Fraxel Lasers and TCA Peels (Facial) for Middle Easterners/Indians?

I'm of Middle Eastern/Indian descent and have light brown skin, and would like to get fraxel treatment or TCA peels to deal with wrinkles under my eyes. But I've heard that these procedures are riskier for people who aren't caucasian, and may discolor (perhaps permanently?) the skin of people with brown skin tone. What is the risk level for these procedures for people who are not white?

A: Fraxel is a safer choice for wrinkle removal in dark skin Fraxel restore and Fraxel repair laser treatments can be excellent procedures to smooth wrinkles, texture and scars in dark skinned individuals. Although there is some risk for hyperpigmentation, it is a much lower risk than with other procedures such as TCA peels. We have found it to be a very safe treatment in our office with a low incidence of increased pigmentation. When this has occurred it was usually in Asian skin, not Middle-Eastern and it was usually temporary. I have not seen loss of color (depigmentation) from Fraxel. I would avoid any peels other than light glycolic peels if you have dark skin.

Fraxel Restore Vs Botox for Large Nose Pores

I am a 34 year old Asian female. The pores around my nose are very large, much line the skin of an orange..yuck! I clean my pores, but they just appear more visible when "unclogged." Additionally, I'm EXTRA oil after I've unclogged my pores. I've been using tretinoids for years, but doesn't reduce the size of my pores. Please help! What is the most effective and longest lasting procedure for pore reduction, Botox, Fraxel Restore, Juvederm, other lasers, obagi nu-derm, anything?? THANKS!!!

A: Fraxel restore works best to reduce pore size In my experience, utilizing non-ablative Fraxel restore, ablative Fraxel repair, IPL, SmoothBeam, GentleYag, among other laser and light-based devices in my office, non-ablative Fraxel restore and ablative Fraxel repair work the best. Botox and fillers will not affect your pores at all. Skin care products containing retinoids and glycolic acids may temporarily reduce the appearance of pores.

Is It Ok to Place Saran Wrap on Top of Topical Anesthetic Pre-Fraxel Re:store?

I had the 2nd of 6 Fraxel re:store treatments (4 weeks apart) last week & it was more painful the 2nd time (although the settings 2nd time were slightly lower). For 1st treatment the PA placed Saran wrap over the lidocaine on my face for ~60 minutes & treatment was bearable (last 2 passes were the worst). Before 2nd treatment, same anesthetic (thick layer) for about ~75 minutes but NO Saran wrap & treatment hurt worse. PA said Saran wrap was not usually used-is Saran wrap OK? Thx!

A: Saran wrap over topical anesthetics can be deadly

Do not let anyone wrap your face or body with saran wrap over a topical lidocaine or prilocaine anesthetic.  This "occludes" the area which means it concentrates the anesthetic more and increases its penetration into the skin.  There have been reported cases of deaths due to this however because sometimes the concentration of lidocaine gets so high it affects your heart and/or nervous system.  Small areas are probably safe but an entire face or part of your body (when used for laser hair removal for example) can be dangerous.
