I started with a medium skin color of NC35 or so which was mostly permatan, and looked dull and my elbows and the back of my neck were darker than my skin in general. I exfoliated with papaya soap as well as arbutin soap and a scrubber, and applied Palmer's body lotion and a lactic acid lotion for about 6 months while using a strong sunscreen with zinc oxide and titanium dioxide.
Then afterward I began to use tretinoin cream, hydroquinone gels from the beauty supply store such as IKB and Precious, as well as skin whitening sheet masks. I used Triluma on trouble spots as well. I used an Indian version of Triluma called No Scars from Ebay. I did this for 6 more months, using a high PPD rating sunscreen in the morning. Finally I have gotten my skin even and to a color of NC20 which looks great, I have a lot more confidence. Cheers, it takes a long time but it gets you back to your birth color regardless of your ethnicity, and is well worth it.

I'd love to see pictures of your results. It seems like you put a lot of work into your treatments. Are you continuing your regimen or have you stopped?
Right now, haven't got any pictures on hand (will upload) but it's not as complex as it sounds, don't worry. I just use the lightening / tretinoin lotions instead of regular lotions, the lightening soap instead of a regular soap. I stick to either a physical barrier sunblock, or a high PPD European or Asian sunblock such as Biore or BIoderma.
