I had a breast augmentation 2 1/2 years ago and feel like they never “settled” into place. I was very clear about wanting a lot of lower pole fullness and still haven’t noticed any yet. I have 320 cc textured teardrop style Sientras, dual plane. My pre op size was a 32 B.

I don't hesitate to lower the inframammary fold if appropriate, but you may get an excellent result with manipulation of the pocket and breast tissue in that area. You should talk to your plastic surgeon about your concerns and the different options to address this.

Textured implants tend to settle less than smooth implants and this could be one of the reasons your lower pool did not expand. It seems that you might benefit from radial scoring of the lower pool as well in order to allow it to expand as well as a more aggressive dual plane extending above the nipple areolar complex.#StayBeautiful #RealDrWorldWide

Answer: You do not need lowering of your fold unless you are accepting of the risks for bottoming out and having a concave upper pole (where yours are near level and flat). If you want more lower pole fullness and roundness, best way to achieve this is with fat grafting if you have excesses that can be harvested and grafted to your lower poles. Ask your surgeon about what options he/she can provide you.
