Extremely Happy! Went from small A cup to a full C cup (Sientra silicone implants)

Almost two years after my augmentation I am considering explant and fat transfer - just can’t win :( Comment with advice pls!!

Okay - First of all, it’s been a long time since I last ‘contributed’ on here, but I figured the time has come to give a bit of an update and also (hopefully) open a discussion about further options. First, here is a bit of background info for you all :)
PS: I also added some photos for reference. My before photos aren’t the best because they’re not in color and unfortunately I don’t have others (should have taken more, grrr), but they do the job ;)
For as long as I can remember I’ve been unhappy with my small breasts; pre-op 34A (or even AA) and I long thought about augmentation for a more womanly figure. I am not extremely skinny but I’d consider myself slender with a more athletic build and my breasts were always of concern.
**September 2016: In September of 2016 I finally took the step to discuss my wishes with a plastic surgeon and I emphasized that I wanted a natural, but visible result. I don’t currently remember how many CCs per implant, but I know that I went from a small A cup to a full C cup. Round, smooth, silicone implants were used and they were placed over the muscle via an infra mammary incision (in the breast crease).
**December 2016, about three months post-op:
I was very happy with my results - finally I had full, beautiful breasts that felt great and I could finally shop for lingerie and actually feel confident in many tops, bras, etc. that I was previously too ashamed to wear or that felt uncomfortable.
**May/June 2017: About nine months post-op, I noticed that my right breast in particular was starting to feel a bit hardened and not as supple as it once did. Of course I was immediately concerned about capsular contracture and started to take vitamin E and massaged away regularly, without success. Since it still looked decent though (despite it not being as nice as it first was), I put off those negative thoughts and figured that I was perhaps being too perfectionistic and reaching for something unattainable. So I pushed those thoughts aside and still enjoyed my bigger breasts and all of their perks.
**Flash forward to a bit over a year post-op, like December2017/January2018:
Not only is my right breast still hardened (I know that this can’t be fixed without replacing the implant and having a capsulectomy), but I began to notice a sort of squishy and out of place “bump” on the top pole of my left breast. It wasn’t and isn’t painful, but of course I was immediately concerned and made a doctor’s appointment. My PCM referred me to a specialist and she then palpated my breast and did an ultrasound exam and found nothing. She said both implants look great and the bump was attributed to my cycle. My mind was put at ease from a health perspective, but I was disappointed from an esthetic/cosmetic point of view.
Today (June 2018):
The bump/lump or whatever one might call it is still there and has become more visible and prominent in my opinion. My husband says it’s barely noticeable (only in a certain light/position), but of course I still feel and see it. My left breast has also started to not feel as soft and ‘real’ as it initially did, so there’s that. Furthermore, I feel as though muscle tightness and pain in the thoracic portion of my back (plus trapezius, chest and neck) has worsened over time after augmentation. Everything feels tighter and tenser, which I know a lot of docs will attribute to anxiety etc., but I swear it was never this bad before surgery.
All in all, I am a saddened and disappointed that this has taken a turn in the wrong direction fairly quickly and I am now considering new options.
A few days ago I posted a question to the docs on here, to see what they think and whether or not an explant and revision with smaller implants would likely be a good option. A few have answered and said that this could work, perhaps in conjunction with fat transfer to the top pole and implant placement under the muscle (the top of my chest is quite bony, so there’s not that nice, beautiful, and natural looking slope to the nipples).
Anyway, this gave me the idea to research fat transfer as the sole source of augmentation and it appears that the reviews are quite mixed. You can see in my pre-op photos that there wasn’t much there. I am willing to go down in size from what it is now, that’s not a problem. Most of all, I’d like a natural feeling again with comfortable and not misshapen breasts. If I ended up a nice B cup after removal of my implants, plus a fat transfer, I’d be extremely happy.
Perhaps I went overboard with these implants and my body is simply not made to accommodate this additional weight plus the foreign material. I know that my breasts don’t look terrible now and that nothing is perfect - but seeing how they have developed over the course of *not even* two years, how I am in more pain and how they feel and at times look unnatural to me personally, I am strongly considering getting an explant with a fat transfer and hoping that this will leave me with some fat retention to the point where I end up a nice B cup. I’m just not sure I want to do implants again -.-
So, sorry for the “book”, but I wanted to go into detail about my situation. As for photos, I have the before and three months post-op in black and white and the color photos I took today. See how it looks implanted and how they sit unnaturally, particularly in certain positions. To some they may not look that bad and they’re not - but to me the difference is pretty stark. They’re harder, the scar is more visible now (like the implants have travelled upwards) and the slope is just unnatural. I’m also a bit afraid of already needing a lift after they’re taken out in conjunction with fat transfer, because my tissue stretched quite a bit to accommodate the implants.
Thoughts, ideas, reviews or experiences with fat transfer??

I have my BA set for August, so I am new to all of this and I do not have an informed opinion. After reading your post, I felt compelled to tell you that your photos are beautiful and you look fantastic :) I hope everything works out for you.
Well thank you for your kind words :) They don’t look that bad, they just feel off. I just wish I would’ve gone a bit smaller and perhaps under the muscle OR with a fat transfer right off the bat. Are you excited for your surgery? I wish you all the best!

I was a small A cup and due to this I was never truly happy with the appearance of my body. I had thought about getting a breast augmentation done for years but didn't have the funds and final decision until this year. I decided to go to Dr. Jack Peterson in Topeka and had a wonderful experience.
