Although you have some volume loss in the upper cheek most of your aging issues are visible in the lower face and neck. A SMAS facelift is the appropriate operation to correct this. This is a general term however which means different things in different offices. The SMAS is the investing layer of the muscles of the face and must be "lifted" for a long term result with the best opportunity for minimizing scarring. You will do very well with this operation. Volume addition to the face will also be important and this can be done surgically with fat grafting during your procedure. Your upper lids and brow position are good but the fat pads of the lower lids can be addressed during your facelift.
Fine lines and wrinkles are best addressed by a laser resurfacing procedure which I do at a separate time.
Your breast implants can be removed at the time of your facelift procedure. Staging the breast lift is an excellent idea.
Seek out a board certified plastic surgeon with advanced training in facial aesthetic surgery.

The answer to this question is based completely on how much the appearance of your breasts concerns you. This would be an elective cosmetic procedure as there is no medical necessity to do anything.
The implants are sitting in the same place they were 10 years ago but the breast has slipped off the implant. This is most apparent in the profile view.
Aesthetically we like to have the nipple at the most projecting point of the breast. Your nipple is well below that point and breast tissue has descended past the breast fold and is resting on the chest wall.
The corrective procedure for this would be implant replacement with a breast lift and lower pole breast reduction.
If you are going to have children within the next year I would wait to have surgery until after having children. If there is no immediate plan for children and you want a more youthful "perky" breast you could have surgery at any time.

It sounds as though you are interested in a breast reduction which will in fact also provide a lift. This is an excellent option as you have ample breast tissue. The procedure is limited by the amount of upper pole fullness you can create. The implant will not lift the breast. It simply provides more upper pole fullness than techniques which use your own tissue.
An advantage to a lift alone in a combination procedure is that the chest muscle will not be manipulated. This will make your recovery easier as you will be able to compensate for your core limitations with your upper body.
After about 3 months if you feel you need more upper pole fullness than you get from the procedure you can always return for a breast augmentation. A staged procedure is an easier recovery and statistically the need for revision surgery is significantly less.

A: Thank you for your question. This is actually a challenging one to answer because aesthetic surgeons use very different techniques under the vague name of "mini" facelift.
I am glad to see you included the SMAS in your question which means you have been doing research but the SMAS can also be manipulated in many ways to yield different results.
The term "mini" to me is a marketing word. It makes the procedure sound less invasive or less scary. A facelift is necessary when structural changes have caused a break in the jawline, deepening of the marionette lines and laxity of the skin of the neck. How much you lift and how much skin you remove varies but manipulation of the SMAS is critical yield the best and longest lasting results.
Volume may also need to be added in the form or fat in surgery or fillers in the office to restore the youthful volume of the face. Skin resurfacing may also help in completing the rejuvenation.
My point is that if you manipulate those structures properly the duration of the effect should be the same whether it is a minimal correction or a more significant one. 5-10 years is reasonable to expect.
Seek out a board certified plastic surgeon to evaluate you and discuss their technique and expectations for duration of their results.

A: Absolutely. Fat transfer to the face can be done under local anesthesia.
The areas fat is going to be taken from will also need to be made numb in addition to the face. The face is easy to make numb and keep you comfortable. Harvesting the fat is done with liposuction and dilute local is used to numb the regions fat is taken from.
As far as reliability, I believe roughly 50% of the fat placed remains. This is an important discussion as the procedure may need to be repeated to achieve your desired results.
