Dr. Rival has a lot of positive reviews for primary rhinoplasty, but very few reviews at all from patients who have had revisions. His assistant tells me he does revisions frequently, however. Any of you have experiences with Dr. Rival for revision rhinoplasty? His prices are better than some of the other surgeons I have interviewed in Toronto, and I also just liked him better as person but I am very nervous to make the wrong decision.

Who Drove You Home After Surgery? Toronto
Hey,
I am wondering if any of you Torontonians have a recommendation for a service to do the pick-up from clinic and drive home post-op. I am trying to avoid telling my family or any friends about this procedure. Someone really should start a company that specializes in picking people up from plastic surgery!

Second Revision done with Dr. Rival
Hello,
I just got a revision rhinoplasty done with Dr. Rival (previous 2 ORs were not with him).
As background: for my first rhinoplasty, I had a bump shaved down and tip narrowed. It was a typical rhino with a cast/major facial bruising (fading, but lasted 1.5 weeks until fully gone) and eyes practically swollen shut and burst capillaries (lasted almost 3.5 weeks). No breathing through the nose until the splints came out 1 week after. It was not fun.
I wanted a revision mostly because of tip bossae showing through the tip (note: after the first surgery it probably took 1.5 years for the bossae to look really prominent and 2 years before I finally got fed up with it).
My first revision was a very minor OR, done closed approach, where my original surgeon just tried to suture the bossae so it didn't poke out so much.
So the first revision didn't really take, the bossae looked softer for a bit, but then became very prominent again (possibly the suture became loosened/undone). As least that surgery was free.
So onto my second revision. Another 2.5 years later (so 4.5 years since the first surgery). Tip bossae seem super prominent and close to the surface particularly on the left (skin looks white over the bossae, seems to be under just 1mm of skin). It is noticeable to others although I haven't brought it up myself. Tip is just generally pointed and narrow and veering to the right. I had consultations with 3 surgeons in Toronto. I went with Dr. Rival as he was very personable with many years of experience, and seemed to have good reasoning for his plan and a conservative approach. (One of the other surgeons seemed to want to fix too many things that I hadn't complained about--basically seemed to be building me a whole new nose, and the other one seemed very good, but quoted such a high risk of skin necrosis (10%) that I would have been out of my mind to agree to that). The other MDs had recommended conchal grafts by Dr. Rival argued that with my think skin, hard grafts would likely eventually show through. I had also really wanted to see an MD in Michigan with a good rep, but he only does closed-approach, and everyone seemed to think I would definitely need open.
So the surgery that Dr. Rival proposed and executed yesterday: Open approach, reduce the projection of the nose and reduce the show-through of the tip bossae by curling in the cartilage/bossae, as well as temporalis fascia graft over everything to thicken my skin/add some cushioning. Apparently the cartilage on my left side was longer than the right, so by evening those out, the tip should be more symmetrical as well. He also took a small amount of septum to graft as support to the tip. I have small cast and some tape on my nose. No packing or splints inside. Tiny sutures on the incision which seems almost invisible already. I have been able to breathe through my nose while sleeping since the get-go, which is nice since the mouth-breathing post-op is one of the worst parts. I am using a moustache dressing for small amounts of seepage, and started with saline spray and polysporin as directed today. On the same say as surgery, I did have some slightly blue areas on my tip (just two spots) which I had been warned by Dr. Rival this would happen with thin skin tip revision and went away on their own. The colour is now normal/pinkish.
My pain, which is only in the tip area was maybe a 5/10 same day as op. and now a 2/10. My pharmacy wanted to verify my px with the MD last night (but it was after hours) so I haven't had any prescription pain meds (and now I won't be leaving my house for week to pick those up). Extra strength Tylenol is working well.
Oh, I also have an incision on the right side of my head above the ear where the temporalis fascia graft was taken. It is just sensitive when I'm chewing, and I can feel some sutures there, but the pain is minimal.
I will post before/after picks next week when the cast come off.

Hello Johnny, im glad everything went well at surgery. I wish an easy and fast recovery. I'm looking forward to your updates. My surgery is in 4.5 weeks and I cannot wait!!!

Who did your primary? I used Dr Peters and I regret it to this day. He left it an over projected beak and he narrowed the part between the eyes to be pinched while the base was still wide. He cut apart my alars and left me with a large U shaped nose. I wish I had of done more research.
