I've had my breast implants for 5 years (under the muscle), and throughout that time every 3/4 months I wake up with really bad pain in my right breast (never the left). The pain feels like a pulled muscle/bruised skin and the biggest source of the pain is below the breast and the inner breast. The pain almost entirely disappears within a day or two. It remains soft, and does not appear swollen, however I have trouble sitting up, lifting things, and bending over for the day that it is in pain.
