I had a lipoma on my my forehead which I first tried to have my dermatologist remove. She told me it was a routine outpatient procedure, until she cut into my skin and could only remove a small portion of it. It ended up being transmuscular - meaning the majority of it lay underneath the muscles in my forehead. Although it was a complete disaster for me, she charged me full price as if she had removed the lipoma, when in fact she had left me still lumpy and newly scarred. Curse you, Dr. Alison Stallings in Tarrytown, New York! I then contacted Dr. Jacobs, a local plastic surgeon who very competently removed the lipoma with just a little extra local anesthetic, because I did feel some sharpness from the scalpel. He also agreed to do the procedure without using my insurance, for only $500 - which was about 40% of what I was charged after insurance for my dermatologist's botched operation. End result is that I was happy the second time around.

Dr. Jacobs was a breath of fresh air after dealing with Dr. Stallings. When a doctor cuts into your skin to remove things, they had better be confident and adept, which Dr. Jacobs certainly was.
