Dr. Richard Zienowicz earned his medical degree from Brown Medical School, followed by plastic surgery and hand micro surgery residencies at the Cleveland Clinic and Harvard University / Massachusetts General Hospital. Dr. Zienowicz is now the Chief of Cosmetic Surgery at Brown University's Department of Plastic Surgery. A: Hello and thank you for your question. Unfortunately answering your question is complicated, as patient anatomy, patient goals, medical history, nicotine use, BMI, breast measurements, skin quality, and abdominal muscle separation all factor into which surgery/surgeries are best for them. The most effective way of getting upper pole volume is with a breast implant, then fat grafting if a patient does not want implants. Correcting asymmetry is typically best done with a breast lift. A breast lift can be done with or without an implant and with or without fat grafting to the breast - be sure to discuss all of your options with your board certified plastic surgeon. A: Hello and thank you for your question. I use Vectra3D imaging at my practice to measure not only BWD but many measurements of the chest. I typically do recommend an implant that is wider than a patient's natural BWD as this typically provides breast volume in the center and sides of chest that patients are looking for with augmentation. A: Hello and thank you for your question. Unfortunately I do not think that there is much to do other than have time pass to reduce the volume from Sculptra injections. Hopefully they were still recent and some swelling will reside. You can certainly consult with a plastic surgeon to have them examine you in person and discuss options.
A: Hello and thank you for your question. It is difficult to tell based on your photos but it is unlikely that your back rolls will go away without skin removal surgery. If you are at a lower BMI you could likely have some reduction of the volume with liposuction and perhaps a skin-tightening treatment such as Renuvion, however, a complete removal of the rolls can only be achieved with skin removal. I would discuss the risks and expected results of liposuction alone with your surgeon as you will have to consider these before going forward with surgery to ensure you are happy with your results. Very artfully and carefully placed fat grafting can offer a naturally beautiful rejuvenation of the face - whether the volume loss has been due to aging or weight loss or both. The cheek and temples are common sites for facial fat transfer. I typically recommend fat grafting to be done with many of the facelifts that I perform for best results. Your surgeon should be able to give an idea of how much volume (typically small volumes) and where it will be placed as well as what your results can look like. Best of luck to you!
