Do You Recommend a Lift Or Can I Do With Only Implants? (photo)

Full C want DD - would I need a lift too or would the implant correct the sagging? I also want them closer together, as of now my nipples kind of look AROUND you, hah.

A: Lifting breasts with implants

In truth, the answers to your questions are in this blog. Yes, you would need both a lift and an augmentation. I would likely use a "lollipop" incision around your areola and down. The size of implant used determines the amount of loose skin that can safely be removed. In time, fat grafting may be used, but the jury is definitely out. The biggest error that can happen, in my opinion,  is excessive skin removal leading to breakdown and implant infection. Many feel that doing this operation in stages is safer.

What Size Is Best? I Have a BWD of 11.5, Want to go From 34B to a Natural 34DD.

I Am 5'7" and 145pounds. At my consultation my ps suggested a 425cc high profile with a base diameter of 12.5 and projection of 5cm or a 500cc ultra high profile at 11.9 bd and 5.7 projection. Which selection, if either, would give me a natural D/DD? Thank you...my pre-op is in 1 week!

A: Natural DD

The answer is "it depends"!

On many factors like your skin tone, breast volume, chest width etc...

Are you a smoker?

Have you Breast Fed?

How droopy are you?

Where will your implants be placed?

Your height and weight don't really help.

I find that women best communicate their desire by showing me what they want by trying on implants with a sheer bra and an elastic  tight white top on. White shows size, while colors conceal it.  The muscle pushes down (erases) about 50 cc of volume. Therefore, if you like the look of 450... get the 500's. The most likely error is wishing you'd have gone larger. That is an expensive error>

Am I a Candidate For Fat Transfer in One Breast Only? I Have Uneven Breasts and Already Had Encapsulated Implants Removed.

I am 42 5'2" & about 130 pounds. I had breast implants 20 yrs ago. They encapsulated and caused me lots of complications. I got them replaced & removed 4 years later. I now have uneven breast - 1 A cup & 1 AA cup & they are different shapes now. My question is -since Im not a candidate for implants, would you recommend fat grafting for me if I only want to inject fat into the smaller breast (AA) so that it can be the same size & shape as the (A) cup? How much fat would I need? thanks!

A: Fat transfer may be your answer

Fat transfer is tricky business. The process requires some estimation and you should plan on  2 procedures. Some fat lives and incorporates into the breast, some doesn't. Dr Del Vecchio has a large experience with this procedure, but nobody is perfect. Most people have fat to transfer, but this is a new technique in PS and errors will occur. If you adopt the attitude that you are augmenting your smaller breast naturally, you will accept the tradeoff! At A or AA, you may want to seek 2 or 3 opinions prior to any surgery. I would likely recommend implants again.
