Three months further, the swelling has come down, but i can say that i am not happy with the results.

Eventhough they fixed my bite, and my profile is more balanced, there is now a lot assymetry in my face. Upper teeth are off the midline, canted incisors, the arch is not symmetrical because of the assymetrical SARPE and unbalanced teeth movement between both sides.

I am going to discuss the assymetry with my orthodontist and surgeon and what the possibilities are from here, but i expect that they are not going to do anything for me.

I am reconsidering another round of orthodontics and another BSSO surgery, but i don't know if i can make this happen. It would probably take a completely new team. Besides this, it will probably not get covered by insurance since there are no medical problems.

What i think that needs to happen is that all my upper teeth need to be moved 2mm to the right. Then the upper jaw would need to be realigned to correct the midline of the incisors by moving the upper jaw 2mm to the left. Then the movement in the lower jaw would need to be changed to match the position of the upper teeth. Because they rotated both the lower and upper jaw, the rotation would need to be reversed to fix the assymetry.

Hi Donny, I’ve had exactly the same experience as you - Lower jaw reduced too much and not liking the look. It’s been a good few years but could you give an update on how your experience has been.

Lower Jaw Too Small After Jaw Surgery

2 and a half years ago, i started the trajectory to fix my bite. My orthontist immediately told me that my upper jaw was too narrow and i had a protruding lower jaw (class 3 malocclusion).
So i had SARPE surgery which resulted in 9mm asymetrical expansion. It then took two years to align my teeth with orthodontics.
On 27 august 2018 i had double surgery. Everything went really well and it only took 2 and a half hours. I'm now 2 weeks post op. I did have some numbness but it it is already getting better. I only worry a bit about some numbness in my tongue.
I still have residual swelling, but you can only see it when you look at my face with a 45 degree angle. Most is visible at my cheeks
The asymmetric upper jaw has now been aligned, but my lower jaw has been shortened so much, that i do not like my facial profile anymore, even though my upper en lower teeth have been properly aligned. (my lower teeth are touching the back of my frontal incisors).
My nose is also kinked because they moved my upper jaw forward and upward. I was never told that there was a chance that the shape of my nose could change as a side effect.

I think you look great!!! Two weeks post op is not ur final result. I had double jaw surgery 7 months ago and I still feel like my face is changing. Also lower jaw will look too far back (particularly the chin) at first due to swelling. That will change in a few weeks or months. I remember thinking my chin looked recessed the first couple of weeks but that changed as swelling decreased.

I just underwent double jaw surgery 11 days ago, August 31st, and I keep overanalyzing the early results I'm seeing. I also feel like my lower jaw isn't forward enough, but swelling definitely could still be messing up the visual balance. Also, my nose also had some unforseen complications, so my septum almost seems out of line at the base. I guess all we can do is wait and see how our swelling goes down.

I think you are right. It will get better when the swelling is gone.
However, i'm pretty sure that in the end, there will be minor problems that you can recognize but others not.

Something that i worry more about now, is that because of 2 previously extracted upper teeth, my molars are not aligned anymore. i guess i just have to trust the orthodontist and hope that i can still chew in the end. I'm going to discuss this next week.

Don’t worry your lower jaw will be longer after 5 to 6 months. You are just 2 weeks after the surgery. I experienced that too thinking my Lower jaw was too short and small. But after agonising 5 months of waiting I could see my lower jaw developing and more prominent now. Phew!

it looks so good to me. they did this to me in an opposite way and i look so manly. i love how your jaw is back, i think it's way more attractive, if you don't mind me saying.

I find that very strange, that they would even *conceive* to bring your lower jaw back. What you should have had done was a Le Fort 1 Osteotomy, or a maxillary advancement.

I underwent the same procedures, and actually had the SARPE and maxillary advancement (same jaw) in the one procedure.

Moving forward, you should consider the possibility of having a BSSO - a bi-saggital split osteotomy. It involves both advancement of the top and bottom jaw in the same procedure.
