Q: What is this bubble/blister I always get after lip filler? (Photo)

A: This looks like an area where filler was injected superficially and is visible under the skin. Dissolving the filler would remedy the problem. It may seem bigger after injections because of the associated swelling accompanying injections and lip filler's hydrophilic properties.

Q: I had Restylane Lyft placed in high lateral cheek bones. Loved the results but it wore off too fast. Any suggestions? Age 36

A: Typically, Lyft lasts longer than that but potentially something like Sculptra or Radiesse could last longer as they are clinically indicated to last 2 years and 18 months.

Q: How long does it take for Restylane Lyft to set?

A: The effect is immediate however, there is swelling when you initially inject it. I find that the filler usually looks its best about a week after injection when it's integrated into the tissue and the swelling is gone.

Q: What can I do about permanent asymmetry in chin caused by Dysport?

A: The injector likely hit the DLI (depressor labia inferioris) instead of the DAO or mentalis. This can create an asymmetrical smile. Normally I recommend just allowing the dysport to wear off if this occurs. Since the other side was also treated and potentially overtreated, I would avoid treating in this area until the muscles completely recover. Some providers, including myself have seen some success with using Upneeq to temporarily increase the muscle activity in the DLI by placing the Upneeq in the mucosa of the lower gums. But, I do think, with time the muscles will go back to normal.

A: It sounds like you may have some post-inflammatory hyperpigmentation (PIH), hemosiderin staining or potentially extremely thinned skin from over hylenexing. It could even be that some of the filler has migrated so close to the surface that its creating a blue tint called the Tindel Effect. Treatment would depend on the actual cause. IPL that addresses red pigment could help if its staining or (PIH). If the skin is very thin, then treatments to increase collagen and perhaps even a thin filler meant for perioral lines could help. If its Tindel-more hylenex should be used.
