I think part of your problem is that you have skin that has lost its elasticity. And although a forearm lift will moderately improve that, some of it will definitely return because surgery does not change the intrinsic nature of the skin. The attempts that you have had are geared toward improving the elasticity, and as I would have told you before you had those attempts made, we have nothing that truly improves elasticity.

I have performed forearm reductions because I am expert on body contouring of the body and extremities, but if I were to find you a good candidate, you would have to have realistic expectations and the willingness to tolerate a fairly long scar.

You seem like you have some knowledge of the anatomy of the nose and lip. It would be very difficult for anyone to give you good advice without seeing a full set of pictures(multiple poses) and/or examining you. So I will answer the question in a general manner. You can lengthen the nose which also can be accompanied by decreasing the nasolabial angle, the angle between the nose and upper lip, which can give the appearance of a shorter and more relaxed upper lip. This type of rhinoplasty needs to be performed by someone who does a lot of rhinoplasty, not someone who does one once a year. So go see a plastic surgeon or facial plastic surgeon who does a lot of rhinoplasty, let him or her examine you, and help you with the decision of whether you should or should not have surgery.

The picture you supplied is not enough to give you a complete answer. Usually saddle nose deformities are much more extensive than what you see on that one less than ideal picture. If anything you look like you have something called an "inverted V deformity" which can occur in some post rhinoplasty patients who had a big hump removed. Off course I maybe wrong because again the picture is not helpful enough.
I would recommend you go see a plastic surgeon and or facial plastic surgeon who do a lot of rhinoplasty.

You request information on a fairly complex problem that is not easy to answer without examining you. If you simply have collapse when you take a very deep quick breath but don't have problems breathing in almost all other situations, and you like the appearance of your nose, you don't need surgery. Nasal valve collapse of the kind you describe most often is not going to get worse but there are certain skin conditions (skin becomes thicker over time due to skin disease) were it might.
I would recommend that you visit a plastic surgeon who does a lot of rhinoplasty and/or an ENT doctor to evaluate your nose.

The answer to that question is best answered by your operating surgeon. Most often you should be close to medical care for the first 24 to 48 hours because most really bad things that can happen occur during that period. It is not to say that other less dramatic problems can not occur after that but that is the critical period. I would make sure you have excellent help if you are doing to add arm surgery to a body lift because you generally need to use your arms to help yourself move after a body lift and if that is operated on, it will complicate things for you. However, every surgeon has their own approach to how much surgery and how many areas should be done at the same time.
I personally never do arms with body lifts, but your surgeon may have a different opinion and experience.
