Granuloma Removal For Under Eye, From Collagen Filers? Other Options?

Do any cosemtic surgeons remove granulomas under the eye from collagen fillers? Or do they suggest a lower bleth to remove the granulomas? Is there any success of preventing indents or large scars with two lumps from granulomas?

A: Granuloma?

It is not typical to have granulomas after collagen injection.  Hyaluronic acid (HA) fillers are also low risk for granuloma formation.  Sculptra has an association with granuloma formation. Massage is the first treatment option for a presumed granuloma.  If that doesn't work, steroid injection or use of Hyaluronidase (with HA fillers) can help resolve the process.  Finally, if that doesn't work, excision either directly or via a blepharoplasty can be a final option.  However, time will tell.

Be patient.

Excessive Tearing 3 Months Post-op Eyelid Surgery, How Can I Make it Stop?

3 months after surgery,I started experiencing excessive tearning in both eyes.My surgeon was an Opthamologist.I have been seeing another opthamoligist about this problem. He has flushed my tear ducts 2 times with saline, prescribed dexamethasone-pf drops, plus a slew of other drops. After flushing, my eyes are 90% for 1 or 2 days, then they start the excessive tearing/redness again. Could this surgery have caused this? It did not appear for 3 months post op. What can I do to make this stop?

A: A second opinion is certainly your right!

Hello,

I think that Dr. Steinsapir is correct.  A second opinion might be in order.  Excess tearing, or epiphora, is typical in the early stages after blepharoplasty.  If you have late onset epiphora, you may have trouble with how your lid is apposing to your globe vs. lacrimal drainage problems.  A fellowship-trained Oculoplastic Surgeon is a great resource to evaluate your problem.

Good luck,

Dr. Shah

Recommendation for Blepharoplasty? (Photos)

I'm a 22-year-old male, and for some time I've felt embarrssed about my eyelids - they're both droopy, and the and they're also asymmetrical. (I usually feel bad about how I appear in pictures.) So that's... dermatochalasis with mild ptosis? I'm thinking about having an upper blepharoplasty done; there appears to be a pretty good oculoplastic surgeon in my area (central Massachusetts) who I could visit. What are your opinions/recommendations?

A: Go see an Oculoplastic surgeon!!

Hello,

I think you are smart to see an Oculoplastic surgeon about your concern.  You don't seem to have extra skin on your upper lids in your pictures.  I doubt that an upper lid blepharoplasty is necessary.  A ptosis repair, however, is a different matter.  The Oculoplastic surgeon that you are planning to see will help you determine what surgery, if any, is best for you.

Good luck,

Dr. Shah
