I'm an asian with weight of 51kg and height 156cm. I would like to have a more dramatic, curvy body but I'm afraid that I will not have enough fat to achieve that with just a BBL. From my photos will I be able to achieve my ideal look? If not, is it a good idea to combine BBL and butt implants in a single surgery? Or is it a better idea to separate the surgeries.

A BBL requires 360 lipo, which is lipo of the upper and lower abdomen, the upper and lower back and the flanks. If you do not have enough fat in these areas, liposuction can be performed in many other areas of the body for fat transfer to the buttocks, such as the upper arms and chin. Based on your desired results, you may be asked to gain some weight. I recommend you schedule a consult for a proper evaluation. I would advise against implants (due to complication profile) and consider Sculptra once the surgical results have settles from the BBL.

Answer: Combine procedures I do not recommend contemplating having gluteal implants with a BBL as combined procedures. I think you’ll find plastic surgeons to either be pro implant, or pro BBL. The majority of plastic surgeons in the United States, do not offer gluteal implants. There is a good reason for this. The procedure has a notorious high complication rate, failure rate, and lower, long-term patient satisfaction rate. There are plastic surgeons who learned to mastered the procedure, and get better outcomes than those who only do the procedure intermittently. It appears that the best results come from those who specialize exclusively in gluteal implants, use intramuscular placement and keep implant size to what fits appropriately within the gluteus muscle. Patients should recognize there are inherit limitations of body modifications, and that fat distribution and the use of implants cannot override, basic, skeletal proportions more than a certain degree. Your wish pictures do not represent normal anatomic body contours. Attempting to create body modifications that are drastically different than what you were born with often leads to long-term poor outcomes including multiple revision procedures and long-term low patient satisfaction. Keeping expectations realistic and understanding what can and cannot be accomplished with each procedure is key to consistent patient satisfaction. In the end, patient candidacy and provider selection of the two single most important variables. Understanding your own candidacy, for the procedure is an integral part of having the process be rewarding. Delivering consistent quality, liposuction and fat transfer results are more difficult than most people believe. There is substantial difference in outcomes, depending on who you choose as your provider. This is also true for gluteal implants where provider selection is critical. I think most plastic surgeons are probably going to steer your towards Liposuction and fat transfer is the more appropriate choice to start with. Gluteal implant, augmentation only gives volume to the gluteal muscle, and does not give hip augmentation.I suggest patients have multiple in person, consultations with local plastic surgeons who have expertise in the procedure you’re interested in.
