home. Somehow, she makes recovering from major surgery feel like a vacation!
Simply stated, my experience with Dr. Lev and his whole team was wonderful.

Hi Folks:
I am 14 mos. post-surgery, and my facelift from Dr. Lev is holding up well. As you can see, I am 64 years old and look a lot less tired than I did at 63. For those who wish to know if I would do it again, the answer is yes!

Dr. Lev did a beautiful job of taking away the things I hated. At 68 years old, I wasn't hoping to look like a 30 year old. I was mostly concerned with the sagging neck and jowels along with my lower eyelids. Dr. Lev recommended and performed a lower face lift and lower blepharoplasty. I wanted a natural look that wasn't going to scream 'facelift' to my children and grandchildren. That's exactly what he gave me. Six weeks after surgery, I attended my granddaughter's wedding and was around a lot of family. I got comments about looking great but that's all. I highly recommend Dr. Lev... you'll be in very good hands. He is very thorough, kind, professional, and takes personal interest in your happiness.

Her home is conveniently one block away from Dr. Lev center. You have a problem you're not a bumpy ride an 1/2 hour away he is right there literally in your backyard!
There were many recommendations to me for places to stay. I am so happy Mercy had an up had an opening for me! I truly believe God placed me there.
Now if you read this far I hope you read the rest. Dr. Lev's compassionate and understanding! A true artist! This is not your cookie cutter push you through to the next number kind of outfit. He listens he understands and he can see the things you cannot.
He will not do Procedures you don't need. He will not do most multiple procedures that are on safe. Your Care and safety is at the top of his list. I love him so much I will be back!
finally if you're still here I was warned of all of the terrible things that could happen going out of the country to have surgery. Don't Believe them best experience ever! The first thing I can tell you is don't believe everything you hear. Bring your credit card it is usable bring your cash it is usable. Be aware as you would at any place. Eat love pray ! I walked all around everywhere downtown, high-rises, parks, everything bakeries and had absolutely no problems everyone was kind and loving to me.
Also people think Third World country nothing could possibly be going on there but you would be wrong they are SHOPPING! (and usually I hate to shop) is bar non way better than anything I have near my home!
Five stars seems way to little for my experience!

He was very understanding of of my fear of being poked a tun of times because I am a very hard stick. He put the gas mask on and I went to sleep and he went to work. Two try’s I never felt it. When I woke up it was over! Yay! I did have some pain but he fixed that as well. He is very good and comforting! Loved him too!
P.s. I don’t think anyone has ever gotten me in two tries.

Pros love the look I didn't want it over done and I didn't want every wrinkle removed. I wanted to look fresh and not tired and that is exactly what was delivered by Dr Lev!Pro Mercedes place to stay recover and feel fabulous!!! She is the best!!!!!! Spend the money it is worth ever penny!Cons when you lose weight is isn't good for a face lift. I did not try to lose weight I got sick. I am better and at almost what I was pre surgery.

Yep I did the right thing for me. This may not be for everyone but for me it is the best thing I could have ever done for myself. Love visiting Costa Rica would definitely go back! If you’re thinking it might be good for you too then look into it.

We sincerely recommend high-quality cosmetic surgery done by Dr Alejandro Lev Schtirbu. We just returned from our medical tourism to San Jose, Costa Rica, and we are extremely pleased with the results only 4 weeks later.



Dr Lev performs many different procedures. I had a full facelift and my wife, being already close to perfect, only the lower face and neck. Essentials we found with Dr Lev in Costa Rica: his surgery is exceptionally high quality as evidenced by lack of scarring, very low risk as proved by his patient history, much less total cost compared to Canada or the US, proximity and time-zone convenience and the benefit of a couple weeks of pleasant R&R (recuperation and rest).



First, Dr Lev is a dedicated and scrupulous craftsman with scalpel and suture. The surgical clinic where we chose to have our procedures is basic and sterile but without the usual North American decoration and design features. It is a quality facility and we rate the overnight aftercare as outstanding compared to that received in Canada for similar procedures in certified plastic surgery centers. You can also opt to have your procedure in the modern Hospital CIMA for an extra fee, just as in Canada you can use a hospital if you desire for a fee. The anesthesiologist, Dr Greenwood, has collaborated with Dr. Lev for many decades, an experienced doctor and fine person who devotes ample time to his patients. Together they perform surgery on just one patient per day. We had a pre-op consultation, next day surgery for 5-6 hours, overnight observation in the clinic, followed by 7 or 8 post-op examinations. Dr. Lev reviews the sutures and staples, removing them at just the right moment to reduce possibility of scarring. We had two very kind English-speaking nurses monitoring our post surgery status.



Second, Dr Lev’s supremely able International Patient Coordinator is Didi, who advises patients in detail how to plan their medical visit to Costa Rica. The process explained on the website is well organized, but you, the patient, must take responsibility for reading, planning and executing. It is wise to adhere to Didi’s advice and recommendations. Read everything on the Eternally Vain website, then clarify with Didi. She is endlessly patient and helpful.

Third, Casa Mercedes, the recovery residence where we chose to heal, is first class for making all aspects of our stay convenient, with expert and caring assistance during recovery. Casa Mercedes is owned and managed by the highly professional and helpful Mercedes who has been managing Dr. Lev patients’ recovery for over twenty years. She looks after your every need to aid rapid resumption of daily life on your return home. While at her all-inclusive recovery home she unfailingly provides you with personal assistance, and useful advice to bolster your recovery, will order your medications, feed you healthy, nourishing meals, and arrange transport to your almost daily post-op exams with Dr Lev. His office is just two blocks away. Casa Mercedes has private recovery rooms and communal dining either inside or outside in the attractive shaded patio.

We are even more pleased with the outcomes of our face and neck surgery, 27 weeks after, performed by Dr Lev in Costa Rica. Friends observe we look different, but cannot quite express how. For Tibor, who had more extensive surgery including blepharoplasty, forehead lift, face and neck lift, there is still a little tightness at the forehead hairline and behind the ears; Megan feels completely healed. We’re both delighted with our refreshed appearance and agree that these procedures were great decisions and a few weeks of discomfort were well worth the results and rewards.

Dr. Lev’s work is complementary to our daily efforts to remain youthful and energetic through exercise and diet. Now we look as young as we feel.

Amazing! Dr Lev was great though-out the consult to the day I left CR. I had two procedures; upper blepharoplasty (eyelid surgery) and a breast augmentation with 245cc subglandular (over the muscle) implants from under the breast. I was nervous about the implant placement at first because I didn’t read as much about them but he assured me it was the better way to go and he was right! I didn’t want huge breasts, I just wanted enough to feel more proportioned and feminine. As a 5’9 woman, 245cc was as small as Dr. Lev was willing to go, which was a good call. I’m glad I didn’t go bigger because I got the exact size I wanted, and I had no pressure from him to go larger (which is common, I know people who’s doctors made them go bigger than they wanted). The procedure was easy, and I stayed overnight which I didn’t even realize was included (a big plus). Thankfully had amazing local friends that took care of me the rest of my recovery until I was able to travel home to NYC. Way worth it. 1000000% recommend.

I started research into having a neck-lift a couple years ago. And to be honest the choice of surgeons and procedures can look daunting I came across real self and liked the fact actual patients gave reviews and to be fair if you go back far enough even the best have at least one bad review! I found dr lev on real self and I noticed straight off what a wonderful team he had I could see that your aftercare was well taken care of. Having the procedure in the states wasn’t an option the cost was just too high for me.

But going to a foreign country and not speaking the language also looked to be a challenge. Nothing was farther from the truth. I came into contact with Didi. (Eternally vain ) who has works with de lev for many years. And has had several surgeries with de lev. She holds your hand every step of the way her advice and expertise is second to none. I came to Costa Rica on the 3rd of nov 21. saw dr lev on the next day. he is as everyone says warm approachable and utterly professional I knew I was in safe hands. On the 5th I had surgery at the clinic.

Everything went fine and came back to verdeza recovery center the next morning. Verdeza is amazing I could not recommend anywhere better round the clock nursing wonderful food a lovely staff your every need is taken care of. I am now still in verdeza 10 days post op. the first few days are uncomfortable as I never sleep on my back and it’s a must for a couple of weeks. Dr lev has been more than attentive taking stitches out incrementally even the taxi rides to the clinic are arranged.

My face is twenty years young. so many people say that have had the best surgeon and there are undoubtedly many but what makes dr lev at the top is not only his manner and skill but those others on his team that have made this for me not only a life changing experience but also an adventure where I felt safe and looked after every step of the way. I came to Costa Rica looking a tired 63 yr old I travel back looking nearer in age to my oldest child. I’m going to post pictures later so I can show start to finish. So watch this space.

On day 21of my journey. You do need patience small changes very day now and bruising subsiding. I have always been a bleeder so expected the bruising ! I never thought I would have got this result with my neck. I had the genetic neck from the family which did age me especially as I have a small frame. Dr lev has done a fantastic job. And just to say he did a small chin implant to balance the face which I was hesitant about but it was he right advice and glad I did it. Will post more later.
