I look so old and tired. My eye lids are droopy and I noticed that my forehead skin is loose now as well.I have bags under my eyes and on my cheeks (puffy), marionette lines and nasolabial folds.I stay out of the sun, don't smoke, use moisturizers, wear sunscreen, etc and it just keeps getting worse. I have tried an injectable filler for my nasolabial folds but it didn't help. My right side is worse than my left side, but it is quickly catching up.

A: Avoid Surgery

Hi Selena,

I see from your photos that you are unhappy with the distribution of volume (fat) in your face and how it is changing.  As others have said, this can be a sign of medical issues (like thyroid problems or metabolic disorders) or even stress.  The good news is that at your age, things can change for the better too.  You certainly do have elasticity in your tissues, which is what people lose with age.  Your main concern is the location and amount of fat you have.  If your BMI is on the low side, sometimes a little healthy weight gain will help.  If you have a more normal BMI, then follow up with your doctor.

On the cosmetic note, Injectable fillers are probably going to fall short unless the volume you need is in the 1-2cc range.  On the other hand, you may benefit from a little more volume than that - which is probably more like 6-10cc per side.  This can be done with facial fat transfer which is minimally invasive and can be used around the eyes, in the nasolabial folds and the upper lateral cheeks in higher volumes.

Before considering any of that, though, you should first get a complete medical work-up with your primary physician to ensure that you do not have some other process causing your issues.

Rhytidectomy

i am 19 years old and thin.my cheek skin is very loose and thus gives my face round look instead of chiseled look.i want to know that facelift will be good for me or there are other procedures available.

A: Rhytidectomy No, but there may be other options.

Indeed, I echo my colleagues in saying that it is not appropriate to undergo rhytidectomy at age 19.  It can be difficult to articulate exactly what you desire in your appearance and specifically, if that can be achieved with your anatomy.

The best idea is to consult with someone in person and go over your concerns and options.  It should be an educational experience for you and you may find there are simple things you can do (that don't involve surgery) to get the look you want.

What is the Best Procedure for a Drooping Lower Face with a Strong, Full Jaw and Chin?

I would like to correct my flat drooping cheeks. I have extra skin and some fat in my cheeks and chin that becomes much more pronounced when I smile and my nose tip droops. My under eye whites are visible and my mouth is slightly down-turned. I like the way I look when I pinch and lift my cheeks.

A: Drooping lower face and strong jaw

Based on your photos and the result you are trying to acheive I would recommend two options.  The first would be a fat transfer which would volumize your cheeks giving a more pronounced enhancement of your cheek subunit.  It also would act to artificially lift at the nasolabial fold region and midface giving you a taller more elegant cheek bone appearance.  If the droop in your nose on smile is bothersome a simple procedure to separate the muscle known as the depressor septi would improve this problem and decrease the nasal droop.  This can be done through a tiny incision on the inside of the nose.  Standard lifting procedures would be a complete waste of your time and energy at this point in your life as you are quite young and attractive and those porcedures would not give you the desired result.  Hope that helps!
