Implant Removal Under Local Anesthesia - Personal Experience

The posts on RealSelf were so helpful and informative for me and helped me feel good about my decision to have my implants removed so I wanted to share my implant history and personal experience in hopes of helping others make the decision to explant! I am 29 years old, 135 pounds, and explanted after 9 years last week (scroll down to bottom for explant information :))I had breast augmentation at 20 with saline implants under the muscle. At the time, I chose 300 CC implants and my PS ended up overfilling them to 350 CC on one side and 375 CC on the other (a common practice I am told). I went from a A/B cup to a D cup and was never truly happy with the decision as I was very athletic and always felt 'weighed down' by the implants, doubling up on sports bras to do almost anything. After about one year, I made the decision to have my implants removed. I chose a different PS and after the consult, he convinced me at 21 years old that I would be very unhappy with my breasts if I explanted because they would be saggy and deflated. He advised me to exchange the saline implants for a small silicone gel implant and I agreed. I exchanged my saline implants for 225 CC silicone gel (gummy bear) implants. The silicone gel implants looked more natural but I was still unhappy with the way they felt. I could never sleep on my stomach, running was uncomfortable even with two sports bras, and I could always feel them moving around in the capsule. I had these implants for 7 years when the right implant started causing me pain. I would have an aching pain in my breast. I also started having joint pain in both of my knees, elbows, right wrist, and right ankle. I don't know if that was coincidental but I saw my doctor and was tested for several conditions that could cause the pain including Celiac, Rheumatoid Arthritis, and a Rickettsial titer (for tick born diseases). All blood tests were negative so I started on a high dose of a joint supplement. The pain waxes and wanes still but improved with the joint supp.I read all the forums on Real Self about removing implants under local anesthesia and found a PS in my area that would explant under local (through RealSelf). I called my previous PS that placed my gel implants to get the records on them and they told me (without telling me) that explanting under local was crazy and I should not do it. My implant removal surgery was 2/13. I took an Ativan and a Valium 30 minutes prior to my procedure and the entire thing took about 45 minutes. I was so nervous that I would faint as I have a tendency to, but once the meds kicked in, I was totally relaxed. My PS told me to wear headphones if I wanted to, so I listened to music for the entire procedure. I think it helped me a lot! I was awake the entire time, but kept my eyes closed to stay relaxed. My PS used an inframammory incision and numbed the area (which was the worst part). The first prick felt like a bee sting but then the lidocaine kicked in and I didn't feel any other injections. There was one area that was not totally numb and as he was using the cautery, I felt a pinch. I told him and he added more lidocaine. I didn't feel a thing after that. The whole procedure was over before my playlist :) I sat up and put on my own clothes, walked out to the lobby and got into the car by myself (my husband was with me). I slept most of the day of the explant. The second day I was still feeling the effects of the sedative but by the third day I felt so much better. I only took one pain pill on the day of the procedure and then switched to Tylenol after that. I have been icing both breasts 4 to 6 times per day.The location of the incision has been the worst part as my bras are constantly putting pressure on the incisions. I am almost a week post-op and feel so good other than the soreness of the incision! My boobs are so small but if they don't 'fluff' back up, I don't even care. I feel more feminine now than I ever did with my implants. I am an A cup now and am guessing I will probably end up a full A/small B cup. My breasts are missing the fullness on the top that the implant provided but are not 'empty'. Each day they look better and better. I go back to my PS tomorrow for my post-op appointment and I will see if I can get my before/after photos and post for reference. If you are considering explant surgery, please don't make your decision based on other people's opinions. It is such a personal choice and only you should decide what is right for you. I regret waiting as long as I did to have mine removed, but am so glad I finally made the choice to have them totally removed!

Amazing story and amazing ps! Who was your surgeon? I would love to have mine explanted locally with headphones!
Thank you AllNaturalMe! My PS was Dr. Jeffrey Zwiren at Prima Center in Duluth, GA. He and all of his staff were very relaxed yet knowledgable and made me feel very comfortable.
And wearing my headphones (the over the ear kind) and listening to music helped me stay focused on something else. It was a great idea!
Thank you! Did you have capsules removed as well? I'm trying to decide if it's necessary or if the body will just absorb them over time.
I did not have the capsule removed. My PS said it was not necessary as the body does resorb it and removing the capsule would have required general anesthesia and a longer recovery. I have kept a sports bra/compression wrap on 24/7 to reduce the risk of fluid build up or seroma from occurring in the capsule.
Thank you for sharing your story. I LOVE the part where you say you feel more feminine without your implants. I look at my implants and I don't see them as sexy anymore. Instead I look at my girlfriends with the small, natural, "imperfect" breasts and I envy them. I wish they could see how perfectly, beautiful they look to me. Congrats on your removal! I'm jealous ;)
Thank you Blonde mom! You would think the opposite that implants would make you feel more feminine (and for those who love their implants that is probably true!), but I do feel girly and feminine and have even more confirmation that it was the right decision for me. Are you considering explanting?
Hi, i also want to rwmovw my implants but im so scare, im happy u did it and hope u have a good recovery i just think of the day that i don't have this things in my body, when u first got implants they were place by the imfamamory incision?? Submuscular o subglandular??? Did u loss sensibility in ur chest??i don' t speak really good english hope u andersatan.
My implants were placed under the muscle. Initial implants were placed by axillary (armpit) incision, the exchange was areolar (nipple) incision, and my explant was done by inframammary (under the breast) incision. So I have all three scars :) I did lose some sensitivity in my nipples after the exchange when the incision was under the nipple but never experienced any change in sensitivity due to the location of the original implant. Are you having issues with your implants?
I also got my implants at 20, have had them 9 years and went from an A to D-DD! I am 5'2" and 130 lbs. Its so comforting to read your story! Mine are saline over the muscle though, and I've had/nursed 3 babies in these 9 years. I've been to multiple consults and
Sorry hit post on accident! Anyway, had a couple drs say I would need a lift and one dr said I should be fine since I'm still young and did well bouncing back after pregnancy, I wouldn't need a lift. So I know for sure I want to explant but now I don't know if I should go ahead with the lift or not. I don't want to regret not doing it, but also don't want to pay or go under general if I don't have to. Did your dr not recommend a lift? I'm not just worried about sagging, my areolas are a lot bigger now so I'm not sure if they'll shrink back to normal after the implants are out...
Hi Mandeline! I told my PS that my husband and I are planning to start a family this year so it did not make sense for me to have a lift done before having children and breastfeeding. I had some 'sag' to my breasts already that was noticeable with the implants but it has not gotten any worse without the implant. I do have less fullness on the top of my breast and more on the bottom without the implant if that makes sense. That is the major difference that I have experienced. No cleavage without a push up bra. My areolas are actually smaller now without the implants and my nipples are a little larger...I suppose it's because they don't have the pressure of the implant on them anymore. Personally, if I were considering explant vs explant with a lift, I still think I would have explanted under local and then given my body time to totally heal before considering a lift. I am very, very happy with how my breasts look post-explant already and it has only been 8 days.
That's so inspiring! And makes total sense about not doing a lift yet. I have 4 kids and hubby got a vasectomy so we are done. That's why I want to go ahead and do explant/lift and get it all over with. I am contemplating explant first in office though and lift soon after. It does seem like results are better with some healing time in between. Thanks so much for sharing you story. It's so nice to hear of women being happy after implants :)
You had a great PS I can see! I lived in Savannah, Ga. years ago for a year. Some of the best girlfriends I have ever had were there in Ga. I'm so happy for you to have your explant done and without general! Happy healing girl! I'm next!
Thank you! I love Savannah!! Best of luck to you with your explant...you will do great!
