Dr. Robert Cohen is a board-certified plastic surgeon with practices in both Beverly Hills, California and Scottsdale, Arizona. Dr. Sigalove is the best! You can tell he truly loves what he does and it shows in his work! I’ve struggled with self confidence issues due to my breasts for years. Q: How do dual plane and sub muscular implants affect chest muscle strength long term?

A: Dual plane or total submuscular coverage does not affect the strength of your pectoralis muscle. There are studies, however, that do show long term shoulder girdle muscle weakness which would potentially affect someone who was engaged in vigorous strength training or fitness. Breast augmentations that are performed in the dual plane or submuscular position can indeed result in hyperanimation deformities which can be noticeable and potential unsightly. To minimize the likelihood of this, especially in an athletic patient who enjoys doing chest workouts, I would recommend a subfascial breast augmentation. #sigalift

Q: Are my breasts tuberous and what would be the optimal placement of an implant?

A: Implant selection is truly something that is based not only on your objective measurements obtained from bio dimensional analysis, but also your subjective goals regarding the aesthetics of your breasts. It is impossible to tell you what implant you should get strictly based on your measurements alone. The projection of the implant dictates a lot about your ultimate aesthetic outcome, for instance are you looking for an extremely natural look or do you want to look like a stripper. You have a mildly tuberous breast defect, and, in my practice, I treat these exclusively with subfascial augmentation plus lower pole support with Galaflex. Additionally, a circumareolar mastopexy is almost always used in order to create a proportionate looking and appropriately position nipple areolar complex. In some instances, sometimes even an inverted T mastopexy is utilized.

A: Based on your photos, you do have inframammary fold malposition, AKA “bottoming out”. You can absolutely go larger than 375cc and I feel that 500-550 HP implants would be extremely reasonable and would give you the result you are looking for. Mandatory in your repair, I believe, should be placement of a soft tissue support scaffold such as Galaflex to maintain your inframammary fold over time. You could either keep your implant sub pectoral or switch them to subfascial and that is a conversation you and I would have at the consultation. The majority of my practice is breast revisionary surgery, please feel free to call my office if you have any questions.

A: I think that you would be well served with a revision. Although not medically necessary, I believe that there is an excellent opportunity for you to feel extremely satisfied. You have a classic waterfall deformity which happens over time where the breast tissue slides down inferiorly over the pectoralis muscle but the implant remains in reasonable position within the pocket. It gives the classic elongated and oblong looking breast, the treatment of which, is a full mastopexy. By tightening the skin and breast tissue overlying the implant you should have a beautiful and appropriately positioned breast again. At the same time, I would perform an implant exchange and any necessary pocket adjustment based on your aesthetic goals and ultimate outcome.

A: First of all, those breasts are never going to round out on their own. Your inframammary folds appear stable based on this photograph. However, it appears that you have an elongated and stretched out lower pole giving you an extremely long nipple to fold distance. This is a common finding when only a circumvertical mastopexy is performed without using an inverted T incision. Without examining you, I cannot tell if you have capsular contracture which could also be causing the distortion in your lower poles. You definitely need a revision with a full inverted T mastopexy where all of that stretched out and elongated lower pole skin can be resected giving you a tight and crisp inframammary fold and lower pole.
