Q: Why don't all doctors use Exparel for Mommy makeovers?

A: Exparel is a long acting (72 hour) local anesthetic. It’s commonly used in tummy tucks as well as other plastic surgery procedures. I find that the results are inconsistent- which is why I don’t use it routinely with all tummy tucks. Meaning that some patients get very good pain relief with it and others get very little improvement. This is likely related to how each person’s body metabolizes the product. If you are very concerned about pain relief, it is something your surgeon should be able to add quite easily. The added cost is about $300-350. I hope this helps. Best wishes, Dr.Bruno

Q: Can I ask my surgeon to reinforce the pocket to prevent double bubble and the implants moving to the armpits?

A: If you are concerned about double bubble or implant malposition, you can inquire about using an internal support device. I prefer Galaflex which is dissolvable and adds strength to the repair- it will be reabsorbed in 18 months and replaced with collagen. Best wishes, Dr.Bruno

Q: I want a BBL & lipo on my whole back and arms. How long do I need to wait to have a tummy tuck and breast lift? (Photo)

A: You should wait about six months to allow the blood supply to normalize and allow yourself to recover. Some would attempt to perform all these procedures at once, however the risk of complications (namely a blood clot) is too high and the recovery is too difficult for most patients. Q: Why do some women bruise less than others after a BBL? Is it technique? Smaller women vs fuller women?

A: There is no way to completely avoid bruising after liposuction, but there are ways to minimize it. Taking Arnica and bromelain before and after surgery can reduce the visible bruising. Also, the technique used does matter. After injection of the wetting solution or tumescent solution, the surgeon should wait at least 10-15 minutes before starting liposuction, as this allows the blood vessels to shrink and therefore less bruising.
