A: Depending upon where exactly the fat is located it can typically be removed with liposuction or direct excision(removal surgically). It is not caused by wearing garments that are too tight but can be made more apparent with certain tighter fitting garments. Best to see an experienced plastic surgeon for a consultation and recommendation.
A: I am sorry to hear that you are now having problems. Several things can happen to cause this but first and foremost, you need to see your plastic surgeon. There is no one else who is best at determining what is going on. A physical examination by the PS is most important and will likely result in getting a diagnosis or assistnace with a test such as an ultrasound to rule out a hematoma(blood collection)or seroma(fluid collection). I have seen this before and it can be quite painful and worsen with time. It can be treated relatively easily as well. I hope that this gives some clarification and guidance.
A: The way you phrase your description of "flat" nipples instead of inverted nipples leads me to think that you might not have enough nipple projection from development. Some patients respond very well to elongation procedures, but when there is a congenital deficiency, then it is less likely that a long term improvement can be obtained. Fat injections will likely not help for the long term. A good test to see if you could achieve permanent projection is to pull on the nipple itself and see if it will elongate to your satisfaction. If so, then an incision at the base and suture techniques may help. Good luck.
A: Thank you for your question regarding incision placement. I do think that there is a difference in the outcomes and although hard to say that capsular contracture is more likely with a periareolar incision, it can lead to more scarring, and an unacceptable cosmetic result. It is almost always noticeable in the nude and when it gets infected or wide due to tension and damage secondary to placing a larger implant in through a small incision it can scar poorly. An inframammary incision is so inconspicuous, leads to easy visualization of the anatomy and in my opinion after doing several thousand implants, is by far the best place for an incision.
A: From the appearance of the photo you have a slight wound separation at the inframammary incision site. It looks good from here but should be seen by your PS or the nurse. Most of these heal rather nicely and shouldn't pose any compromise.
